OCCURRENCE SUBMITTED TO DETAILS RAAUS: Instructor pilot reported reduced engine performance during flying training circuit operations. The aircraft was grounded for inspection. Subsequent ground inspection of the aircraft engine revealed No.2 cylinder exhaust valve to be damaged along with Muffler assembly internal baffles.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: RAAus aircraft was observed to infringe into Class C and D airspace north-east of Albury, during its flight from Mouth Beauty to Holbrook. The PIC had reported that he was descending the aircraft to 3500 ft while approaching Albury airspace and intended to remain clear of the space. It was also reported that he believed to track far north-east to be clear of Albury airspace and altered course to the east before continuing to Holbrook.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: RAAus registered Sling aircraft was heading south along Bribie Island coastline after departing from Caloundra aerodrome. The PIC was intending to follow around the southern tip of the island before heading North-West towards Glasshouse. The PIC was maintaining 2500ft and observed a Jabiru was travelling east calling from Bribie Island Bridge at 2200ft with intentions to follow southern tip of Bribie Island and head north. Due to the potential proximity of two aircraft, the PIC decided to climb 300ft and maintain significant separation, which had accidentally put the sling aircraft into class C airspace. The PIC of the sling aircraft subsequently descended to 2500 feet after being contacted by ATC, and corrected course to the North-West at the same time to exit the class C airspace.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The PIC was flying RAAus registered Tecnam aircraft at 1500 ft to overfly Caboolture airfield then descend on the non-active side to join crosswind for active RWY 11. Before the aircraft reached the field, at 1 NM north of Caboolture airfield tracking south, a flock of black and white birds had crossed the aircraft nose (from east to west and approx. no higher than 10 feet above above the cabin). Hence the PIC nosed the aircraft down and to the right to avoid the flock of birds, and levelled off at 1300 ft. Subsequently, the PIC did a control check before proceeding to climb back to inbound height of 1500 ft. Once over head the field, the PIC had followed the circuit procedure and made a full stop landing and brought the aircraft back to the hanger, where upon inspection found a bird strike on the left leading edge of the wing. RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: RAAus registered aircraft was performing a flight from YBUD to YBRK. On approaching YBRK, the PIC made an inbound call overhead VFR Pirate Point requesting clearance, which was granted by ATC. However, it was notified by the ATC that the aircraft had entered controlled airspace when the PIC contacted them.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: RAAus registered Foxbat aircraft operated during a flight between The Oaks and Taree airport and return was involved in an airspace infringement into controlled airspace at Williamstown airspace and Sydney basin. The aircraft had reportedly flown above the LL for the airspace. The PIC has reported that an inflight check showed the QNH for the area setting were correct and that the aircraft was below the LL for the airspace as indicated by the altimeter. The PIC also added that an inflight discussion was made to fly at a lower altitude by approx. 300ft to avoid another infringement through Williamtown airspace. On return, the PIC was accompanied by another aircraft due to which a series of comparison checks were carried out on the altimeter and ASI with both instruments reading within 20ft and 3kts. The PIC hence believed the transponder was transmitting the incorrect altitude, which was then confirmed during ground checks. However, a second infringement had occurred during the flight which the PIC has reported was due to a sudden burst of wind (Southey buster) causing the aircraft to climb and exceed the LL for the airspace as well as causing the aircraft to clip the restricted airspace for Richmond. Once realised evasive action was taken to reposition the aircraft.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The PIC was conducting circuits. During the third circuit, the pilot landed, cleared flaps and adjusted trim. And then, the pilot applied full power for take-off when the plane veered to the left more than normal. The pilot observed that there was little effect when he applied more right rudder. The aircraft then became airborne and at around 20ft when more rudder application was required but the pilot observed no positive effect to keep on-line. The pilot then pulled the power back fully and aborted take-off. Once back on the ground the plane was on the left-hand side of the bitumen then onto gravel. Subsequently, the aircraft rolled into the open space and came to a stop approximately 70 m on the east of RWY within airport boundary, after clipping a white plastic drain marker. The pilot exited the aircraft after making it safe.