OCCURRENCE DETAILS SUBMITTED TO RAAUS: Aircraft departed the left side of the runway, at low speed, after landing in a light crosswind from the right. Aircraft was stopped by long grass. Pilot in command reported that they mishandled the crosswind landing by not adding enough aileron, and failed to maintain directional control. |

STATUS: Review Complete.
Whilst practicing crosswind landings, the pilot lost control of the aircraft resulting in the aircraft exiting the runway to the left and coming to a stop in the reeds. The pilot has since completed additional flight training with a suitably qualified flight instructor. Pilots are reminded of the importance of operating within personal minimums and to ensure they are current when operating in conditions such as with a crosswind. Pilots are encouraged to improve their skills with an appropriately qualified instructor on board. |

STATUS: Review Complete
OCCURRENCE DETAILS SUBMITTED TO RAAUS: Out landing due to engine failure due to probable fuel starvation. Heavy landing causing major structural damage to aircraft.
OUTCOME: Damage to aircraft during out landing caused by an up sloping field and stall prior to impact resulting in the aircraft touching down left wheel first. Damage consisted of bent fuselage boom, bent and detached engine mount and broken wing wires. Member did not sustain any injuries during the occurrence. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT A fixed pitch propeller blade was removed for an inspection by the manufacturer. After removal, a substantially elongated propeller bolt hole was found. The '~8mm' bolt hole measured 10.57mm (outside) and 10.03 (engine side), the elongated length of the hole extends in the same direction as the length of the blade. The propeller is reported to have run for 2 hours and 5 mins from brand new. All propeller bolts were found to be appropriately secure.
All the propeller blades are being sent to the manufacturer. |

STATUS: Review Complete.
After a standard and stable approach, on rounding out and approximately 5 to 6 feet above the ground, the aircraft dropped unexpectedly resulting in a heavier than normal landing. Review of this occurrence with the pilot in command highlighted that this likely occurred due to higher than required round out during the landing phase leading to a heavy landing after a sudden decrease in lift. The pilot has been debriefed on the incident and completed further training to avoid future occurrences. |

STATUS: Review completed.
An instructor and student were conducting circuits with approximately 8kts of wind down the runway. Two glide approaches were conducted by the student with good landings. The instructor then demonstrated a powered approach, and the student then conducted a powered approach. The radio became quite busy with commercial traffic during this circuit. During landing the round out was normal but during the hold-off at approximately two feet, the student unexpectedly pushed the nose onto the ground. The aircraft bounced and the instructor immediately took control and landed the aircraft. The instructor noticed that the nosewheel was stiffer to steer with the rudders on the taxi back. The nosewheel was partially disassembled and it was identified that the bottom bush for the nosewheel assembly was slightly misaligned with its housing, resulting in the stiffness. This occurrence highlights the importance of control management for instructors. The student was debriefed by the instructor and conducted further training. |

|
STATUS: Review Complete. The flight was a dual training flight with a returning pilot who had not been active for 2 years. The aircraft departed and planned to maintain 2000 feet during flight. Whilst monitoring the student pilot during the return to the airfield in very turbulent conditions, the aircraft climbed to 2500/2600 and inadvertently entered Class C airspace. The instructor debriefed the student on the importance on maintaining altitude when transiting near or below class C airspace. The instructor was de-briefed by RAAus in relation to the incident and was able to generate a strategy to ensure future breaches of class C are avoided in the future. All pilots are reminded of the importance of conducting thorough pre-flight briefing and monitoring position and altitude during flight, particularly in complicated airspace, in order to avoid an airspace infringement. |

STATUS: Review Complete.
OCCURRENCE DETAILS SUBMITTED TO RAAUS: On a local flight to Yarram the pilot noticed the mechanical fuel gauge reading at min fuel pressure, so they activated the electric fuel pump and pressure returned to normal for a standard landing back at YYRM home base.
OUTCOME: The aircraft was inspected by an L2 maintainer and determined that the fuel pump was faulty. A replacement pump was ordered and replaced. |

REPORT FROM SAFETY MANAGER
An RAAus aircraft was conducting circuits with a converting pilot on the into wind runway to the south. The AWIS confirmed 15 knots from the south and when the Regular Passenger Transport (RPT) aircraft called inbound the CFI assumed it would join downwind. The ETA was also noted as 10 minutes away, which was unusual.
During the landing phase the CFI became distracted due to the converting pilot landing long and when they conducted a go around they noticed the inbound aircraft was ahead and on a straight in approach, directly conflicting. While there was no immediate risk of collision, the CFI ensured the RAAus aircraft moved to the right and provided access to the north facing runway.
Subsequent interactions identified a number of factors, including assumption of the runway direction used for the RPT aircraft, distraction of the CFI and failure to effectively communicate between the two aircraft. The Safety Manager and CFI debriefed and the report has been closed. |

STATUS: Review Complete.
OCCURRENCE DETAILS SUBMITTED TO RAAUS: Prior to run ups, the pilot of a Jabiru had given a taxi call advising they were taxiing to runway 21. After runups in the run up bay, the aircraft taxied to the 21 holding point and held there whilst the pilot made a radio call with intentions to enter and line up on runway 21 and it included that the departure was to be to the south west climbing to 2500. Prior to entering they held for a Europa that was on short final touch and go. After it had passed the Jabiru they once again did a visual check, entered the runway and proceeded to do a turn to the east and conducted a 360 degree turn to visually check base and final prior to lining up on runway 21. The area was clear, and they were not in receipt of any radio calls. The pilot then commenced a take off roll applying full power and a few seconds later an aircraft landed over the top of them on the same runway.
This aircraft registration was later confirmed as a VH registered aircraft used for parachute jumps. The Jabiru pilot immediately cut power and applied full braking and requested that the aircraft transmit their call sign. The VH registered aircraft then vacated the runway. The pilot of the VH registered aircraft advised that they had not seen the Jabiru and admitted they had their aircraft nose high on the approach and that they were very sorry. They advised that they saw the Europa do a touch and go. They didn't admit to hearing the Jabiru's radio calls and they had not made any calls in the circuit to their knowledge. This occurrence was forwarded to CASA for review. |
