OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During 100hr inspection. Rudder mass balance weight was found to be broken. Weight and arm had separated (mounting tube failed next to weld) and was floating free in top of fin.
DETERMINED OUTCOME: This has been referred to the manufacturer for further investigation. |

The engine started to run rough and vibrate and lose power immediately upon turning final for the runway. The pilot raised the flaps to make the runway and performed an uneventful landing.
Determined outcome: The tapered plug that covers up the access hole for the bottom cylinder head bolt on cylinder number three came out and worked it's way between the rocker arm and rocker cover, jamming the valve open, which caused the push rod to jump out of it's socket. |

Airspace infringement: Flying on final leg, from YPKS to YGDH to YSPT, around YIVL there was a build-up of clouds and it was initially decided to land at a suitable airstrip in that vicinity. It had been relayed back to the pilot that the cloud was only patchy and there were plenty of holes, so they proceeded above the cloud to a level of 9,000 feet. The pilot found no holes to be seen so then decided that to contact Brisbane centre re a clearance. At that time as a code was read back to the pilot but it was garbled and unreadable. The pilot then changed headsets and radio to source the problem and then responded to Brisbane centres calls and established contact and received a transponder code, proceeded through the clouds and landed at YCAS for fuel, and the pilot rang Brisbane Centre by mobile and proceeded with clearance via YBCG to YSPT.
OUtcome: RAAus have investigated this incident and provided remedial training as necessary. This incident demonstrates how ATC can assist pilots when in need of assistance. |

The aircraft was flying back to Batchelor from Crab Claw Island. At 2000ft having just come out from under the 2500ft Darwin airspace step when the aircraft engine suddenly seized over thickly wooded country. The pilot trimmed for best glide, identified a potential landing site in a clearing a couple of miles to the north and called 'Pan Pan' on the Batchelor frequency citing complete engine failure and location. The pilot tried to restart the engine without success. Best glide speed was maintained and repeated radio calls until a response was received from other area traffic that the call had been relayed to Brisbane Center. The aircraft touched down in a clear area of tall spear grass, but the underlying surface was waterlogged and boggy. Aircraft rolled about 30m and decelerated rapidly, but as it slowed the wheels sank into the bog. The aircraft flipped forward and came to rest inverted. Pilot and passenger both exited the aircraft uninjured. |

During solo flight training the aircraft was in a straight and level cruise attitude configuration when a sudden gust of wind shook the aircraft, and the doors of the aircraft simultaneously bowed inwards and the glass on the door broke inside of the aircraft.
Determined Outcome: This incident has been investigated by the RAAus Technical and Operations team. Further remedial action has been taken for the pilot in control and technical related aspects of the incident have been addressed. This is an isolated issue and has been rectified by RAAus and the associated Flight Training School. |

Although the pilot downloaded and read the NOTAMs for this flight, they overlooked the activation of procedures for AIP (SUP) H16/18. As a result, they were unaware of the need to lodge flight details for ADFIZ Delta and the activation of a Temporary Restricted Area which effectively closed the lane Victor 1. The pilot did not lodge a notification of flight prior to entering the ADFIZ Delta and inadvertently entered a Temporary Restricted Area at Long Reef while tracking to Victor 1.
OUTCOME: RAAus have investigated this occurrence and provided remedial action where necessary. RAAus continues to engage with ASA on trying to find ways to make NOTAM briefing easier. |

DETERMINED OUTCOME: An investigation identified that the wheel had small fatigue cracks which led to the failure. Members are reminded to follow the factory's system of maintenance when servicing and should conduct a thorough pre-flight daily inspection to reduce the likelihood of these types of occurrences. |

The aircraft was returning from to YSBK via TWRN from the training area. At TWRN the instructor made an inbound call to YSBK Tower on 132.80 and didn't get a response, though they could still hear the tower and other aircraft. The instructor made 2 more calls with no response then started flying away from the airport, they made a call on ground frequency with still no response. The instructor tried re-plugging the headset, and got the student to transmit but no response. The flight instruments display was flicking on and off.
The instructor turned the master off and on. The flight instruments display now stayed off, the radio and transponder came back on but still wouldn't transmit, and now the radio wasn't receiving any broadcasts either. The instructor flew back to TWRN, set 7600 on the transponder and called YSBK tower on their mobile phone. The tower instructed them to overfly, join downwind 29R then land without light signals.
After landing, the instructor turned towards the right exit on A4, then the aircraft stopped and wouldn't move forward while still on the RWY. The instructor found the park brake had come back on. They turned it off but the plane still didn't move. The aircraft was shut down and exited.
The brakes were inspected and they appeared to be stuck shut. The instructor and student pushed on the aircraft and it eventually started moving forward and went off the RWY onto the TWY. An unsuccessful attempt was made to restart the aircraft, it appeared to have a flat battery. With the assistance of the airport ground crew, the aircraft was pushed back to the hanger.
Outcome: RAAus Technical team have reviewed this incident and determined that there is no action required by RAAus at this time. |

DETERMINED OUTCOME: It is likely that this occurred as a result of the aircraft landing on its nose wheel. This type of landing would commonly cause the nose wheel to collapse. RAAus will not be investigating the technical factors of this incident at this point in time. |

During solo circuit training, approaching the RWY for a touch and go, the aircraft was travelling at 55-60kts when the wheels first made contact with the RWY which caused the aircraft to bounce a few feet above the RWY. The student held the nose up expecting the aircraft to touch down but the second time the wheels touched the ground the aircraft bounced approx. 10ft off the ground and the right wing tip lifted high. The student then applied full power to climb out however as the right wing was high the aircraft banked to the left. The student then tried pulling the elevator back to lift the aircraft and apply right rudder, the aircraft kept banking to the left at level height approx. 10-15 feet AGL whilst turning a full 180 degrees and then dived into the ground nose first with the left wing tip down. The aircraft came to rest approx. 20m to the West of RWY 32 Gympie Aerodrome.
This accident has been investigated by RAAus operations and managed at the local level. This incident has been used as part of the professional development programs to raise awareness for instructors and students the importance of early decision making regarding aborted take-offs and/or go-arounds. |

After 3 circuits completed satisfactorily power increased to full for take-off. At that point the nose wheel strut collapsed and sheared off. Immediately power was cut. The aircraft slid, remaining upright, for about 10m before coming to a stop on the RWY. As the aircraft came to a stop the engine and fuel was shut off and the aircraft evacuated.
DETERMINED OUTCOME: This aircraft has been removed from service and will be repaired by the manufacturer. No further action required by RAAus at this point in time. |
