OCCURRENCE DETAILS SUBMITTED TO RAAUS: While returning northbound for Archerfield on descent from the southern training area, the pilot momentarily lost a visual fix of their location and breached the 4500ft BN step. Brisbane centre ATC (125.7) contacted the pilot prior to the airspace violation however the violation of CTA was already committed. The pilot responded, notifying that they will comply with a left turn starting a descent to 3000ft.
LEARNING OPPORTUNITY: The pilot identified that the GPS position shown on their electronic flight bag (EFB) was not accurate resulting in the pilot entering controlled airspace. Pilots should not become reliant on information provided by EFBs are to be reminded that GPS location should be used as a reference however should be visually confirmed using relevant charts in order to avoid inadvertently entering controlled airspace. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Upon the completion of an instructional flight to the training area, the aircraft was conducting a touch-and-go landing on runway 11R at Bankstown (YSBK). During the initial climb out after the touch and go, the PIC observed less than full power with mild surging and notified the tower. Between 350-400ft AGL, engine power continued to wind back below a suitable level and climb performance was significantly degraded. The PIC elected to conduct an opposite-direction precautionary landing on runway 29R and executed a 180-degree turn with partial power. The tower managed conflicting traffic and provided a landing clearance; the aircraft landed uneventfully. Further investigation after the incident found that the fuel filter in use was too fine and consequently limited fuel flow to the pumps. Maintainers are reminded of the importance of ensuring correct parts are used when conducting all maintenance to avoid failures from occurring. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual circuit training at Riddell Creek airfield, the student had completed two circuits, was on the downwind leg of their third and had just completed their landing checks when they heard a radio call stating another Topaz was leaving the Penfield circuit. No response to this call was made by the aircraft operating at Riddells Creek. Given the proximity of the Penfield airfield, Riddell Creek circuits are to the East and Penfield circuits to the West to ensure clear airspace. It was at this point that the student saw the second Topaz heading south towards them. The instructor then took control of the aircraft and made a turn to the right avoiding the second aircraft whilst making a radio call stating restating their position. The instructor handed the aircraft back to the student who then completed another uneventful circuit.
Both Instructors were reminded of the need for clear responsive radio calls when the potential for conflict exists under CAAP 166 requirements. |

New parts have the correct hole spacing for the wheels. (Wheels and disks now renewed). |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft departed with 30L of fuel on board. Approx. 15 minutes into the flight the engine began to surge (approx. 6000rpm to 1300 rpm). The electric fuel pump was turned on to try to alleviate the issue to no avail. The aircraft began to lose height whilst flying back towards airfield. Aircraft was landed in a paddock uneventfully with nil issues or damage.
Outcome: The rubber on the inside of the fuel hose supplying the carburettor was found to have degraded causing a blockage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft operated for 3 hours on the morning of the incident. It refueled, the fuel was checked for contaminates after refueling. The aircraft departed for another station 120nm south of Packsaddle. The engine failed after take-off at 400ft AGL. The pilot tried to restart it but was unsuccessful. The aircraft landed in a cleared area of the paddock.
Review identified failure was likely due to fuel contamination as the aircraft was refueled from a fuel drum on a farm. Since the incident fuel management and testing procedures have been improved to prevent a repeat occurrence. Members are reminded of the importance of proper fuel management ensuring fuel is not contaminated prior to use. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual navex flight training a small loss of power was noticed, 50-100rpm or so. The instructor suspected maybe carby ice, however carby heat made no difference so removed the CH. About 30s-1 min later the aircraft experienced a sudden onset of severe vibration and loss of power - it felt like a cylinder had stopped, the instructor took over at this point. Reducing throttle to below 2000rpm got rid of the vibration a bit. Adding it back and the engine immediately started to vibrate badly and clearly not run on all 4. The aircraft had insufficient power to maintain altitude. The instructor chose a field to make an emergency landing whilst trying carby heat, fuel pump and mags to no avail. The aircraft landed uneventfully. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Touchdown on RW29R was normal, but during the rollout, the aircraft tended to veer to the left.
When moderate braking was applied, and Aircraft tracking corrected, the Brake lever went completely slack and Braking action was lost completely.
After exiting the runway, there was a tendency to track to the left and i felt that the left brake was dragging however i was able to taxi back to the hangar under power without assistance. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The electric elevator trim control appeared to be not working or very slow in flight. The trim position indicator on the Garmin G3X display confirmed little movement of the elevator. The elevator on this aircraft is a flying surface, there is no trim tab fitted.
Subsequent ground inspection showed that when the stick mounted trim button was operated, there was no movement in the elevator when any force was exerted on the stick. If the stick was not loaded (no force applied) the trim button did move the elevator very slowly.
A flight test was conducted which again confirmed the original observation. During the test, the Garmin Autopilot was engaged. The Flight Display showed TRIM UP & TRIM DOWN flags at various times in the flight, operating the trim control did not cause the pitch to change or the flag to be removed. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft landed successfully on a private strip, however during landing roll the propellor made contact with a sheep fence at approx. 20-30kmph. RAAus investigations determined the pilot was not a current member, had not completed a BFR and the aircraft was not registered at the time of the flight. The pilot was aware of these non-compliances, yet continued with the flight regardless. He has now been suspended pending completion of requirements to address the non-compliances. Members are reminded there may be serious consequences to operating outside the RAAus Flight Operations and Technical Manuals, including insurance implications, disciplinary action by RAAus and CASA and potential convictions as a result of breaches of the Civil Aviation Act (1988). |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft departed Yarram airport and headed to the south east of the field. Upon reaching the coast it was noticed that engine coolant was coming out from the engine cowling and dripping onto the dashboard, the pilot turned the aircraft around to head back to the airport. Shortly after turning back smoke started to fill the cockpit so the pilot called Yarram’s frequency to advise aircraft was experiencing engine troubles and was intending to return to the airport. As more smoke filled the cockpit the pilot opened the latch to their side of the canopy in an attempt to let some of the smoke out, it helped however visibility was still impaired.
When the aircraft was approx. 2 miles from the strip the engine started to seize and had no power. The passenger (also a pilot) attempted to land the aircraft in a clearing in front of them, as the pilot could not see clearly due to fogged glasses. Just before the clearing they saw a glimpse of the tree canopy through the smoke and realised that we were not going to make it. The aircraft then made contact with and remained suspended in trees approx. 7m above the ground. One occupant fell to the ground and the other managed to climb safely to the ground.
During the review it was not possible to determine the cause of the failure. Pilots are reminded to consider a precautionary landing rather than trying to press on to an airport when experiencing technical issues. |
