OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had just had an annual service completed. Wind was nonexistent and after about half an hour of ground run to ensure the engine was running correctly, the pilot taxied out for the runway 36. The pilot made 3 starting runway runs given the low time in the aircraft and recent service to prove the engine. This was basically start of take off, lift the tail, cut the motor and drop the tail back down without lifting off. With all Temps and Pressures in the green, the pilot took off on 36 and had no issue or hesitation of the engine. The aircraft began climb out at 75kts and at approx. 300ft, the engine began to lose power and stumble. The pilot immediately lowered the nose to maintain height/speed and turned on the fuel booster pump. Knowing the engine was struggling and the airfield was behind them, the pilot began a gentle S turn to the left to line up for final on runway 18. The pilot then called "Mayday" on 127.6 but heard no response. At approx. 50 feet, the engine cut out and the pilot was able to glide back onto the runway and stop before the fence line. Both Pilot and aircraft were un-damaged from the incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flight the pilot and passenger noticed a loud noise followed by a flapping noise during straight and level flight. The flapping become more violent and was coming from the left side wing. Immediately the pilot pulled power slowed the aircraft and found a place to land, which was a dirt road. The pilot briefed the passenger on what to expect from an emergency landing.
After landing and confirming there was no immediate danger, the pilot and passenger exited the aircraft to find a screw missing from the leading edge position on the left wing root fairing. The first portion of the fairing had come loose hence the flapping heard from inside the plane. The pilot rang a friend in the next closest town to bring some duct tape to tape the fairing down until the aircraft could get back home.
The aircraft took off successfully with no issues and flew back to Jamestown. On landing at Jamestown the pilot removed the tape and purchased a replacement screw and fixed the issue. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After landing the nose wheel collapsed at about 15 knots when lowered onto runway. Review identified that the centring spring didn't return the wheel to neutral and the aircraft landed with the wheel not centred. Previous work had been carried out on the aircraft nose wheel following a previous accident with additional work 10 hours prior to this occurrence. It is possible that this occurrence may be related to maintenance not being carried out in accordance with the aircraft maintenance schedule. Members are reminded of the importance of following the maintenance schedules when conducting maintenance on any aircraft. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was on a routine morning local flight in a tailwheel aircraft. On arrival back at the airport they noticed kangaroos on the runway. They completed 1 pass to clear the kangaroos. The aircraft was landing into a light wind with the morning sun in their eyes. On touchdown, due to the sun, the pilot found it hard to see where the kangaroos were and applied too much braking. The aircraft ended up on its nose damaging the propeller. The pilot was not hurt. RAAus reminds members of the need to take external factors into account when flying and consider that even when flying at a location that is familiar to assess each flight and any potential risks every time. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An Instructor was flying circuits with a student on RWY 30. They were established on final approach when they heard a radio call of an aircraft (C172) turning final. They were about 250 feet and assumed the second aircraft was higher and behind them. However very shortly after this radio call the C172 flew over the top of them while turning onto a short final. A R22 helicopter was hovering off to the side of the runway. The helicopter pilot alerted the pilot of the C172 that there was an aircraft beneath them and the C172 pilot executed a go-around. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After conducting a solo check ride with the student the instructor sent them for solo circuits. While taxing the aircraft the student failed to maintain directional control and caused the aircraft to leave the taxi way and roll into a ditch causing a prop strike and the aircraft to come to a stop with nil injury to the student. the aircraft has a castoring nosewheel, which may have been a contributing factor in the loss of control. An assessment of the student following the incident was completed and any required remedial training was carried out. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft made a straight in approach to runway 13 for a “Touch and Go” and was attempting a Full Stop landing on the second circuit. The wind had dropped off and was almost flat calm on the second approach. The approach was a bit high and the aircraft floated more than usual. The flare was also high and aircraft drifted to the left. As the pilot decided to go around, the aircraft sank heavily onto the runway and bounced to the left, becoming entangled in the barbed wire fence at the side of the runway and coming to an abrupt stop. The pilot was unhurt and was able to turn off fuel, radio and electrics and exit the aircraft via the right side (passenger) door, because the LH door was obstructed by barbed wire. The barbed wire fence had to be cut away to extract the aircraft.
Subsequent to this accident the pilot undertook a review flight with an RAAus examiner at the same landing area and it was determined that the pilot did not recognise a quartering tail wind effect at the beginning of the threshold, believed to be caused by high cane field crops on the approach to the SE runway. It was recommended that future landings move the initial aiming point further into the available landing area which was more than adequate for the aircraft type. Delayed reactions and decision making were also identified as further contributing factors to this event. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While conducting a landing at an aerodrome, the aircraft struck light fixtures to the side of the runway and hit a fence. The pilot noted possible confusion within the cockpit resulted in the throttle being reduced to idle during the go-around. The accident was not reported within the required 72 hour timeframe.
RAAus reminds members of the importance of clear communication by the pilot when carrying passengers and the requirement under Section 4.08 of the RAAus Flight Operations Manual to report all accidents and incidents to RAAus which fulfils the reporting requirements under the Transport Safety Investigation Act (2003) for the Australian Transport Safety Bureau requirements. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After having maintenance completed on the aircraft, the pilot conducted two touch and goes before departing to ensure everything was running smoothly. Everything was fine on the two touch and goes so the pilot departed the field and started tracking for YCVA. Roughly 42 NM into the flight 0.4hrs the pilot started to smell a burning smell from the aircraft. It was at this point they powered back slightly and the smell went away. Shortly after this they noticed the aircraft was losing oil pressure rather quickly and they noticed the burning smell again. The pilot immediately started looking around for places to land and realised they were about 8NM from a friends’ airfield and decided to land there.
The pilot landed with no issues and taxied up to the front of the hangar. They exited the aircraft to find oil sprayed all over the left hand side of it, from the nose wheel the whole way along the belly and main wheels to the elevator. The cowling was removed to see where the oil was coming from. At first it was not clear as there was a lot of oil spread around. The pilot then rang the maintainer to tell them the plane was covered in oil, the burning smell and that the oil pressure dropped during the flight. The maintainer advised the pilot to return so they could look what was wrong. During the call the oil was found to be coming out of the rocker oil tube. The pilot told the maintainer that the rocker oil tube is visibly loose and oil was definitely coming out of it. The maintainer then replied that they forgot to do up the rocker oil tube. They then instructed the pilot how to do up the rocker oil tube, replace the oil. (they had 2x w100 piston engine oil bottles with them). After the pilot did this and cleaned the oil off the aircraft they completed a successful engine run up on the ground. They then flew directly back to Jamestown rather than stopping at Clare valley as originally planned. The flight back to Jamestown was uneventful and once landed, rechecked the oil and it was still at an appropriate level. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Following pre-take off checks in the run-up bay to Runway 36, the pilot lined up for take-off after touchdown of another aircraft, which landed long and proceeded to taxi to the runway exit point (which is the cross-strip 05/23). The pilot had clear forward sight of the other aircraft and upon seeing it commence its turn onto Runway 23 but did not waiting for a "clear-of-duty-runway" radio call. The pilot judged that it was safe for them to depart and gave their "rolling" call and commenced take-off.
For the future, the pilot will ensure a call advising clear of runways is received and will confirm the runway is clear before proceeding. |
