OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a CFI Flight assessment in the Bribie designated training area the cabin began to fill with white smoke. Both pilots initiated emergency procedures and proceeded to conduct an emergency landing at Caloundra AD. The cabin smoke was focused on the left side and venting was managed as best as possible by both pilots- ongoing checks were conducted for any incapacitation between the pilots and while uncomfortable no deterioration in consciousness occurred.
All electrics and comms were lost and it was identified as a probable battery failure as it had been replaced prior to the flight with a temporary borrowed battery.
A successful straight in approach was completed on RWY 05 in between aircraft operating on RWY12. At no time was engine power lost but rough running was observed at lower rpms possibly due to air filter damage.
Determined Outcome: Inspection revealed the aircraft's Voltage regulator was faulty, charging at 13.8 volts at idle but rising to over 18 volts at full power. This was not a lithium battery compatibility issue. No other systems were found to be damaged as a result of the over voltage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst taxiing to the runway the right main-wheel tyre deflated suddenly. The aircraft was steered in the direction of the deflated tyre until it was clear of the taxiway. On examination of the tyre, the hub-bolts were found to have sheared, but it was not possible to determine whether this was due to the tube failing first, of if the bolts had failed before the tube failed. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot landed outside the area marked by gable markers into resulting long grass, adjacent to runway 27, which caused the nose wheel to dislodge from housing and the prop striking the ground. The pilot has discussed the incident with the local CFI and will ensure all repairs are to standard, including propeller and crankshaft assessment of the engine prior to returning to flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was going to complete a simulated forced landing in a Tecnam. The altitude before starting the simulation was at 3,500 ft. The pilot started the normal procedures to do the simulated forced landing, throttle off, then adjust the attitude to keep a gliding speed of 68kts by trimming and at the same time doing the check of the fuel pump, fuel valves.
After these checks, the pilot started to look for a place to land safely, however they then had the sensation the aircraft was stalling so then put the nose down more and increased the power to level off. Once the aircraft recovered the pilot noted the airspeed was at 120kts. They then cut the power but strong turbulence was experienced. The pilot then made a mayday call to Lethbridge airport and landed the aircraft, resulting in some damage due to long grass in the selected paddock.
The pilot was mentored by the local CFI about his actions, the difference between ground speed and airspeed, how to manage turbulence and decision making. The CFI also provided confirmation of appropriate minimum conditions for solo private hire to all Instructors at the FTS. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a trial instructional flight the aircraft engine was reported to splutter and then stop, the pilot conducted forced landing into soccer field at Cromer Park. The pilot reported that they saw children on the soccer field and flew the aircraft into a fence to avoid the children on the field. Both the pilot and student suffered injuries but were reported to be in good spirits. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The tailwheel aircraft was landed on the main wheels with aileron deflection for crosswind component held during rollout. As speed reduced and the tail wheel dropped, the aircraft was turned into the wind causing a ground loop which ended on the grass windward side of runway.
The pilot and training CFI were both interviewed in relation to this incident. The pilot was on a supervised solo navex and wind conditions had changed significantly on return. A formal debrief focused on personal minimums with respect to cross wind conditions, correct tail wheel landing techniques (wheeler) and currency of flight operations. Pilots are reminded to review and use RAAus resources in relation to Loss of Control prevention and the RAAus Currency Barometer on the Member Portal. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual circuit training, the student had successfully completed three touch and go landings. 0n the forth, at the application of normal breaking the aircraft yawed to the left and the impression of a left flat main gear tyre was predominant. The instructor took over and without further braking vacated the RWY at TWY A3, where the aircraft stopped and shut down. After inspecting the tyre, the indication would conclude a wheel lock-up however there was no impression of a lock up during landing or braking. The earlier touch and go landing also gave no impression of any damage to the tyre or a developing flat tyre. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A pilot certificate holder commenced an instructor rating and decided to fly from the right hand seat to gain experience in their own aircraft. During the landing phase the aircraft hit the ground hard breaking the nose wheel and propeller. RAAus reminds members of the differences in control input feeling and sight picture flying from the instructor seat and to always undergo type familiarisation with an experienced instructor aboard before attempting to fly an aircraft from the opposite seat. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was a solo student flying circuits, in a Sportstar, on runway 14 (into wind runway) with several other aircraft. Another aircraft, a VH registered twin engine Baron, was conducting flying training circuits on runway 03 with student and instructor onboard. After several circuits the pilot of the Sportstar conducted another touch and go on runway 14. When they were airborne over the intersection with runway 03 they saw the Barron pass below them on the ground on that runway.
Pilots are reminded of the importance of "alerted see and avoid" principles as provided in CAAP 166-01(4) and the use of standard radio calls to avoid proximity events at non-controlled airports. |

On the third pass and final pass of landing area they decided grass was too high to land but instead of pulling up and gaining altitude they turned left at low altitude to have a quick look in a paddock alongside which had not scoped out. This resulted in the aircraft colliding with a power line and impacting the ground.
The pilot has recognised the dangers of low level operation away from airports and prepared airstrips and will complete Low Level flight training with a RAAus CFI prior to completing any further out landings in paddocks. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was on late final for runway 24 and the pilot under instruction observed a flock of Galah's on the ground approximately 100 metres in front of the aircraft slightly to the left. As the aircraft continued the landing and began to flare, the flock of birds took flight and traversed from left to right directly in front of the aircraft at a height of approximately 10 feet. The aircraft flew through the flock of birds. Shortly thereafter a loud thump was heard from the rear of the aircraft. The Pilot under instruction landed the aircraft and during the taxi back, observed a single Galah in the middle of the runway approximately where the thump had been heard. The aircraft was parked and a visual inspection showed that there was no damage. The pilots then went to the location of the bird, still on the ground, to assess its condition. A short distance from the bird, it took flight in a normal manner apparently unaffected by the event.
Birds are listed as a hazard at YCFN in ERSA. RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |
