OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst back tracking down the runway the front wheel collapsed at very low speed causing the propeller to strike the ground. Pilot advised that the nosewheel dug into soft soil in the bottom of a divot in the runway surface, causing the nose undercarriage to exceed its design strength and collapse. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot misinterpreted the Maroochydore CTR LL of 1500ft. as the overlaying 4500 ft class C airspace limit. A subsequent flight planning debrief and 2 flight reviews were undertaken to provide assurance the pilot was competent in identifying and remaining clear of controlled airspace. Subsequent to this event the pilot has also successfully completed CASA licence training and been issued with a Recreational Pilot Licence (RPL) with a controlled Aerodrome and Controlled Airspace endorsement. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was making a return flight from Townsville to Atherton. After refueling at Atherton Airport the aircraft took off. When it reached an altitude of approx. 3,800 - 4,000 feet the engine started to shudder and vibrate for approx. 2-5 seconds before locking up and stopping completely. The pilot made one mayday call on 126.7 and then several more on 132.9 on the CTAF. The pilot completed a successful emergency landing into a paddock causing minor damage to the aircraft and no injuries to the pilot.
OUTCOME: Pilot made a successful landing in a field after catastrophic engine failure which caused minor damage to airframe, but no pilot injuries. The engine was later dismantled for investigation. During the investigation the engineer found that a valve stem had failed causing significant internal engine damage and the engine to seize after severe vibration. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft took off before a safe flying speed was reached. The Instructor took over however, attempts to maintain runway alignment were futile. The aircraft failed to gain height and control, further diminished until terrain was contacted, some 50 metres from the runway.
Pilots are reminded of the importance of ensuring a safe take off speed is established and maintained during take off, particularly for high drag, low inertia aircraft |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An aircraft conducted right hand circuits instead of left hand as listed in ERSA at Ballarat AD. The pilot of this aircraft changed to left hand circuits after being advised. The pilot was interviewed and advised they were familiar with the local procedures but due to recency had reverted to previous direction for RWY 18 and 13 which requires right hand circuits. A subsequent flight review was due and the examining CFI reviewed the incident with the pilot and assessed them as competent to continue to exercise the privileges of their certificate.
Pilots are reminded to operate with vigilance and conscious situational awareness at all times and this should be high on pilot's checklist's especially if currency has been affected by external factors. This is highly relevant with the current restrictions in force in various states due to COVID 19. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A Jabiru J-230 entered and backtracked with the intention of taking off. The Jabiru entered the runway with a C172 and a PA44 already established on the mid- and early final approach phases respectively for the runway. Both aircraft completed a missed approach and go-around to prevent a further breakdown in landing separation and in response to the runway incursion.
The pilot of the Jabiru was interviewed and could not recall any separation issues on take off. Initially the date provided for the incident was misreported in error. A review of rules of the air and other regulatory requirements was undertaken by RAAus Operations with the pilot by telephone and the occurrence recorded for future monitoring if required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was landed on the main wheels on the dirt strip. The aircraft was held with positive forward pressure and some left aileron control to hold wing into wind down. As the airspeed dropped, the tail wheel gently descended to the ground, right at the moment the aircraft crossed over the bitumen runway 36/18. The right wheel (into wind) lifted off the ground slightly and the aircraft ground looped without contact of wing or propeller.
The pilot was interviewed and confirmed that he was too relaxed in his technique as the aircraft came to rest in the 3 -point attitude. Increasing cross-wind strength during the flight interval was also a contributing factor. A review of control management focused on increasing application of the into wind aileron as the angle of attack is effectively increased while transitioning to the 3 point attitude. The old adage that "its' not landed until parked at the hangar" continues to be a saying that all pilots but particularly tail wheel operators should remain vigilant to. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was conducting circuits. On the 3rd circuit the aircraft engine had a partial power loss and began to surge. The pilot determined that the aircraft had sufficient height and power to return to the airfield. The aircraft was landed uneventfully on the runway in the opposite direction to take-off and taxied back to the hangars in order to diagnose the issue.
Investigation revealed that fuel flow out of the fuel filter was minimal. The fuel filter was replaced, the aircraft was refueled and test flown with nil issues. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft departed Class D airspace at Launceston tracking on a northerly heading for Georgetown (YGTO). Listening on Melbourne Centre and Georgetown (YGTO) CTAF on standby frequency. A white aircraft flew within metres of the left hand side of the training aircraft. The aircraft immediately took evasive action to avoid that aircraft and broadcast on Melbourne Centre as well as Georggretown (YGTO) CTAF to try and make contact with the other aircraft with no response. The aircraft maintained visual separation with other aircraft until they were safely clear of it.
Pilots are reminded of the importance of "See and Avoid" and never assume no other aircraft are present, simply because no radio calls have been received. Refer to CAAP 166-01 and CAAP 166-02 for more guidance. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Aircraft bounced on landing. The pilot did not apply power immediately to gain control and the aircraft landed on its nose wheel. The nose wheel leaf spring collapsed. The nose of aircraft hit the ground and skidded to a stop. The pilot noted a review of required actions in the event of a bounced landing was appropriate. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot planned to fly to Mudgee Airport to refuel but was diverted due weather to Bathurst. After refuelling the pilot taxied along the taxiway to the holding point where they were in radio contact with a Cherokee 6 coming in to land from the circuit. The pilot of the Tecnam advised they would hold short of runway until the Cherokee had landed and was clear. After sufficient time after it landed and the pilot no longer had sight of it they assumed it had exited onto the taxiway so entered the runway and began back tracking for take-off from runway 35. The Cherokee was not yet clear of the runway as it was backtracking for the exit onto the taxiway, however the pilot of the Tecnam took off having not seen the traffic on the runway due runway slope. Due to lack of planning from inflight diversion and with no other traffic in the circuit, the Tecnam pilot made a left turn at 750ft AGL to track their next destination not realizing that the circuit direction was to the right.
A review of flight planning and operations at Non towered aerodromes was conducted by the flight school with this pilot undertaking endorsement training. Subsequent retraining and testing of the candidate was later completed for both RPC Cross Country and RPL privileges. |
