OCCURRENCE SUBMITTED TO DETAILS RAAUS: The instructor was conducting a stall exercise when afterwards the engine started to run rough. The rough running of engine commenced following a stall exercise with nose high attitude, power idle and small nose drop and moderate power was used to recover. Normal power continued with nil mag drops and all instruments were indicating normal operations. Upon landing, the engine was runup to 2000RPM however the right magneto had a drop of 400RPM. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: DEFECT: Counter weights were added to the original elevator as a manufacturers mod to improve control in turbulent conditions. The original elevator structure design did not allow for the addition on balance weights to the elevator. The extra load combined with prop wash has cracked the spar web angle at both outboard positions. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: While climbing right crosswind on RWY 13 at Gawler (YGAW), the pilot of an RAAus registered aircraft saw a VH registered aircraft crossing below them flying contra (LH) to downwind circuit. They called out in the blind to see who was flying a contra circuit, and the pilot of the VH registered aircraft advised they were turning final on 13, while they were in fact turning final onto 31. When the pilot of the RAAus registered aircraft advised them of their error, they called to say they were departing overhead the field, even though they would have only been around 500' AGL and heading into an incoming aircraft that was joining the circuit. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The pilot was conducting circuits at YNRM when on the 5th take-off, the engine lost partial power on upwind. The pilot reduced RPM/throttle and engine maintained power. The pilot maintained glide distance from the runway, completed the circuit and landed without issue. The aircraft was taxied to the hangar. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: After two earlier flights in the day without incident, the instructor was with a student conducting a pre-solo check flight in the YJIN circuit, off RWY 30, in a Jabiru.
Weather conditions were good with a 5-10 kt headwind. After the first Touch and Go, they commenced the climb out upwind, flaps up at 300 feet. At 400 feet, the engine started to miss its rhythm and lose power. The instructor immediately took control and used the failing power to turn back to the runway. On completing the turn, the engine failed completely. The instructor determined that they had sufficient height to glide back to the runway, conducting a safe, flapless landing in the undershoot and bringing the aircraft to a halt on the runway. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The instructor was on their last flight of the day. For their final flight, they went with a student for circuits.
The student completed the preflight inspection. Afterwards, they boarded the aircraft and began their flight. They taxied from the aeroclub via Taxiway Juliet, then Alpha, to holding point Alpha 1, where they conducted run-ups without issue.
After receiving clearance from the tower, they entered runway 31 for take-off. As per their brief, the instructor took control to demonstrate a full standard circuit for the student. The instructor applied full power, and the aircraft began to accelerate normally. Upon reaching the appropriate rotation speed, the instructor lifted the nose off the ground, and the aircraft took off as expected.
However, shortly after reaching 200 feet, the engine started running very rough and lost power, making it impossible to continue climbing. The instructor reduced power and tried adding it again, but the engine continued to perform poorly. The instructor switched the fuel pump off and on, in an attempt to correct the issue, but this had no effect.
At this point, the student asked what the problem was, and they instructor explained that they were not sure but that the engine was malfunctioning and they would need to perform an emergency landing. The instructor immediately contacted the tower, explained the situation, and requested priority handling as they were not sure if the engine would provide enough power to complete the circuit and land on runway 31. The tower granted them full priority.
The instructor managed to execute a low left-hand circuit, staying close to the runway and ready to land on either runway 13 or even on Taxiway F if necessary. As they approached the runway, they knew we would be able to make it to runway 31, so they cut the power and transitioned the aircraft into a glide for the close base turn.
The instructor made a safe landing on runway 31 and was able to vacate the runway via Alpha 2. The instructor then taxied back to the aeroclub without further incident. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: DEFECT: At 50 hourly engine maintenance service/inspection, engine bay was found to have a Ryco Z15 plastic fuel filter, with only 2.8mm inlet and outlet. This area within engine bay should not have a meltable/flammable fuel filter, and one with this size of fuel flow restriction. Fuel filter was changed to a metal fuel filter with 8mm inlet and outlet, and correct fuel hoses to match. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The pilot was flying from Wentworth to Swan Hill. The pilot arrived over Swan Hill Aerodrome sometime in the late morning and joined the circuit intending to land on the gravel runway 22. As the aircraft touched down, the pilot felt an intense vibration and noise from the left-hand undercarriage. The pilot immediately applied full power and aborted their landing. The pilot broadcasted a Pan-Pan call on the CTAF frequency. After reaching a safe altitude, the pilot checked to see if all their controls were functional, all of which were working normally. The pilot could not see the undercarriage from their position but concluded that the left-hand undercarriage leg, the wheel or the tyre was damaged.
The pilot then decided to complete an emergency landing on Runway 22 and broadcast their intentions on the CTAF Frequency. The pilot treated the landing as normal except that they endeavoured to keep the left wheel off the ground as long as possible by using the flight controls. As speed was lost, the left wheel eventually made contact with the ground. The aircraft slewed to the left. The pilot tried to control the aircraft but when it came off the gravel onto the grassed area, the nose impacted the grassed area shattering the propeller. Once the aircraft came to rest, the pilot turned off all the switches, removed the key and vacated the aircraft as quickly as possible. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: DEFECT: Following a successful run up and departure on a training flight from YHEC, an EFIS alarm state indicated over temp on EGT 4 shortly followed by a flashing LAN 1 alarm. The instructor took over control and positioned the aircraft for a precautionary landing and did initial troubleshooting checks in circuit. The flight was terminated and the fault continued to present above 4000 rpm on run ups. The sortie was terminated and the aircraft MR endorsed with defect and L2 advised.
A post incident inspection revealed EGT no.4 sensor was faulty and was replaced as well as EGT no. 2 sensor. The L2 advised this does occur regularly as the sensors burn out over time. The fault was cleared and aircraft returned to service. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: DEFECT: Whilst carrying out Annual inspection of Foxbat aircraft, the 4 EGT probe mountings were found to be loose and leaking exhaust badly into engine bay. 1/8 inch probes had been placed into 5/16 inch mount holes, with excess clearance and leakage. Engine was a Rotax 912ULS. In the same engine bay, mounted on firewall was a plastic case fuel filter. |
