OCCURRENCE DETAILS SUBMITTED TO RAAUS: The engine start-up occurred normally, with rough running observed thereafter. Between Idle and 3000RPM rough running was observed, with visible shaking of the engine cowl. A large magneto drop in excess of limitations was observed at 2000RPM. The engine functioned normally above 3000RPM, with a normal magneto drop observed when tested. A decision was made to commence taxi to the runway and monitor the engine on the taxi. During the taxi to Runway 22 at YTDN, power was set at 2500RPM for the taxi. During the taxi, the engine RPM was observed to decrease until the engine was near idle with rough running and eventually cutting out. An attempt to restart was unsuccessful at which point the aircraft was towed back to the hangar to be inspected by maintenance. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A runway incursion occurred after the pilot was directed to holding point B2. The aircraft slowly taxied and crossed B1 and runway 11C. The aircraft was directed to stop, complete a 180 change of direction and return to parking. The pilot had 2 maps open (OzRunways and airport FAC hardcopy from ERSA). The pilot reported confusion started due to FAC not identifying B2 on both sides of runway. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot turned from base to final higher and faster than normal. The aircraft came over the runway threshold in the same configuration and floated longer as a result. The aircraft was in a nose low attitude and made contact with runway nose wheel first. The nosewheel buckled slightly, resulting in the propeller striking the ground before full power was applied for a go around. A further circuit was conducted and the aircraft landed without further incident. The pilot has been required to complete retraining prior to flying an aircraft as pilot in command and has been debriefed by RAAus Flight Operations Staff. |

Jabiru Service Letter JSL021-2 states as below for in-flight over-voltage situation management.
3.3 Inflight overvoltage situation
If there is a malfunction of the charging system that causes an overvoltage situation there are steps that can be taken to minimise risks of damage to instruments and charging system.
An overvoltage situation does not affect continued operation or the performance of the engine.
1. Reduce rpm until voltage reduces to within limits. Maintain for 5 minutes or as long as possible before increasing power again.
(The alternator is a permanent magnet device and its absolute voltage output is proportional to rpm.)
2. Proceed to nearest safe landing area. Manage voltage using rpm as much as possible. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had flown a local flight and conducted one touch and go on runway 27 at West Sale. The wind on the East Sale ATIS was indicating 270 at 8kts and the wind at West Sale was similar but potentially up to 10kts. After one circuit and during the second touch and go, at approx. 30ft with full flap the airspeed rapidly decreased to approx. 50kts and the aircraft nosed forward and impacted the runway on the nose wheel causing the nose wheel to collapse and the tip of the prop to strike the runway. The aircraft was shut down and pushed from the runway. The ARO was contacted and immediately conducted a runway inspection which revealed a small amount of debris from the composite prop. The pilot and passenger were uninjured. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A pilot arriving to Sunshine Coast Airport was requested to maintain A015. On arrival into onto the downwind leg the pilot was observed by ATC to be at A011 without clearance. The pilot was then given a visual approach and landed without further incident. The pilot has undergone retraining on the use of controlled airspace and has subsequently complete a passenger endorsement. The procedures as the FTS are also being reviewed to help mitigate a similar incident occurring in the future. |

One emu was on the port gable markers and two emus were on the starboard gable markers, all well clear. The single emu attacked aircraft without warning (probably protecting other two emus) The pilot turned the aircraft away from the attacking emu but it ran into the rear of the propellor and was instantly killed. The engine stopped/stalled and the aircraft was towed back to hangar for inspection. |

OUTCOME: Exact cause of accident was unable to be determined. The pilot lost control of aircraft during the take-off phase of flight. This may have been due to gusty conditions at the time of the incident with improper or insufficient rudder input or as a result of a foreign object limiting the moment of the rudder pedals during this stage of flight. The member has conducted further flight training with a qualified instructor. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While an A320 was approaching runway 06 to land at Ballina Byron Gateway Airport, a Jabiru J230-D was tracking south toward Evans Head Airport, NSW. About 22 km south-west of Ballina Byron Gateway Airport, the two aircraft's flight paths intersected, with a vertical separation of approximately 600'. The A320 traffic collision avoidance system (TCAS) traffic advisory provided an alert to the A320 crew, however this has been determined to be a separate issue.
RAAus members are reminded of the importance of ensuring transponders are calibrated and operating with Alt when in flight and encouraged to review the CASA Be Seen, Be Heard, Be Safe guidance material, in addition to CAAP 166 when operating in an around busy non-controlled airports like Ballina. When the aircraft banked away from the empty fuel tank air was sucked into the fuel line and got a fuel pressure warning and the engine lost power. The aircraft landed in a paddock. Both Pilot and Manufacturer have subsequently taken actions to ensure further fuel mismanagement does not occur. The manufacturer has published an updated POH to advise a minimum of 10L of fuel must be carried in each wing tank. |

During the conversation, the instructor was told that they had conducted one of the circuits in the wrong direction. The non compliance with the airways clearance has been addressed satisfactorily by the CFI and Safety controller for the school. Attention to changes in ATC circuit direction clearances and noting them in flight can assist in mitigating the "expectation bias" than can occur when operating in CTA. |
