During a solo training flight, the pilot was too high on approach so lowered the nose to reduce height resulting in an increased airspeed. Flared late causing nose wheel to hit the ground first, this action resulted in 3 bounces, the third of which bent the nose wheel and reduced sensitivity of rudder controls. Went around and was intending to try again but was directed to the training areas while circuit cleared then came in to attempt landing on RWY 23. The first attempt resulted in a go-around, the second attempt was successful however the aircraft ran off the RWY strip due to compromised rudder control.
The pilot reviewed appropriate approach speed management and techniques verbally and practically with a Senior Instructor |

OUTCOME: Pilot identified a number of contributing factors based around his actions and specifically bouncing on landing and choosing only 1 stage of flap combined with the effects of wet grass on this 360m landing strip. |

Whilst on very short final for runway 31R, a flock of birds took off from the ground and flew across in front of my aircraft. One impacted low on the front facing radiator below the propeller. A full stop was made. No damage was visible.
Outcome: RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |

Whilst refuelling an aircraft with a 20L plastic Jerry Can, the spout came loose and the can was dropped spilling a significant amount of fuel and splashing it in the pilot's face.
RAAus urge all pilots to use extreme caution when refuelling from Jerry Cans.

After take-off, as the aircraft became airborne the pilot noted the RPM was lower than normal however had previously shown to be adequate to become airborne. On the downwind leg, the pilot completed trouble shooting checks (magnetos, fuel taps, quantity) and turned early base to be near to the end of the RWY. Turning on final, the engine stopped dead. The pilot pulled back to best glide speed but it became clear that the aircraft would not make the runway.
There were small trees ahead so the pilot aimed for a large lignum bush alongside to cushion the impact however the starboard wingtip clipped a small tree. This had kicked the aircraft sideways before hitting the lignum bush. The nose of the aircraft impacted the ground next to the bush less than 15m from the tree clipped initially. The aircraft came to rest about 45° to the direction of travel with the tail up on the bush and nose on the ground 90m from the RWY. Pilot reported minor injuries.
Determined outcome: Conditions on the day would have supported carburettor ice. The PIC advised he may have inadvertently closed the throttle which may have caused the engine to stop on short final. |

During a high speed taxi run down the strip the aircraft lifted from the airfield about 20ft. The power was cut but there was insufficient RWY safety to land and roll the so the pilot decided to apply power for a go around. At approx. 150ft the during the climb the engine stopped without warning. The pilot applied nose down control in maintaining correct airspeed and level wings, and landed about 20° off the RWY heading into a field. After landing shut off fuel and power, opened the canopy and walked back to the airstrip. OUTCOME: The pilot has provided the appropriate post incident analysis. The pilot was appropriately qualified to conduct the test ground runs and pilot the aircraft and the aircraft had appropriate provisional registration. |

The aircraft experienced a partial engine failure at 450ft on upwind leg at YSBK. There was a 500 RPM drop from 5200 RPM to 4700 RPM with throttle full forward. AUX fuel pump intentionally switched off for 300ft as per SOP's just before partial failure. As aircraft was no longer climbing the pilot maintained height, kept throttle position at full, fuel pump on and carburetor heat on as per company SOP. The pilot requested low and close CCT to ATC as the engine was "rough running" this was given. Landed safely and without further incident. No emergency call was made. New filter and fuel pump installed |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: While pulling out the knob to instigate carburettor heat, the shutter in the air intake manifold was not operating. There was no indication that it wasn't working as the knob travelled in and out as it normally does and while thinking the carby heat was on, it in fact wasn't.
Determined outcome: Manufacturer response - If the small size hose clamp is missing, the cockpit control knob will not allow the flap of the carburetor heat box to close. It is identified as part of the pre-flight inspection for the correct and smooth operation of the carburetor. In the Foxbat, it is easy to visually inspect the operation and it is indeed part of the recommendation for owners when the engine cowling is removed for the inspection of the engine bay. In addition, at 4,000 rpm during the pre-flight engine check a slight drop in RPM can be noticed when the carburetor heat is applied, a more significant drop of rpm can be noticed at 4,500rpm and above and it is farther increased during flight when the ambient temperature is reduced. The clamp may slide because of improper tensioning. A design change may be incorporated should further occurrences be reported. |

The pilot was backtracking to perform a strip run and was having issues with the brakes so opted to cancel and return to the hanger. Whilst returning the pilot was having to use excessive brakes to keep straight resulting in RH brake fade and the aircraft ground looped off the left hand side of the RWY. The pilot was advised over the radio that there were flames visible under the RH wing. The aircraft was directly in front of the airport fire building and they responded immediately. Damage sustained is consistent with a brake pad fire that has spread to the wheel spat and ruptured the hydraulic line. |

A flight instruction lesson for aborted take-offs on the runway was to be conducted. The aircraft taxied to the start of the runway to begin the lesson. As full power was applied a sudden loss of power was noticed and the take-off aborted. While the aircraft was still on the runway, the engine stopped at idle. PIC attempted to restart three times within 5 minutes without success.

The pilot was preparing their aircraft to leave the airshow when another pilot approached them and advised that an earlier departing aircraft had struck the aircraft twice as they were repositioning their own aircraft prior to start-up. It was observed that they had come into contact with the aircraft once, then realising insufficient room was available tried to move the aircraft clear. Not knowing how to reposition the aeroplane they manoeuvred their aircraft once more, striking the aircraft for a second time.
The pilots have been provided with contact details to discuss the occurrence directly. Members are reminded of the importance of reporting any damage to aircraft while maneuvering, as this may impact flight safety |
