While instructing a student on circuits and touch and go, the aircraft encountered rapid rate of decent on late final. Full power was applied however it failed to arrest the decent and the aircraft struck the RWY heavily collapsing undercarriage.
Outcome: The flight conducted was compliant to all requirements as a training flight for a returning pilot converting to RAAus. Various pilots reported changeable conditions on both runways with varying degrees of turbulence, with wind gradient and associated sink. The instructor acknowledged that a combination of delayed response from the student to an unacceptable approach profile and the conditions at the time created a scenario that the instructor was not able to effectively regain control from. Further contributing factors identified were the performance characteristics of the particular aircraft variant at low airspeeds and an approach airspeed that was within but at the lower end of the recommended range.
The CFI has reviewed the accident and has conducted a preliminary post accident flight with the instructor and will conduct further slow flight and control management exercises as part of ongoing proficiency checks. |

During dual instruction, the student gave a 'downwind' call. Then immediately after they heard a call "Redcliffe traffic YMV overhead Beachmere at 3500 on descent inbound" there was no mention of 'joining base' in that call. The student was on late downwind 07 and about to turn base when a VH registered aircraft suddenly appeared at their 1 o'clock high. The aircraft used for instruction is high wing limiting visibility of other aircraft from above. It was approx. 200m from them, on steep descent and very fast. The VH aircraft suddenly pulled up when they saw them and took evasive action to go around. The instructor radioed to ask if they had seen them however heard no reply. The VH aircraft then re-joined the circuit crosswind and gave a call joining crosswind as the instructing aircraft was on final.
Outcome: This matter has been referred to CASA Sport and the APF. |

The pilot experienced a loss of control on landing resulting in a ground loop the left, the right wing dug in pulling the aircraft around to the right. The left undercarriage then collapsed, the left wings dug in and separated from the aircraft. The aircraft then came to rest standing on its nose.
Outcome: An investigation found that the members BFR and medical declaration documentation was not current. This issue has been rectified. no further action required. |

The pilot was on the seaplane boat ramp Rathmines determining the best way to negotiate the flotilla of boats and random swell in the middle of the Rathmines inlet for a departure. A strong NE wind was blowing causing considerable swell. Approx. 10 boats had been moored to the east of the ramp along the foreshore and extending approx. 300m from the foreshore into the waterway. By the time the pilot chose to depart there was only one moored boat remaining which was positioned about 500-600m to the NE of the ramp and approx. 300 m from the shore. With a water take-off run of 250m in my a/c the pilot planned to follow smoother water in a path about 150m from the shore which would mean that they were airborne at least 250m prior to the moored boat and would pass at altitude, approx. 100-150m abeam. Whilst on the plane and just prior to becoming airborne a series of random waves hit the seaplane which caused it to change direction some 15 degrees towards the moored boat. At this stage the pilot was about 300m from the boat and they considered there was still a sufficient safety margin, but having never rejected a water take-off at high speed, elected to continue. The seaplane became airborne and started to climb but once clear of the tree line height on the foreshore, it was buffeted by wind and turbulence which affected the climb performance and even though airborne and at a height of about 50', this buffeting pushed it closer to the moored boat.
Whilst there was no incident, the reason for writing this report is that the pilot recognizes that there was probably a 'surprise' factor for anyone on board the boat. The pilot also acknowledges that although the unknown can't be taken into account, a better departure route may have been considered.
Outcome: While conducting a take-off in a waterway with significant boating activity and confined area, the pilot realised the effect of swells and mechanical turbulence and shielding from trees on the nearby shoreline resulted in the aircraft operating closed to a moored vessel than was ideal. Waterborne operations require consideration of additional factors such as other vessels, the effects of swell and mechanical turbulence and this pilot is to be commended for reporting and highlighting these considerations. |

During touch and go’s, on the third CCT the pilot decided on a glide approach. They completed pre-landing checks early downwind and made glide approach radio call abeam southern threshold, whilst downwind. The pilot kept to an aim-point 1/3 down RWY 36, flared, then touched down somewhere between 1/3 and 1/2 way along RWY 36. They applied the brakes firmly, felt some slowing, but then thought that slowing ceased, concluded that brakes had locked. The pilot released the brakes to unlock and tried applying again quickly, but less firmly. The pilot judged there to be approximately 200-300m remaining of RWY, so kept trying brakes then realised at about 150m to end ditch that the aircraft was still moving too fast. The pilot was holding full back on the stick whilst applying brakes and tried turning slightly left to increase available run as it was too late to go around, so kept trying brakes to no avail and the aircraft ran into the ditch.
Determined Outcome: RAAus are continuing to develop learning materials and training aides for pilots to aide in reducing the number of runway los of control (RLOC)occurrences. This accident is a timely reminder to pilots of the importance in picking go/no go points for touchdowns and go around procedures to mitigate the risk of RLOC. |

An instructor was conducting Stalls with a student in the Eastern Training Area, and believed to be in correct area up to 3500ft. Upon their return to Archerfield they were advised that they had infringed CTA.
Outcome: The pilot has indicated their acceptance of this mistake and learnt from it. No further action required from RAAus. It is essential that members report airspace infringements (AI) to RAAus no matter the severity. These incidents allow RAAus Safety to analyse AIs at a national level and communicate with AirServices Australia and the Military regarding airspace violations, improvements and access. |

The aircraft was on the apron 30m from the Club House warming up the engine. The park brake was applied lightly and the revs were increased. The aircraft moved forward and the pilot reached to apply more brake. In doing so their right hand became entangled in the passenger seat belt. The pilot then took their left hand off the throttle to untangle their right hand, and inadvertently bumped the throttle which increased the revs dramatically. The aircraft surged forward and veered to the left. Before the pilot had time to untangle their hand the aircraft had stuck the building.
OUTCOME: No further action required by RAAus. Pilots are reminded to ensure all items are secure and that no objects will restrict on the safety operation of the aircraft. |

After take-off, being about 200-300 AGL, still climbing a vibration started and significant power was lost. Once power was lost, the pilot set the glide attitude, doing some of the trouble checks, slipping the machine to try and get it to the runway first. Once it was obvious the aircraft wasn't going to make the runway, the pilot selected a field and landed in it.
Outcome: Some very minor wear in one of the carburetors was identified. 2 new carburetor kits were installed and the aircraft has been operating since. An inspection of the suspect carburetor identified that it had flooded causing the engine to stop but the internal wear didn't actuality appear to be the cause. No further action required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During CCTs the pilot noticed an engine vibration and made full stop landing. An investigation revealed low compression on No 1 cylinder, further investigation revealed a cracked cylinder along a machined surface. All cylinders were replaced with replacement cylinders supplied by the manufacture which included a modification to the machined surface.
DETERMINED OUTCOME: This issue has been referred to CASA Airworthiness and Engineering section. As it is a registered LSA RAAus is limited in what it can do to resolve this issue. |
