OCCURRENCE DETAILS SUBMITTED TO RAAUS: The fasteners on the cowl became loose in flight. The pilot requested a priority landing made a full stop. They then taxied to the run up bay and checked the cowl, there was no visible damage so taxied back to parking and had the aircraft checked by engineer.
Subsequent investigation revealed the pins the fasteners pivot on had broken resulting in failure to remain secured.
The school has implemented additional processes to regularly check these pins. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On completion of a solo training flight, on landing the student thought the rudders felt harder than usual and had difficulties controlling the aircraft after touch down. This resulted in the aircraft running off the edge of the RWY and impacting with the gable marker.
Subsequent investigation revealed the Service Bulletin regarding routine rudder cable replacement had been completed, with no identified issues. The student completed routine dual checks. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting aborted take-offs at Bankstown airport for training purposes, ATC instructed the pilot to clear centre due to traffic which they did. After clearing the RWY ATC stated that they had tried to contact the pilot prior however the pilot had not heard them resulting in another aircraft having to go around. OUTCOME: Operations reviewed this with the CFI. Change in radio procedures has been applied to ensure students have confirmed and monitor 2 VHF comms. with tower whilst under airways clearance. Aircraft radio serviceability was also checked by maintenance authority. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student on their first solo completed the circuit and their approach was good, however the landing was a little fast and a bit flat. On roll through they over controlled and left the RWY. The student got the aircraft under control very quickly and re-entered the RWY safely. There was no damage to the aircraft.
Operations have reviewed this report and determined no further action required by RAAus. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Approach on 28R YBAF, just post flare, the aircraft dropped causing bounce and prop strike (though prop strike not realised until after landing). Upon loss of lift and subsequent bounce, the pilot immediately applied power for go around and enacted an uneventful landing. OUTCOME: Operations made a formal request for the PIC to complete a flight review. The identified primary cause was positioning of aircraft for most convenient approach and disregarding ATIS information regarding wind conditions. Contributing factors include normalisation and impatience at the end of a long business related flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After landing from their second circuit solo a student vacated at TWY B RWY 35R and was instructed to hold short of RWY 35L. The student came to a stop but had crossed the yellow hold short line. This resulted in a runway incursion. An aircraft on final went around and the student was given clearance to cross the runway.
Procedures at the FTS have been reviewed by Operations and the CFI, with changes to Instructor processes to reduce recurrences. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft engine seized at approx. 80ft above RWY. The aircraft landed on the remaining RWY without further incident.
Initial causes leading up to the failure are undetermined however the final cause of engine stoppage was the number 1 con rod bearing cap separation and piston skirt failure after number two main bearing seizure. Indications leading up to this failure were a rapidly increasing engine oil temperature and sluggish engine acceleration. No further action required |

After reviewing the brief of evidence and the RAAus report, Deputy State Coroner determined that the cause and manner of the deaths were able to be established, and that there was no basis to conduct an inquest. On 11 December 2019 they formally dispensed with holding an inquest. |

The electric pump was turned on and the engine returned to life, however it wouldn't keep pressure up without it. OUTCOME: Primary cause most likely vaporization of the fuel due poor quality. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst ferrying an aircraft to Wakefield from Cowra there was a planned fuel stop at Dubbo, however as the aircraft approached Wellington the engine began to misfire. The condition gradually worsened so the pilot decided to fly to Bodangora airfield to make an emergency landing.
The pilot had phoned a fellow pilot who lives in Wellington and had been informed of the drag races being conducted on the day. The engine continued to misfire as the aircraft approached, the pilot radioed twice that they were to make a straight in approach on RWY 09 but received no acknowledgement of the calls on Unicom 126.7. The aircraft landed without incident.
Follow up with the aircraft owner revealed the spark plugs were replaced, with the engine operating normally as a result |

The event actually started with a departure from land, with no change in wheel position. Distraction during the landing phase resulted in complacency regarding completion of the checklist.
Waterborne pilots are recommended to use triggers to conduct pre-landing checks at nominated points in the landing phase, including a final check for surface, confirming wheel position at that time. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting a check ride, the approach airspeed decayed to 50kts in gusty conditions and the aircraft was caught by a wind gradient, dropping it onto the ground heavily. The left undercarriage leg was bent. Upon taxiing back to the hangar the left leg failed completely and the aircraft came to a stop. It was reported after the event from ground observers that the aircraft suffered a serious gust of wind at the time of landing that also upset other aircraft parked on the ground nearby.
No further action required by RAAus. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight training the instructor was to demonstrate a simulated engine failure at 100’. The take-off was normal and at 100’ they reduced the throttle to the idle position and set glide attitude, however they descended at a much faster rate than normal resulting in the aircraft bouncing on the RWY and the NW collapsing.
Instructors are reminded to ensure the aircraft is managed carefully when maneuvers of this type are attempted. RAAus aircraft have significantly reduced inertia compared to heavier CASA registered aircraft. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During 100 hourly inspection cracks and a split undercarriage bulkheads were found. On further inspection and removal of underwing skins on LHS found a very poor repair of earlier damage from prior to current owner.
This aircraft sustained damage to this exact area at 91 TTIS, it was repaired at the Brumby factory and returned to service. Unable to determine the cause of this current damage however appears consistent with a hard landing. Repairs carried out by a qualified LAME In accordance with the instruction and repair scheme from the factory. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Previous radio issues with aircraft. The radio cables/antennas have been replaced and needed to be checked. This particular flight was a circuits lesson with a student.
All transmissions were normal and standard until the fourth circuit where we were not receiving any transmissions from tower. Troubleshooting was attempted with no sucess. The tower gave the green light for the landing as no radio contact was able to be made. |
