OCCURRENCE SUBMITTED TO DETAILS RAAUS: The pilot of the aircraft breached Class Delta airspace. They realised immediately, descended and turned left to avoid the airspace.
The instructor was new to the flying school and area and was assigned the navigation lesson at the last minute when the original instructor for that lesson had called in sick. Student had flown off track and this was not immediately noticed and CTA was penetrated. A combination of inattention, unfamiliarity with location and last minute change of instructor were in no small part factors in this airspace incursion. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: Student pilot was on solo flight when they lost control of aircraft on ground during touch and go leading to runway excursion and impact with airfield lighting post.
This was not the student's first solo. Student debriefed post incident, rechecked and is now ready to resume solo flying. FTS concerned is reviewing its procedures for sending solo to see if improvements can be made to mitigate against any future occurrence. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: During a flight from YGYM -YGYM, the pilot was tracking south along the coast from Noosa at 500ft AMSL.
As the pilot approached Castaways Beach, 2 aircraft were tracking North above them and allowed those aircraft to pass overhead before starting their climb to 1200ft AMSL. The pilot intended to do a U-turn and head back up the coast to Castaways Beach before turning left for YGYM. As the pilot reached 1000ft AGL, they made a right U-turn and started tracking North.
As they completed the turn and was tracking North, they heard Brisbane Centre broadcast as the aircraft entered controlled airspace overhead Peregian Beach. The pilot responded and was asked to call up a separate frequency. The pilot dialled the frequency and announced their aircraft. The controller asked what their intentions were, and the pilot responded that they needed to climb to 1000ft AGL before they could turn right over built-up area. The pilot then proceeded directly to YGYM.
Pilot was aware of the CTR proximity but delayed turning to avoid the airspace due to conflicting traffic, by the time the turn was completed, CTR was penetrated. Discussed not flying right up to the CTR boundary before taking avoiding action but instead to plan to commence avoiding manoeuvres some distance away from the boundaries. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During cleaning the aircraft, a small crack in the left hand inner side of the exhaust was noticed. During the previous flights there was no indication of increased CO levels on the RAAus CO electric monitor, which was used throughout flight. The owner contacted a LAME, who further investigated immediately. They found more cracks in the centre section of the exhaust. Exhaust was removed, repaired and re-installed in accordance with manufacturer's instructions by L2 and recorded on HAM.
The manufacturer was notified. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: While conducting circuits in a Sling 2 aircraft on runway 28 at YHEC, the instructor took note of increased bird activity due to recent rain.
After a touch and go, with the student in control and full power applied, one of approximately 10 wedged-tail eagles, flew immediately ahead of the aircraft and attempted to evade the aircraft while it accelerated. The aircraft struck the bird on the propeller and the right-wing root. The instructor did not see the bird strike the propeller, but did see it strike the right wing root as it was directly out the window. The instructor took control of the aircraft from the student and executed a rejected take-off.
After vacating the runway, the instructor mentioned on the radio that there was likely a large bird on the runway, since one of their colleagues were also operating in the circuit. The colleague conducted a go-around on the next circuit and told them that they could see the bird on the runway. The instructor stopped their aircraft and when it was safe, removed the bird carcass from the runway. The instructor parked the aircraft and inspected it for damage but was unable to identify any aircraft damage.
RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The pilot was joined for circuits at Bathurst Aerodrome. The pilot was on the wrong side as they joined for left-hand circuits and was told that they were on the wrong side.
The pilot rejoined for right hand circuit, and there was another aircraft in the circuit that the pilot had spotted and joined the aircraft. The pilot noticed that a second aircraft came in front of them but was lower in altitude and was at a higher speed. The pilot observed that the second aircraft was distant and not close to them in the circuit but became in front of them later at a lower altitude. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: Whilst the instructor was supervising the pilot on a check flight, they extended downwind too far and infringed CTR. The instructor informed the pilot to turn before the intersection which marked the edge of the restricted airspace, and the pilot advised they had it in sight. The pilot had looked at the wrong intersection, as they did not fly in the area for some time.
As the individual was experienced with the area, the instructor did not brief him regards to airspace even though he had not flown for a while . There is an airfield photo with airspace indicated in the club house and the instructor will not assume anything in future and always run individuals over it before they fly. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The instructor and student pilot landed heavily. They tried to go around with full power causing loss of control and the right wing hit the ground followed by the left wing. The nose wheel then collapsed resulting in prop strike.
The Instructor had been conducting a TIF at the time and had made one previous approach which was high and had elected to go-around for another approach which was subsequently high also. Pressed on and selected full flap to steepen the path (against CFI instructions until the instructor had accumulated more experience on the type due to its handling qualities in the go-around phase). The second approach continued on to be a heavy bounce landing from which a go-around was attempted but due to a miscommunication with TIF student, this student applied left rudder at the point of go-around resulting in the loss of control of the aircraft. Spoke with the CFI of the FTS who said that the instructor would be undergoing some remedial training before continuing with any instructional duties. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The instructor and student of the RAAus aircraft had a near collision in flight during circuit lesson with a VH registered aircraft at Canberra Aerodrome. The initiation of the problem was most likely misidentification of aircraft by ATC which was further compounded by not being able to make correctional calls on the radio due to radio chatter. Effective use of see and avoid in this case prevented a collision. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The student pilot on their first solo did not hold off or flare sufficiently to arrest decent on landing. The aircraft’s nose landing gear collapsed and resulted in prop strike. Student had been performing well up to that point but for his solo approach, the aircraft was found to be out of trim after the incident. The student felt he needed to push forward on the control column, the landing resulted in a bounce and instead of going around, pressed on which ultimately collapsed the nose gear. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The pilot was flying with a group to William Creek and from previous discussions, were with the understanding that there was a group clearance completed to fly through the airspace. The pilot presumed that they were included as the group knew that they were there and followed out after take-off and through the airspace. The pilot did make themselves known to the air traffic control and they did not say that they could not be there. The pilot was further unaware that they were not included in the flight plan.
Pilot has learned not to make assumptions and not rely on others. |
