OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight training at Hervey Bay, the student identified RWY 29 was the active RWY. As the instructor was unsure a radio call was made to the RPT aircraft on the ground for its intentions and which RWY it was going to use. The instructor did not hear a response, therefore allowed the student to join mid field crosswind after descending on the dead side of RWY 29. The student turned to close to the RWY not giving themselves space for the base leg which led them to not having space for finals (too high). After turning for finals the instructor looked at the wind sock and realising it was a tailwind, took the controls to demonstrate that even with full flap they would not make it. Eventually they proceeded to conduct a go round, making a go around call and departing the circuit back to Caloundra. Upon landing at Caloundra, it was determined the captain of the RPT aircraft had raised concerns about the communications.
Further investigation by the CFI determined that the student had decreased the volume on the radios, which was not apparent to the Instructor and caused miscommunication and the inability to hear other calls correctly at Hervey Bay. This was corrected after departure and the rest of the flight the instructor was able to hear radio calls. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was directed to join circuit for landing with a 15 kt crosswind. The pilot completed touch down successfully but when the nose wheel contacted the runway the aircraft veered off toward right (into wind) and came to a stop on the grass. The pilot then added power and taxied off the RWY successfully. Actions were undertaken internally with the flight school to address this RLOC incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On completion of an annual inspection the aircraft was taken up for two circuits. Upon landing on RWY 05 with a 20 knot northerly wind the pilot ballooned the landing and bounced on the second landing. The aircraft was taxied back to the hangar and found the front nose spat damaged. There was no other damage to the aircraft. The damaged spat was removed. Pilot has recognized the need for recency and currency in the prevailing conditions. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During 100hr inspection the stabilator bearing was found to be worn. The stabilators were removed from the aircraft for access to replace the bearings. Once removed a crack was very noticeable on the RH empennage skin just forward of the stabilator attach fitting. A closer inspection found the crack and one rivet found loose, the crack seemed to be coming from the skin strengthening bend relief hole just forward of the rear bulkhead.
Investigation: Apparent defect in the manufacture of the longitudinal stiffener at the point of attachment with the vertical stabilizer, cracking started at the radius of a bend in the metal that was not de-burred at manufacture. Cracking amplified by worn bearings in the stabilator. Manufacturer notified of defect report. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a dual training flight, a distinct metallic rattling noise was identified. Immediate action drills of carburettor, engine temps and fuel quantities were checked with no apparent change in noise and rattling. Power was reduced to 3800rpm with the sound still evident from the engine. The aircraft was flown by the instructor slowly at approx. 60 knots back to YSBK. The aircraft landed safely under its own power. With the metallic sound abating when the power was reduced below 3000 rpms. After leaving the RWY, the aircraft taxied under its own power back to its hangar, where it was handed back to engineering staff. Upon investigation it was discovered that an insect was blocking the Pitot probe preventing airspeed indication. Steering post application of the brakes was hampered by an imbalance in the adjustment of the brake pads |

Outcome: RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot assumed a specific runway was the intended one in use and did not follow ATC instructions given a number of times, PIC was qualified correctly with CASA qualifications to operate in the aircraft, but was unfamiliar with airspace. Pilot was participating in controlled aerodrome airspace refresher training. Lack of compliance with ATC directions resulted in increased workload for controller on duty and delay to other aircraft in the area.
The pilot will continue the controlled airspace training to ensure no repeat event occurs. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While flying from Bendigo to Albury on a regular flight route at 5400 AMSL, the Brumby LSA pilot observed an approaching Cessna aircraft on a reciprocal heading and closing. Immediate avoidance action was taken by the Brumby pilot and both aircraft passed with in 100-200 m of each other.
Investigations conducted revealed the Brumby pilot was flying correct hemispherical heights and monitoring both appropriate Melbourne centre and CTAF frequencies for the sector in the Waranga basin near Rushworth and subsequently took appropriate action to avoid collision.
Further identification of the Cessna aircraft was not obtained and subsequent to this event the pilot has fitted additional secondary surveillance equipment to his aircraft. The pilot further reported that as the other aircraft passed he clearly noticed the pilot of the Cessna was focused inside their cockpit.
This incident highlights the need for constant and effective visual scanning as part of Threat and Error Management (TEM) and adherence to flights at hemispherical level references above 5000 AMSL but also wherever possible in lower level uncontrolled airspace. |

Investigations were only able to identify the RAAus pilot and a discussion on alerted see and avoid was undertaken. It is recommended pilots read the August Sport Pilot magazine Safety Insert on practical application of "alerted see and avoid principles and circuit planning at non towered aerodromes". |

The aircraft was on late final 20-30ft, when the aircraft stalled. The left wing dropped resulting in very hard landing on left main wheel followed by the nose coming down heavily, breaking nose wheel and flipping the aircraft upside-down.
The pilot was uninjured, but the aircraft was badly damaged and will be rebuilt in due course. The pilot also conducted remedial training in short field operations in this style of aircraft |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Pilot was coming in on second attempt to land the aircraft. While on final flare above the threshold, the pilot was distracted by the position of the flare and wandered off the centreline of the RWY to the verge (while transitioning to the ground). An observer (another pilot) midfield on the RWY had to take several steps backwards to ensure separation with the aircraft as it moved left off the RWY.
Full power was applied as the pilot identified the poor control over maintaining the centreline and a go around was initiated. The pilot then conducted a final circuit and conducted and uneventful landing. No damage to the aircraft, pilot or bystander occurred. The pilot will undertake additional training on the type, which is new to the pilot, prior to further flight. |
