The pilot encountered radio failure inbound from Carrum. The pilot tried troubleshooting e.g. changed headsets, turned radio on/off, reduced power load and tried contacting the tower several times to no avail. The pilot then squawked 7600 and continued inbound via Carrum to RWY 17R.
Outcome: The pilot in command followed the correct procedures for radio failure. The pilots actions ensured the safety of his aircraft and other airspace users was maintained. |

Coming in to land on RWY24 after a 45min local area flight with in the training area. Approach was normal until short final when the pilot went to reduce power could not get the rpm below 3000. After checking the friction lock was off and throttle fine tune was fully reduced the pilot decided on a glide landing, rather than to go around and possibly have further engine issues. The pilot turned off the engine, applied full flap and landed normally, although slightly long and rolled to RWY intersection and turned off onto TWY. When restarted, the engine rpm was still at 2000 and would not go lower. The pilot taxied and hangared the aircraft, noting issue in maintenance log and reported it to aircraft owner and local maintainer.
DETERMINED OUTCOME: The aircraft has been looked at by a L2 / LAME and it appears the issue was due to wear and tear of the throttle cable. The cable has since been replaced. |

The aircraft engine surged 5 times during climb out, at approx. 3500 ASL. The pilot returned immediately to Canberra airport, no further problems during remainder of the flight. The pilot ran an engine check after landing, magneto test & carby heat, all were OK. On close examination, the underside of the carburetor was found to have extensive fuel stains. New carburettor installed. No further issues identified. |

Normal take-off on RWY05. At approximately 300ft the engine suffered significant loss of power accompanied by vibration/rough running. With insufficient runway remaining to land, performed a gentle turn back towards the threshold of RWY14. The pilot found that with reduced throttle the engine could maintain about 1800rpm. The pilot elected to perform a tight sweeping approach back onto RWY05 (which was more into wind) rather than land downwind on 14. The landing was uneventful. Upon shutdown oil was found leaking from around the exhaust manifold #4 cylinder, the plugs on #4 cylinder were totally oiled up.
DETERMINED OUTCOME: This occurrence has been referred to CASA engineering and airworthiness branch and the manufacturer for further monitoring and action where required. It appears a valve failed. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: The aircraft owner reported that the seat back locating track had torn metal. An investigation disclosed that on pilot's side one track was pulling the anchor nuts through the floor pan. There was then distress discovered with all of the rail attachments. The floor pan and structure below the floor is of .020 material and not strong enough to take much load.
OUTCOME: This has been reported by the RAAus Technical Manager to the manufacturer and in time they will respond. The owner has also been advised to register their concerns directly with the manufacturer to ensure the issue is resolved to satisfaction. |

The pilot maintained too much pressure with their right foot on the rudder, resulting in the aircraft veering sharply to the right and hitting a tree sustaining minor damage.
DETERMINED OUTCOME: A detailed ground based review was conducted with CFI to revise strategies for control management and authority during critical flight phases. The student training records were reviewed and prior training deficiencies discussed. The aircraft has subsequently been fully repaired by the manufacturer and returned to service. |

Climbing through 300ft AGL after take-off to the SE from the western grass at RAAF Richmond a slight change in engine note and rpm reduction was detected without corresponding throttle movement. A right turn to improve the forced landing options available was commenced and approximately ten seconds later the engine failed abruptly. An uneventful right circling glide approach to the cross runway (RWY28) was made and touchdown occurred with 4000ft of RWY remaining. The aircraft was hand towed to the gliding hangar and it was noted there appeared to be no compression. Removal of the cam cover revealed that the cam chain had derailed from the cam chain sprocket.
Outcome: The only damage to the aircraft is the cam chain fault which caused the stoppage of the engine. Both the inlet and the exhaust valves appear to be closed, although the lack of compression would indicate partially opened valves. The engine is a 2 stroke and was maintained by the owner per the book. It appears fair wear and tear contributed to the engine failure. |

A student’s first solo at Bendigo Airport after completing CCTs with a Senior Instructor was incident free until the flare. As the aircraft entered the flare the nose yawed slightly to the left and the student immediately attempted to correct this with right rudder (which felt a little stiff). As the wheels hit the runway the aircraft banked to the right scrapping the right wing before tilting the nose forward and contacting the prop with the tarmac.
Determined Outcome: The training practices of the flight school were reviewed with the CFI and confirmed that appropriate guidance is provided to flight instructors via Standard Operating Procedures on risk mitigation and aerodrome familiarisation for new solo pilots. The training notes were reviewed and all familiarisation and competency assessment was completed satisfactorily by the authorising Senior instructor. The pilot was reminded to operate the aircraft in accordance with the Pilot Operating handbook with regards to flap selection and it was confirmed the pilot had been assessed as competent in all landing configurations. |

The PIC of the Jabiru noted they were positioned outside the gable markers in the locally accepted location for run-ups. There are no taxiways or run up areas at Caboolture and no radio calls were received from the Cessna pilot. |

A student was building solo hours in the circuit. After approx. an hour of circuits the student had a hard landing where the nose wheel of the aircraft collapsed and subsequently the prop struck the runway. The impact brought the aircraft to a sudden stop on the RWY.
Outcome: No further action required from RAAus. Instructor has taken the appropriate action to remediate the student on the deficiencies. |

A student left the pitot cover on during pre-flight and did not recognise it was left on after engine start or taxi. The student is nearly at the stage for a flight test, and the instructor wanted to see how they reacted to not having an alive ASI on the take-off roll. The student received a taxi and airways clearance for circuits and began to taxi. While waiting at the holding point, a second aircraft that was waiting behind contacted them on ground frequency advising that the pitot cover was still on. The instructor thanked the second aircraft, and then requested clearance from the ground controller to exit the aircraft and remove the cover.
Outcome: This is an unsafe act and the CFI has been directed to suspend this practice immediately. |
