OCCURRENCE DETAILS SUBMITTED TO RAAUS: After touchdown as the aircraft was slowing down, the nose wheel began to shimmy. The nose wheel movement became quite rapid and increased in force. The nose wheel leg collapsed, causing the propeller to hit the ground. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On take-off climber to approx. 200ft - an extreme vibration developed. The pilot slowed but had to keep going to clear fence, roads and trees. The propeller departed with a loud bang, hitting the front cowl and the pilot was able to land in a paddock with no further damage to aircraft.
On inspecting the propeller flange there was a hole drilled and threaded for locking the propeller flange stub to the crankshaft. This caused a fatigue point where it tore leaving half the thread on either piece. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The CFI was taxiing when they heard on CTAF another flight instructor trying to make contact with a Cessna that was cutting them off on the circuit without making any calls. The CFI then witnessed the Cessna was cut off the other aircraft on the circuit by descending on the active downwind leg in front of them and turning to base and final right next to the aircraft doing circuits. As the CFI held short of the RWY they could see the Cessna making left and right turns below 500 feet AAL but at a greater distance than 500 meters of the threshold. The CFI made the call for entering and backtracking on RWY 17 after the aircraft on the circuits landed on RWY 17. The CFI saw the Cessna going left and right on their windscreen at least three times. The Cessna then turned to final while both aircraft were still on the RWY. The CFI had to take their aircraft outside the RWY and notify the instructor on RWY 17 to expedite their taxi to be clear as the Cessna had just landed while both the aircraft were on the RWY. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft lined up and took off at full power on RWY 32. Towards end of the RWY at approx. 300-400ft AGL the engine lost power. The pilot turned and landed safely on RWY 10.
Outcome: Causal factor was fuel starvation, due to incorrect fuel filter part number installation. An aftermarket fuel filter that looks similar was at some stage preceding the incident installed. During operation an internal screw fitting had loosened allowing the filter to partially block off the outlet port of the filter assembly body, allowing enough fuel for low powered operation but an inadequate supply for high power. With a minimal amount of fuel being stored in the carburettor bowl, the supply issue would not be evident until approximately 15-30 seconds after application of full power. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The flight was for to allow the pilot to take their father up for their first passenger flight. There were some showers in the area so the pilot was briefed by the CFI on how much flight time was available and where to operate to remain clear of weather. The weather started to move in so the pilot decided to land, but rushed the landing and didn’t flare enough causing a prop strike.
A number of human and decision making factors led to the outcome of this flight and pilots are reminded of the importance of early decision when weather is a factor, along with using standard landing techniques. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During solo flight training the pilot was attempting to conduct a glide approach but did not confirm their sequenced position in circuit relative to the twin engine aircraft turning base as number 1 position. As a result the student pilot breached sequencing. The tower advised the student to conduct a missed approach which was successfully completed. Ongoing work is being undertaken between the flight school and Airservices to ensure robust CTR procedures are adhered to for safety and compliance. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Prior to landing the pilot was not aware of the strip being excessively wet. Prior permission to use the location had been obtained from the strip owner, however a visual inspection had not been conducted. As the aircraft landed the wheel became bogged and the aircraft veered off the shoulder of the RWY. As the aircraft came to a stop the starboard wing tip made contact with ground.
Pilots are reminded private strips are not required to meet landing requirements of approved airports. A prior visual inspection or test drive using a vehicle is recommended prior to use, particularly after heavy rain. Refer to CAAP 92-1 section 9 for assessment of surfaces of a landing area. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On landing, the landing gear spreader bar failed resulting in the left wing tip hitting the ground. 30-40 metres later the port wheel rotated under the air craft and the aircraft flipped over forward and stopped.
Information from the owner identifies a weakness in the spreader bar design as the reason for the failure The owner is intending on a redesign to prevent this occurring in the future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On touchdown the aircraft veered to the left and no corrective action was taken until the aircraft had left the sealed surface. Right rudder was applied and the sealed surface was regained. The student made a normal exit from the runway by a taxiway and returned to the hanger area.
The CFI conducted additional check circuits to ensure the student was at solo standard to manage the aircraft.
RAAus created an article for Sport Pilot July edition relative to gaining forecasts for winds even when flying locally to ensure an understanding of prevailing conditions even if intending to only conduct circuits. |

This turn was started too late and while the pilot applied the brakes, this was too late to prevent the aircraft from striking a fibreglass cone with the prop.
The pilot noted for future parking the aircraft would be shut down and moved into position by hand |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Investigation with the PIC identified that taxiing clearance limits were maintained during taxi but due to concerns over a nearby starting jet aircraft the pilot maneuvered further to avoid risk. In doing so and whilst looking over his shoulder he inadvertently allowed aircraft to taxi too close to the parked RV6 resulting in an undetected impact. Subsequent independent inspections have been conducted on both aircraft and respective insurance companies are dealing with the matter.
The pilot has reviewed his taxing practices and situational awareness as a result of this accident and will remain clear of this apron area during future events whilst taxiing. |
