OCCURRENCE DETAILS SUBMITTED TO RAAUS: On final approach the wind was gusting and moving strongly. The approach speed was kept faster than normal due to the conditions. The airspeed indicator was fluctuating rapidly between 60 and 70Kts. On round out just before the flare at approx. 6-8 feet off the ground the aircraft suddenly lost altitude and firmly impacted the runway, resulting in a broken nose wheel, propellor and minor lower cowl damage. The pilot noted several relevant factors, including operating an aircraft type he was not as familiar with which resulted in a lack of familiarity for the type and pressure to go flying from a friend. Flight Operations debriefed the pilot on the above points and pointed out the importance of type familiarisation in all scenarios, especially between different variants of the same make of aircraft. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Pilot experienced a rapid increase in cylinder head temperature to over limits. Power was reduced and cowl flaps opened. Commenced descent and made Pan call to Mel centre. Uneventful landing was conducted. Review identified that hoses were recently replaced as per service bulletin, however it appears one of the clamps was not fitted correctly resulting in coolant loss. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: 0.5 hours into the flight the pilot noticed an abnormal oil pressure indication, showing off the scale HIGH pressure. Approximately 2min later the oil temperature started to drop, followed shortly after by an RPM drop. Power was added to recover the RPM drop and the engine vibrated, tried to stall then surged. The engine continued to run at the new power setting. The pilot conducted an uneventful landing. Review identified engine damage from number 1 cylinder. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was departing downwind at 1200 feet when the canopy opened. The aircraft started to descend and despite adding power and increasing angle of attack the aircraft was unable to maintain altitude. Attempts to close canopy failed. An emergency was declared on local CTAF and the aircraft was put down in a paddock east of the aerodrome. The aircraft touched down successfully but needed to be ground looped to avoid trees and fencing at the other end of the paddock. Review identified a manufacturer service bulletin was in place for modification of the canopy latch but had not been fitted. This has since been fitted to the aircraft. Maintainers are reminded of the importance of ensuring service bulletins are completed as per manufacturer requirements. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was being used in a Straight and Level lesson, and was in straight and level flight at cruise power when the engine suffered a partial loss of power. The pilot made a precautionary landing. The engine continued to operate throughout the flight until shutdown, albeit at reduced power. The maintainer identified piston damage in cylinder number 1. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Take off into training area on runway 29c. It was a solo check so student was the pilot flying. The aircraft experienced a partial loss of power on take-off at 400 feet, the instructor took over control and requested a full stop and advised ATC that they did not have full power on climb. After 20 seconds the aircraft lost 80% of power, the instructor advised ATC and conducted a glide approach onto runway 11R. The aircraft had enough power to taxi the aircraft away from the manoeuvre area.
On review the engine suffered a coolant leak due to a hose rubbing on the exhaust. The engine seized due to overheating and required replacement. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Engine lost power in flight and remained at an idle, switched on fuel pump, no response. The pilot gave a mayday call, set the aircraft up at 60 knots best glide speed and retained that while trying to reach the clear spot ahead. The left wing impacted a tree which slowed the impact. The front wheel broke off, the nose impacted the ground and the aircraft became inverted. The cause of the failure was unable to be identified, however it is possible a small piece of corrosion came adrift from the bottom of the carburettor bowl and became lodged in the main jet. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Upon landing the tail wheel aircraft the pilot accidentally bumped the throttle on, which was immediately pulled back off. There was a resulting loss of control possibly as a result of the large soft tundra tyres and the resulting grip on the bitumen. This was noted as due to the differences in response between landings conducted on grass versus bitumen and is not limited to tail wheel aircraft. RAAP 1 - Type Training is recommended for review to ensure possible handling and operational differences such as this are assessed and managed by pilots and instructors conducting conversions to new aircraft types. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting aircraft testing and focusing on a discrepancy between an EFIS and an analog instrument the pilot inadvertently violated Military Restricted Airspace. ATC contacted the pilot to advise of the entry and the pilot completed a 180 degree turn to exit the airspace. The pilot was debriefed by RAAus operations staff on areas including situational awareness, threat and error management familiarity of the local area. Members are reminded at all times that the pilots most important job is to fly the aircraft even if things may be distracting in or outside the cockpit. Discrepancy was determined to be a static source issue and has been corrected. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot taxied onto an area of very soft earth and the main wheels sank into ground tipping aircraft forward striking prop. There was no contact with any aircraft part other than two prop blades, likely due to low speed and idle rpm. The airport ATIS generally provides information about the status of the grassed areas, however there was no information provided at the time of the incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was on their first circuit of the day. On final approach at about 30 feet the pilot identified they were a little off the centre-line so made a correction. The pilot over corrected resulting in a wing drop with insufficient time to commence a go-around. The aircraft impacted right wheel first, bounced and floated in ground effect for a short time then slid on its belly to a stop on the grassed area to the right of the runway. Review identified the pilot had little experience on the aircraft type having only recently purchased the aircraft. Pilots are reminded of the importance of completing thorough type training with an instructor prior to operating solo. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had just completed a dual training flight conducting circuits. During the taxi from the runway the student applied the brake and the nose gear of the aircraft collapsed and the propeller struck the ground. RAAus reminds maintainers of the importance of thorough inspection during maintenance as well as thorough pre-flight inspections by pilots prior to flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot took off from the airport for a local flight in an aircraft which had been recently purchased by the pilot. He had completed a tail wheel endorsement prior to operating the aircraft. During the time the pilot was in the air the wind changed direction and increased in velocity. The pilot elected to land on the most in to wind runway, which was not the runway used for take-off. This runway was closed due to recent heavy rains. On landing the pilot braked too hard and the tailwheel aircraft ended up nose first in the grass runway doing damage to the propeller and cowling. Members are reminded of the need to read weather/NOTAM's prior to flight, even if it is a local flight, as well as looking for signals that may be presented next to the primary windsock (dumbbell/double cross etc). |
