OCCURRENCE DETAILS SUBMITTED TO RAAUS: After an hour dual check flight, the student was sent for a first solo circuit. The student lost directional control during take-off roll, departed the runway strip by several metres before cutting power and coming to a stop. The student subsequently taxied back to flight school without damage to aircraft, person or aerodrome property.
The Senior Instructor supervising the student revised management of loss of control events with the student and the CFI conducted re-evaluation with the student as part of the schools training assurance. Latent fatigue due to the pilots work commitments was also identified as a contributing factor. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Practicing glide approach under training conditions with a student RWY 12, the approach was flown high but on glide speed for the aircraft with stable approach along the final leg. The aircraft experienced some sheer late on final but it was thought to have had sufficient airspeed to recover with change in attitude alone. The aircraft continued to sink and landed both on the main gear and the tail skid simultaneously on RWY 12. The pilot immediately aborted the touch and go, amended to a full stop and returned the aircraft to the hangar for shut down and inspection. The only damage identified was a broken tailskid. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft lost directional control on take-off and ran off the RWY and made contact with a concrete drain. The impact cracked the right hand undercarriage and snapped the nose wheel off when it came to a stop damaging the front of the aircraft.An investigation with the pilot was conducted and the primary cause related to foot misplacement on the pedals where one foot slipped during take off roll. The owner pilot is discussing improvements with grip on the metal surface and will ensure positive control and traction of the rudder bars in future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After completing an hour and a half flight, the aircraft had just landed when it started veering to the left. The pilot applied hard right rudder but it continued to run off the RWY. The pilot applied some power and lifted the left wing and wheel to clear a landing strip light, and after clearing this obstacle, continued to apply hard right rudder, however the plane continued to turn left. Brakes were applied to no avail, the aircraft ran off the strip and through a barb wire fence, coming to a standstill near some trees. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: As the aircraft landed on runway 22 the tyre blew out resulting in the aircraft running off the runway narrowly missing the airport fence.
The pilot confirmed they were practicing cross winds and may have landed harder then intended, causing the tyre to blow.
The pilot has subsequently completed additional training in the USA on the aircraft type and will complete a BFR in Australia with retraining if or as required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Report received of an accident occurring while flight training was being conducted at a private airstrip at Abergowrie. Student and instructor were uninjured, however the aircraft was destroyed.
RAAus investigations revealed the initial report was inaccurate and appropriate remedial training for the student and disciplinary actions in relation to the CFI were undertaken as a result.
RAAus reminds members of reporting obligations under Section 2.14 of the RAAus Operations Manual in addition to the Transport Safety Investigation Act 2003, to report all accidents and incidents accurately. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Instructor and student had flown 3 circuits and landings without any issues and all landings were of a high standard with quite gentle touch-downs. On the forth landing they were flying a glide approach to a flapless landing. The round-out and hold off were a little flatter than normal, but they touched down at 58kts on the main wheels first. The nose started to come down, but passed through the normal three-point attitude. The Instructor came on controls with the student to apply full back elevator, which the student had already done. The propeller struck the RWY and broke apart. The aircraft came to rest on the upper half of the nose landing gear strut, rolling out straight on the runway centre line. The touchdown was not hard and the roll-out of the aircraft was not violent. The aircraft was immediately secured and the student and Instructor departed the aircraft unhurt.
Cause - Inter granular corrosion - The attachment fitting to the nose wheel strut from the nose wheel yoke corroded inside the bolt holes, leading to failure. Aeroprakt have issued service bulletin No A22LS-19 to carry out an inspection of the nose wheel fork hinge bracket. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After touchdown as the aircraft was slowing down, the nose wheel began to shimmy. The nose wheel movement became quite rapid and increased in force. The nose wheel leg collapsed, causing the propeller to hit the ground. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On take-off climber to approx. 200ft - an extreme vibration developed. The pilot slowed but had to keep going to clear fence, roads and trees. The propeller departed with a loud bang, hitting the front cowl and the pilot was able to land in a paddock with no further damage to aircraft.
On inspecting the propeller flange there was a hole drilled and threaded for locking the propeller flange stub to the crankshaft. This caused a fatigue point where it tore leaving half the thread on either piece. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The CFI was taxiing when they heard on CTAF another flight instructor trying to make contact with a Cessna that was cutting them off on the circuit without making any calls. The CFI then witnessed the Cessna was cut off the other aircraft on the circuit by descending on the active downwind leg in front of them and turning to base and final right next to the aircraft doing circuits. As the CFI held short of the RWY they could see the Cessna making left and right turns below 500 feet AAL but at a greater distance than 500 meters of the threshold. The CFI made the call for entering and backtracking on RWY 17 after the aircraft on the circuits landed on RWY 17. The CFI saw the Cessna going left and right on their windscreen at least three times. The Cessna then turned to final while both aircraft were still on the RWY. The CFI had to take their aircraft outside the RWY and notify the instructor on RWY 17 to expedite their taxi to be clear as the Cessna had just landed while both the aircraft were on the RWY. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft lined up and took off at full power on RWY 32. Towards end of the RWY at approx. 300-400ft AGL the engine lost power. The pilot turned and landed safely on RWY 10.
Outcome: Causal factor was fuel starvation, due to incorrect fuel filter part number installation. An aftermarket fuel filter that looks similar was at some stage preceding the incident installed. During operation an internal screw fitting had loosened allowing the filter to partially block off the outlet port of the filter assembly body, allowing enough fuel for low powered operation but an inadequate supply for high power. With a minimal amount of fuel being stored in the carburettor bowl, the supply issue would not be evident until approximately 15-30 seconds after application of full power. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The flight was for to allow the pilot to take their father up for their first passenger flight. There were some showers in the area so the pilot was briefed by the CFI on how much flight time was available and where to operate to remain clear of weather. The weather started to move in so the pilot decided to land, but rushed the landing and didn’t flare enough causing a prop strike.
A number of human and decision making factors led to the outcome of this flight and pilots are reminded of the importance of early decision when weather is a factor, along with using standard landing techniques. |
