OCCURRENCE DETAILS SUBMITTED TO RAAUS: After completing a number of touch and goes in the Weightshift Microlight (trike) the pilot decided to terminate the flight due to increasing thermal activity. As the final landing was being completed the pilot touched down on the runway rear wheels first. As the front wheel touched, the pilots right foot slipped off the steering pedal. The pilot put it straight back on, causing a small wobble in the aircraft but retained control of the aircraft.
Then the pilot felt the rear left corner sinking and the aircraft started drifting to the left. The pilot tried to compensate, then the aircraft turned 90 deg to the left and stopped 2.5mtrs to the left of the runway on the grass.
An investigation identified the pilot lost directional control during the above landing phase and the resultant damage was generated from side loads applied to the base as it departed the runway.
Contributing factors were changing environmental conditions on this second sortie and further latent conditions brought about by changes to operational procedures at the aerodrome for circuit operations.The pilot was provided advice from Operations on foot positioning and control management and will undertake remedial training and development with the local flight school to mitigate against further occurrences.The aircraft is being repaired by the manufacturer. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: When taking off from a remote airstrip in a fully loaded aircraft the pilot rotated too early and stalled the aircraft. The pilot recovered but at close proximity to the ground the aircraft stalled again subsequently impacting the ground. The pilot sustained minor injuries but the aircraft was substantially damaged. The pilot has been mentored by operations staff on the importance of takeoff technique including Take Off Safety Speed (TOSS). The pilot was required to undergo specific training on takeoff elements at his next BFR. Members are reminded of the differences in aircraft handling characteristics when operating at or close to MTOW and urge pilots to undergo regular type training so they are proficient in all areas in their aircraft. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During landing roll out the left rudder cable snapped. The cable appears to have parted where it passes under a small roller on the floor under the rear passengers feet. This occurred on ground at low airspeed the aircraft suffered a slow speed ground loop.
Zlin Aviation issued Safety Alert No.11 as a result of this occurrence. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: It was the students’ second circuit lesson and it was quite busy at the time. At about 10:30am the dual training flight was cleared for a touch and go to which the instructor assumed was a right circuit. They were approaching their turning point from upwind and started their lookout. As they turned on to the crosswind leg, the tower informed them that they wanted them to climb straight ahead and to now expect an orbit on downwind. They made their radio call on downwind and were told to report ready for base. The circuit after this was uneventful. The incident was reviewed and discussed with school CFI. Changes to training operations have been implemented in addition to a review of the event being conducted with the Instructor to ensure appropriate situational awareness was maintained, which should prevent future occurrences.
An additional factor was major works and current redevelopment being carried at this major regional airport. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst taxiing for take off the pilot of the Hanuman ultralight lost control due to an undetected local gust that was most likely a clear dust devil or thermal release, The aircraft was lifted and impacted a nearby barb wire fence. The brakes on the aircraft would not stop the aircraft due to limited traction at the time. The pilot was uninjured but the aircraft suffered significant damage. The local weather station recorded a 74kmh gust at the time which was not evident in the forecasts for the location. This event occurred in the height of a very volatile summer season with high temperatures, unpredictable winds and fires in the greater area at the time. Pilots should consider the "big picture" in relation to decisions to fly especially during times where there is a high likelihood of unpredictable local environmental conditions. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: Normal movement of the engine cowling in flight allows air (and potentially fumes, fluid smoke and/or flame, if present) from the engine compartment to enter the cabin. Ram air pressure under the cowling causes it to bulge slightly when in flight, creating gaps between the firewall and the cowling. The firewall seal design does not provide any flex to take up and block these gaps. This defect appears to be inherent in the design of the firewall seal. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After 1.3 hours of normal flight in the YSWG training area the aircraft returned to the CCT where it conducted 2 go-arounds’ (only for training purposes) and on the 3rd it landed. As it came to a stop the pilot noted the nose tyre must have been low in pressure. Once it cleared the RWY the pilot stopped to inspect the aircraft and saw the nose tyre had deflated. The aircraft was tugged back to the hangar.
Outcome: No investigation required by RAAus. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Because of the pilots’ weight, (only 25 kg over the minimum for the aircraft) additional ballast in the form of a punching bag weighing 50 kg is fitted in the back seat of the trike, which is restrained with the fitted inertia-reel seatbelt and a ratchet strap because the seat belt is not adequate on its own. The leftover tail of the ratchet strap was wound up and looped on itself the way truck drivers secure their straps, which results in a knotted loop of tape hanging from the ratchet. The nature of the ballast bag has meant that on occasion, the ratchet is to be re-tightened. The resulting extra length of tail was looped over the strap near one of its end hooks so only a short piece of tape with the knotted loop is hanging. It appears the extra tape unwound itself and the knotted loop was pulled to the rear by the Windstream. The tape melted on the exhaust pipe and the knotted loop detached and passed through the propeller. The pilot noted a bump as the leftover tail of the strap hit the propeller, but there was no further indication of damage. The pilot was only two miles from the airfield, so continued on and landed normally. The propeller sustained no damage, as evidenced by the 10 or so minutes the engine ran before the pilot could check what happened. There was no unusual vibration and a visual check shows no nicks or other damage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was being test flown, it was fitted with a new engine. During climb out a loss of power was experienced at 200 AGL. Due to the terrain being less than optimal for an emergency landing the aircraft was flown downwind along a fence line to attempt to make a base turn onto the originating airstrip. When it became clear that there was insufficient altitude or power available for the intended landing, the aircraft completed an outlanding in the only favourable paddock. This was accomplished with no damage to the airframe or pilot. On exiting the aircraft, it was noticed that a copious amount of oil was flowing out of the cowling. On review the engine appears to have suffered significant internal damage. The engine was compression tested showing complete lack of compression on one cylinder blowing air through the crankcase. |

Passing the airport inbound reporting point, the student descended to 1000 feet approaching 3NM inbound and started noticing a blank noise. Student conducted radio failure check to confirm whether they actually had a failure or not. Student took it as a radio failure. Transmitting blind, they squawked 7600 then made call joining downwind RWY17R, did not hear any response. Joining base, the student heard static and then the tower respond and the radio came back live again. The student was advised to change transponder back to 3000 which they did and they were given landing clearance and landed safely. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a dual flight training exercise in the circuit area the tail of the aircraft stuck the ground during a touch & go. The aircraft had already conducted several touch and go's without incident with the student acting as pilot in command. Winds at the time were 7-10 kts from the East. The aircraft was configured for a crosswind landing with flaps at 10 degrees. During the landing flare the aircraft experienced a sudden high sink rate. The student correctly increased power to arrest the sink but unfortunately also raised the nose too high which resulted in the main gear and tail contacting the runaway simultaneously. The high sink rate occurred too quickly for the instructor to take over and correct the sink and high nose attitude. After the initial contact with the runway the aircraft became airborne again and the student called for a go-around. The instructor took over and elected to land and stop due to possible tail damage.
The CFI has since included advice to the Instructors, pilots and students alerting to the potential issues from the proximity of buildings in nominated winds. |
