OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a dual training flight, on final approach the aircraft was below the appropriate profile which led to an excessive flare and ballooning. The decision to go around was left later than expected by the student, resulting in low airspeed and minimum control despite application of full power. A right crosswind drifted the aircraft left, the left wing dropped and came in contact with the ground. Full control was then restored and a go around was performed.
This accident represents another Loss of Control (LOC) event in the low airspeed environment. This is an area where instructor control management, early recognition and responses are critical to prevent unsafe flight outcomes. Focus on these areas in dual training will ensure robust situational awareness and decision making are developed by pilots in their early development.
RAAus reviewed Instructor responsibilities and intervention strategies with the school CFI and RAAus resources were provided including "Control Management for Instructors" presentation. The CFI will incorporate these resources into development training for all instructors to reduce further training incidents. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During solo consolidation, after a successful touch and go and a full stop landing, the student lost directional control and exited the runway onto the grass. They maintained control and got back onto the runway to then exit to a Taxiway.
RAAus has investigated this and other accidents reported at this school and determined the CFI has enacted appropriate remedial training and solo supervision processes in an effort to prevent any future recurrences. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After approx. 30mins of flying the engine suddenly began to vibrate and lost partial power, application of carb heat and other troubleshoot checks failed to rectify the problem. A turn was made towards the nearby airfield and forced landing opportunities assessed. After about 30 secs engine operation returned to normal. The pilot positioned for a high glide approach at the airfield. The engine again ran rough with some RPM loss but continued to produce appreciable power. From CHT indications it appeared that number 3 cylinder was not firing. This was confirmed after landing by physically feeling all cylinder heads and noting that No 3 was much cooler than the others. The cause of the failure was not readily evident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting dual circuits at YBSU on RWY 18 during CTAF hours, following a touch and go by the Sling, a SAAB 340 broadcast that it was taxiing for departure. The Sling offered to extend left downwind RWY 18 to enable the SAAB to backtrack, which was acknowledged by the SAAB. Further calls were made on the CTAF by the SAAB when entering and backtracking, the Sling PIC believed that the SAAB intended to use RWY 18. The Sling continued downwind and the Instructor was unable to keep the SAAB in sight during backtrack due to restricted visibility to the left when sitting in the right-hand seat. The Instructor advised the student to continue downwind and turn base as appropriate to maintain separation from the SAAB.
The Sling commenced a turn onto base at approximately 2nm from the aerodrome reference point. As the aircraft was rolled into the base turn the Instructor conducted a lookout for the SAAB and observed it commencing a take-off roll on RWY 36 with a tail wind, simultaneously the SAAB made a broadcast rolling for a left turn. The Sling Instructor informed the student to make a right turn away from the airfield and to continue the right turn to establish early right downwind RWY 36. The student promptly complied and the Instructor did not need to take over control of the aircraft. As the aircraft turned through HDG 045 the PIC sighted the SAAB rotating off RWY 36 and determined there was no further threat. At no time did the Sling cross the extended centreline of RWY 36. The Sling continued downwind RWY 36 until passing abeam SAAB at 1000ft AMSL, then re-joined final for RWY 18 and completed a normal landing. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot encroached on the controlled airspace of Amberley Air base by flying at 3300 AMSL. The pilot misread the chart and flew at 3500' instead of 2500'. On becoming aware of the mistake they immediately turned South (away from Amberley) and descended to the correct level. On landing the pilot contacted Amberley Duty Officer and discussed and provided their details to them for further reference. The pilot was also debriefed by the local CFI to ensure no repeat of the error. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot had flown the aircraft for 50mins conducting circuits and had previously performed 5 touch and go landings without any issues or concerns. On the 6th circuit landing the nose wheel collapsed.
The Aeroprakt factory has recently revised the design of the nose leg fork pivot bracket to reduce the likelihood of seizure. New aircraft orders include the revised design and are supplying these for existing aircraft as and when requested by owners/maintainers. However, the maintenance requirements and pre-flight inspections remain the same. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After a 1.3 hr flight, the aircraft was approx. 2 NM from the airstrip at about 1200ft when the engine canopy lifted on the starboard side and started flapping violently. It then parted from the aircraft. The pilot made an immediate radio call on CTAF and joined the circuit downwind & landed without incident. The engine canopy was later recovered from a nearby paddock.
Determined Outcome: Pilot determined that the engine cowl was incorrectly fastened prior to the flight, allowing separation under wind load. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting initial flight test after aircraft repair including engine bulk strip and rebuild to zero time by Jabiru. The aircraft departed the RWY and the engine started to lose RPM and make clunking noise at approx. 300-400ft. The PIC immediately turned downwind for a close circuit and identified outfield landing options. The engine continued to lose RPM and oil pressure but the PIC was able to maintain 600-700ft AGL and perform a close circuit return for RWY 22. The engine did not stop but was only holding approx. 1300 rpm at wide open throttle. The aircraft was landed uneventfully. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot reported that while practicing high speed taxiing at the local aerodrome, they applied the brakes which at first worked properly but then failed completely. The aircraft had too much speed to stop before the end of the runway, so the pilot attempted to turn to the left to veer onto runway 36. The pilot managed to make the turn but with too much speed for the terrain. The undercarriage failed and this caused a prop strike.
This was originally reported as an aircraft defect however investigation revealed the accident was part of a an unapproved student flight operation in their own aircraft without CFI supervision or awareness. The investigation identified a number of non compliant flights had been completed by the student pilot prior to this accident .
A suspension of flight privileges was applied and the pilot agreed to conduct operations under a Deed of Agreement confirming their intention to operate in the future in accordance with RAAus Operations Manual requirements.
RAAus reminds members that it is a significant safety issue to operate an aircraft without undertaking sufficient training. Actions such as these are a breach of RAAus requirements and CASA regulations with potentially serious consequences including potential fines, criminal convictions or imprisonment.
RAAus will continue to liaise with the CFI and monitor the students behavior and development for compliance as progression towards a Pilot Certificate is completed. |
