The aircraft had about 48 litres in the fuel tanks. A preflight check was conducted with nothing unusual noted. Both single mag operation was normal and preheat operation and idle were normal. The pilot taxied with the flaps set to take-off and the auxiliary fuel pump on. The take-off and climb were normal and at about 300ft the pilot turned off the auxiliary fuel pump. Everything appeared normal and the plane was climbing quite well due to the light fuel load. The pilot then retracted the flaps to normal flight position.
Whilst flying at about 75kts the engine then began to miss, run rough, and loose power. The pilot immediately lowered the nose to maintain airspeed and reduced throttle to about half. The aircraft was at approx. 350-400ft, the airspeed was maintained at about 70kts. The pilot returned to the airfield and entered in a right hand circuit, normally it is a left hand circuit at MKT. The pilot called pan pan indicating to the problem to other traffic.
The pilot made an uneventful landing with the engine still running.
DETERMINED OUTCOME: This occurrence has been referred to CASA engineering and airworthiness branch and the manufacturer for further monitoring and action where required. |

JRCC contacted RAAus Operations to advise ATC at Williamtown had assisted an aircraft out of IMC in their area. They successfully assisted the pilot to maneuver out of cloud into VMC. The pilot was then reported as safe on the ground.
DETERMINED OUTCOME: The pilot has been required to undergo remedial training before any further cross country flying can occur. |

While conducting a simulated engine out during training the student pilot flared late and the powered parachute bounced. A gust of wind at the time of the landing contributed the nose wheel being lifted higher than normal causing the prop to strike the ground.
DETERMINED OUTCOME: While conducting solo consolidation flights in a powered parachute the student applied flare late and did not arrest the sink rate sufficiently, resulting in the propeller striking the ground. Pilots are reminded of the importance of assessing the flight path and being prepared for a go-around if the landing profile is not correct. |

Outcome: While flying near a controlled airspace step, the pilot allowed himself to be distracted and penetrated the lower limit, requiring Airservices to re-route a QANTAS flight. The pilot has voluntarily suspended himself from navigation until a flight review with a CFI has been completed. Pilots can significantly reduce the risk of violations of controlled airspace by ensuring they reference current charts and use known geographical features, along with careful pre-flight planning to confirm the lower limits of airspace. |

On late final approach to landing on RWY14 approx. 80' AGL and at about 60kts two Ravens flew up from the trees with what appeared to be an intention to 'attack' the aircraft. When about 10-15m in front of the aircraft and at the same altitude as the aircraft the birds seemed to re-assess the wisdom of their 'attack' and folded their wings to fall away. One, however, misjudged and ended up colliding with the leading edge of the starboard wing. A ground search could not find the bird so it probably wasn't killed. An inspection of the wing leading edge showed no damage (0.8mm plywood).
Outcome: RAAus continues to gather statistical data on birds and wildlife that interfere with aviation activities around the country. It is particularly important that these incidents are reported so that RAAus can identify problematic areas and notify the relevant parties. |

Bird strike: During a touch & go with a student, a bird flew up into flight path after take-off and a loud bang was heard. The take-off was aborted and the aircraft was landed on remaining RWY. The aircraft was checked for damage and none was found.
Outcome: RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |

Storm cells with strong winds and hail passed over Narrabri airport. The aircraft was parked at the northern end of the airport buildings and was tied down to stakes driven into the ground, facing a SSW position. An inspection after the storms passed found a large number of dents on the top surfaces and the RH side of the fuselage.
Determined Outcome: No further action required by RAAus. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot went to check the oil level (cold engine) during pre-flight, the dipstick indicated 3/4 full when first removed, they then wiped the stick clean and proceeded to drop the dipstick back in to check the level. The metal rod then separated from the plastic screw cap as it was dropped back in. The rod went into the sump and the screw cap fell into the engine bay. Both parts were retrieved and put them back together.
Outcome: This defect has been reported to CASA. |

During a circuit lesson with a student, after a touch and go landing the engine began to malfunction at about 150ft. The instructor immediately took over and landed the aircraft on the remaining runway straight ahead.
DETERMINED OUTCOME: It was identified that a rubber seal in the Carburettor had failed. This issue has been reported to the supplier and CASA for ongoing monitoring. |

An aircraft inbound to YMMB via Carrum with student was instructed to join a right downwind for RWY 17L. The student read back instruction correctly but then began to track towards a left downwind for 17L – the instructor failed to correct student at this time. Upon recognising incorrect approach route, the tower queried scenario and advised that although they had appeared to join downwind on the wrong side to continue the left downwind for 17L. There was no incident of close proximity to other aircraft.
DETERMINED OUTCOME: CFI advised that YMMB TWR have changed phraseology in circuit joining instruction and there have been no further incursions of this type. IM satisfied that this matter has been effectively resolved at operational level at Moorabbin. |

Airspace infringement: The pilot misread the restricted airspace ceiling in flight planning due to double printing on chart. ATC contacted the pilot 3NM inside Amberley and directed them out of the control zone to the NE.
A full briefing and ground based review was conducted with the PIC by Asst Ops Mgr to clarify flight planning requirements, understanding of airspace classifications and approved source data for flight planning purposes. The pilot was also shown the limitations of current electronic flight planning software in relation to NOTAMS and defining the status of restricted airspace. Further explanation of occurrence reporting requirements and processes were also reviewed.
The Operations Manager conducted a separate review with the school CFI on reporting requirements and amended process have now been adopted. |

The pilot was approaching to land at Mt Jack Station, they were using the rudders to maintain runway alignment and noticed the right rudder had no response to inputs and only had left rudder control. The aircraft was almost at touchdown when the aircraft did a sharp turn to the left and the pilot attempted to control the aircraft keeping the wings level but the aircraft landed off the airstrip and flipped over coming to rest inverted.
Recreational Aviation Australia requested technical assistance from the Australian Transport Safety Bureau in performing a detailed examination of the aircraft’s rudder control cables and associated rigging (pulleys). The purpose of this examination was to identify the factors that contributed to the damage and subsequent failure of the cable. When returning to Charleville airport the pilot noticed a sudden loss of oil pressure - aircraft engine was still behaving normally but they performed a precautionary landing anyway as there were not many options to land had they continued. The pilot made the landing onto a rough dirt road and lost the nose gear which pulled the aircraft off the road resulting in a flip over. The pilot and PAX were uninjured.
Outcome: Incident has been refereed to Airworthiness and Engineering branch of CASA for ongoing monitoring. |