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: RAAus registered Bristell aircraft was operated by a student pilot for solo flight after being assessed by the Flight Instructor. Prior to releasing for solo flight, the instructor had broadcasted on radio that the student was undertaking solo flight from RWY 35. Having not heard any response on the radio, the instructor exited the aircraft and allowed the solo flight to be performed. As the instructor was walking back to office, he noticed a VH-registered fast twin aircraft joining a wide crosswind for RWY 17. Subsequently, the instructor also observed Bristell aircraft turning right downwind for RWY 35 and the VH-registered aircraft turning left downwind for RWY 17. Shortly afterwards, it was also observed that the VH-registered aircraft turning right and head east away from the circuit area. RA registered Bristell aircraft completed the circuit and landed safely. DETERMINED OUTCOME: The incident was reviewed and determined that while conformance with the established circuit direction was not initially followed by the identified aircraft, once in communication with a third party they amended their approach with no conflict in separation.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The crew (instructor and student pilot) were conducting flying training and then paused their operations for a break. Then, before resuming further flying training, the instructor conducted another pre-flight lesson briefing, and the student started the engine. It was observed that the engine was running rough and the instructor decided to put the engine down. On inspection, it was found that the exhaust valve of Cylinder No. 4 was out of its place and wasn’t functioning properly. In discussion with the manufacturer, there were no causative factors identified.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The aircraft overran runway 26 during landing. After touchdown, the pilot applied brakes, but it felt like aquaplaning, and the aircraft failed to stop. The aircraft had no steering functionality. The aircraft was eventually taxied to a parking area, where it is awaiting an L2 inspection before returning to the home base.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: During a circuit training session at Jandakot Airport, while on the upwind leg of RWY 06R and still below 500 feet, the aircraft experienced a total loss of power on the avionics bus, resulting in the loss of all instruments. With the aircraft powered solely by the main power supply and no backup battery on board, the instructor immediately took control of the aircraft from the student upon detecting the fault. They continued the climb away from the ground, maintaining the runway heading and the established attitude for best rate of climb. The engine appeared to be producing full power. As the aircraft was within Class D airspace with a ceiling of 1000 feet, the instructor used visual reference to two preceding aircraft and the horizon to estimate the altitude, ensuring the aircraft did not exceed the altitude of the other aircraft. While continuing to fly the normal circuit, troubleshooting was initiated. The instructor turned off all electrical systems and switched off the avionics, leaving only the Master and Magnetos on. After a few seconds, the avionics were turned back on, but the instruments remained blacked out. Unable to make a PAN call due to the loss of radio communication, the instructor could not contact the tower to obtain clearance for landing. The decision was made to continue flying the normal circuit, maintaining visual contact with the preceding aircraft, observing for light signals from the tower, and proceeding with the landing without instruments. After completing the pre-landing checks on the late downwind leg, the avionics switch was turned on once again, and the instruments illuminated. The indications were normal, and the altitude displayed exactly 1000 feet. Communication was restored, and the instructor was able to call the tower, obtain landing clearance, and proceed with an otherwise uneventful landing. The fault was subsequently reported in the maintenance release, and the aircraft was grounded. A discussion with the LAME revealed that a service bulletin (SB-100-002LSA) had been issued for replacing a self-resettable fuse with a non-self-resettable fuse. The service bulletin described symptoms identical to those experienced in flight, indicating that in rare circumstances, due to operating conditions or environmental factors, self-resettable fuses could cause a reset even when the nominal amp-rating was not reached.

OCCURRENCE SUBMITTED TO DETAILS RAAUS: DEFECT: The pilot was about to taxi to the parking area after refuelling from a flight. They were in the pilot seat with the hood closed and latched. As they were just starting to move, when the right hand front canopy mount broke. This caused the canopy to be raised by about 70mm. The pilot immediately shut down and pushed the aircraft to its parking spot.
