At approximately 00:30 hrs, while the student pilot was flying, the aircraft experienced a vibration that seemed to originate from airframe or propeller as per the instructor, although the engine did not show signs of rough running. The instructor took control, and after reducing power to 4000RPM at around 800 feet, all parameters were checked and found to be within normal limits, however a PAN PAN call was made to the tower. The flight was continued as the vibration ceased. After landing, the maintainer performed ground inspections at full power and checked components like mags and carburettor, revealed no faults. CFI was consulted and the team inspected carburettor icing given weather conditions at the time of flight. Then it was confirmed that recorded dew point and temperature could have lead to icing of the carburettor, and may have been initially misinterpreted as propeller/airframe vibration. |

Pilot suggests that in Canberra each landing touch and go clearance is usually given with a phrase – ‘Maintain upwind or make left or right circuit’. OCCURRENCE SUBMITTED TO DETAILS RAAUS: The CFI was conducting a final circuit with a student on runway 11 at Narromine Aerodrome which was a planned glide approach/practice engine failure. On upwind, a VH registered aircraft made a radio call, that they were joining downwind for runway 11. The CFI and pilot had them sighted and voiced their intention. The pilot of the VH aircraft confirmed they were number 1 in the circuit. They confirmed they were number 2. When they were on crosswind, a second VH registered aircraft made a call they had them sighted at their 1 o'clock, were behind them and confirmed they were number 3 in the circuit.
They maintained correct downwind height of 1800ft QNH for Narromine. As the student was completing landing checks, a glider passed in front (from outward to inside the circuit) of them at the same height. They took immediate evasive action and lowered the nose to avoid collision. They saw the glider turn clockwise. The CFI made a radio call to the second VH aircraft to notify of glider at circuit height turning towards them. They continued their circuit until late downwind and continued with their practice of an engine failure in circuit from base.
The three aeroplanes in the circuit were maintaining separation, had visual sight of each other and communicated clearly. The pilot of the glider did not make any radio calls during the lead up to the incident. They did not join a circuit patten correctly and were circling downwards crossing the downwind path multiple times. The only call the glider made was on short final YNRM 11 after the CFI and student had landed and was backtracking on 04. |

They thought they were on the runway however there is a drainage culvert which they did not see after landing. They were going approx 25kts and were about to apply brakes when the nose wheel went into the drain and damage occurred. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The instructor of the RA aircraft was conducting a dual lesson at YBAF operating off RWY28R during CTAF. A VH-registered aircraft was on eastern apron and called that they were taxiing to RWY 10L for a departure to Gold Coast off Runway 10L. The pilot of the VH aircraft was informed by the instructor of the RA aircraft that they were doing circuits in 28 direction. The pilot of the VH aircraft acknowledged with "Copy." and then continued with their plans and taxied to departure end of runway intent on departing contrary to established circuit traffic.
The pilot of the VH aircraft called that they were at the holding point lining up for departure on 10L. The instructor of the RA aircraft called that they were on short final. When they were on the runway conducting the touch and go, they heard an unclear call that the VH aircraft was lining up. They elected to abort the take off because they could not be sure of what the pilot of the VH aircraft was doing or intended to do. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The instructor briefed the student on the effects of controls during their first lesson. After taking off from Runway 13, as soon as they crossed the controlled zone boundaries, the instructor reported that they were outside of the controlled airspace and received the response "frequency change approved." The instructor then reported their position to the CTAF, switched the transponder code, and began the lesson.
The instructor was demonstrating the primary effects of the elevator, and reduced the power, keeping the aircraft straight and level, and pushed the stick slightly forward. The aircraft’s nose dropped, allowing them to demonstrate the secondary effects. The instructor then levelled the aircraft, raised the nose, reduced the RPM and speed, and levelled again.
As the instructor was about to hand control over to the student, they noticed a burnt oil smell. Immediately after, the instructor made a 180-degree turn to southbound and informed the student that they were uncomfortable with the smell and preferred to return. At that moment, they were about 1-2 nautical miles south of Noosa. For a few seconds, the instructor considered heading towards Noosa airfield but was uncertain about the condition of the runway due to recent rain. Instead, they decided to return to Sunshine Coast Airport.
The instructor established straight and level flight over the coastline, explaining the situation to the student, and reassured them that everything was fine. Shortly after, the instructor noticed more smoke coming from the engine and oil splattering on the windscreen. The instructor immediately checked the engine's temperatures and pressures, all of which were within the normal operating range. The instructor then contacted the tower, reported the situation, and requested priority landing while continuously monitoring the engine.
Continuing over the coastline, the instructor managed the engine temperatures and pressures, which remained within normal operating range. However, the smoke and oil continued to worsen, with oil entering the cabin through the left vent (the student held it shut with their left hand). The tower gave them full clearance and the instructor climbed slightly to give themselves more room for a potential glide approach, in the event of an engine failure.
The instructor turned onto base just north of Mount Coolum and made a high final approach. From halfway through the base leg, the instructor conducted a glide descent for landing with full flaps. The tower asked if they could taxi to the aeroclub. Since the engine temperatures and pressures were still within normal operations, the instructor agreed provided there was no visible fire outside the aircraft. The instructor taxied back to the aeroclub and shut down the engine after coming to a full stop. The fire trucks were behind them and the instructor moved away from the aircraft for a few moments.
After speaking with the firefighters and receiving their approval, they removed the fairing to ensure there was no fire risk. At that point, they noticed significant oil on the windscreen and the aircraft’s belly. They also discovered the oil was leaking from the left front piston, which was later determined to be faulty due to a damaged seal. |

OCCURRENCE SUBMITTED TO DETAILS RAAUS: The pilot was flying above Rosedale area at 6500ft, when the engine began to run rough. The pilot revised their heading to nearest ALA - Baffle Creek. They made a PAN-PAN call to Brisbane Centre on 124.1. Approximately, 2 NM short of Baffle Creek the aircraft experienced complete engine failure. The pilot was not going to make it to the ALA and landed in a cow paddock to south with tall grass/trees.
The aircraft suffered a hard landing with uneven ground causing the landing gear to collapse. The aircraft skidded sideways to a stop with the pilot collapsed landing gear. Sideways skid to a stop. The pilot impacted the door frame head resulting in a cut to the head. |
