OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student was on an approved solo training navigation exercise. During the navex the pilot became unsure of their position and whilst making track corrections inadvertently climbed into controlled airspace.
An investigation was undertaken with both the pilot and supervising instructor with a focus on assessment standards and navigational competency and a review of remedial training actions was undertaken. The pilot undertook appropriate retraining and was subsequently assessed as competent by an independent examiner for issue of cross country endorsement privileges. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On a dual training Navex the pilot did not recognise the increased sink rate during the final stage of the approach and despite late intervention by the instructor the aircraft struck the tail as a missed approach was commanded by the instructor. Contributing factors were identified as a reclined seat position changing the pilots normal eye line referencing and delayed intervention by the instructor. It was also noted this was the first dual flight with the student instructor combination and the landing was not at the schools normal operating location.
Schools and Instructors are urged to attend and review the resources currently being provided by RAAus at Professional Development seminars on "Control Management for Instructors" Effective intervention strategies are vital to safe training outcomes while preserving maximum learning opportunities for pilots under training. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During approved training circuits at Bankstown whilst under a control clearance the RAAus registered Bristell and a C152 lost visual reference in circuit as the C152 conducted a simulated glide approach from crosswind resulting in the Bristell overtaking the Cessna in the circuit.
Both aircraft continued operations without further incident. The primary cause was loss of visual awareness of the C152 by the instructor in the Bristell. Contributing factors were a lack of communications by the controller in advising an approved non standard circuit had been granted to the C152 when the Bristell was asked to follow the preceding C152.
RAAus continues to recommend that training operators undertake joint aerodrome safety meetings with all affected stakeholders to raise awareness of issues such as these and devise procedures and agreements to assist safe and harmonious operations at all training aerodromes. These meetings and devised outcomes can form part of any schools risk management plans and Safety Management System. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During take-off the airspeed indicator was slow to show speed increase. After take-off the airspeed indicator kept increasing through to >100kts even though the aircraft was in climb/low airspeed. The pilot deduced the pitot head must have a blockage. On downwind the pilot requested a full stop, tower asked if emergency services were required which was declined. The aircraft taxied back to base, where an engineer examined the pitot head and cleaned it, dried mud was found inside the pitot tube. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Bird strike. The bird was not sighted prior to impact nor heard on impact, however minor vibrations were felt in flight. On inspection after landing smear marks consistent with a bird strike were found along with minor cosmetic chips on the propeller no larger than 4mm.
RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft joined downwind leg of circuit for a full stop on RWY03. The pilot completed pre landing checks, turned onto base and reduced throttle to 3000 rpm, lowered airspeed and set flaps to stage 1. As the pilot turned onto final heard a large popping noise to their left, looked across and saw the vent was missing, the door then flew open started flapping with the Perspex damaged. The pilot ensured aircraft was stable, reached out and pulled damaged door back down and held it in place and completed landing. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the taxiing phase of the flight the pilot sustained minor wheel fairing damage due to the obstruction of forward vision from the sun and impacting a gable marker on the grass runway. The pilot will in future, take a wider taxiway and factor is the obstruction of vision from the sun that can occur from utilising the grass runway at a given time. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: CFI requested engine out glide approach for an RPC test. Tower advised glide approved. The engine was shut down once landing clearance was received and aircraft landed safely. The CFI then restarted engine once on the ground and taxied to park. Tower request to be contacted in regards to this, so the CFI contacted the tower. The tower advised that they would prefer our engine out glide approaches not to be done in class D airspace.
OUTCOME: A review of training procedures was conducted with the flight school and the communications with ATC at the time of the occurrence were also reviewed in conjunction with the CFI and the duty Airservices controller.
Whilst section 3.02 of the RAAus Operations Manual permits the practice of "engine off" operations under controlled circumstances by a CFI or higher approval holder for emergency training, this was not clearly understood in the communications by the ATC controller.The CFI has modified his training practices to exclude this operation at a controlled aerodromes in future. |

The FTS has implemented a policy of a minimum height of 200'AGL to conduct the exercise and to take account of the student workload and ability to manage this exercise after a complete circuit session. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While flying from Derby WA to Bundaberg QLD to get routine maintenance done a Jabiru, by the pilots’ calculations there was enough fuel to reach Bundaberg but there was concern about going into the reserve. The pilot made the decision to divert to Mundubbera which was approx. 12nm W of Auburn State Forrest where the aircraft eventually completed an emergency landing.
The pilot side fuel tank on the wing gauge still showed a good amount of fuel however the PAX side tank viewed empty on the wing gauge The engine started to splutter and shut down. With carby heat on and pushing throttle in and out the pilot managed to restart it but this only lasted momentarily. The pilot had approx. 45- 60 secs of flight left when they saw what was thought to be a road through the forest however as they got closer it was a single phase high tension power line with a service road under it. The decision was made to land in the forest approx. 30-40m away from power lines with the sole intention of putting the plane between the trees to take the wings off and slow the speed of the decent/landing. As the aircraft came into the trees the pilot had to make two slight deviations, one to the left and one to the right, the main two trees the aircraft hit were not directly opposite each other. The aircraft then clipped some smaller trees, the pilot with the stick full back in the stall position tried to belly it into the trees.
Fuel was running from the wings once the aircraft came to rest. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While warming the engine prior to completing an oil change and due to the pilot's agitated state of mind as a result of issues encountered during setting up the aircraft, the throttle was selected to full power and the aircraft overrode the brake system. The aircraft moved forward and struck a person providing assistance to the pilot, resulting in minor injuries.
Pilots are reminded of the importance of human factors such as distraction and frustration affecting decision making and aircraft management.
The pilot is to be commended for submitting the report. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the second circuit of a solo check lesson, a fuel cap was identified by another aircraft on the runway, unsure of whether it belonged to our aircraft we conducted an immediate full stop. On inspection after shutdown, the fuel cap was identified to be missing on the left hand wing.
The fuel caps were inspected visually on the ground in the initial pre-flight of the aircraft and appeared to be secured. However an inspection by hand was not carried out which lead to this incident. The instructor acknowledges that all inspections must not only be visual but require to be inspected by hand. This not only pertains to fuel caps, this extends to all components of the aircraft as a preventative measure to such an incident. |
