An aircraft was conducting circuits and noted another aircraft taxiing onto the active runway, however did not hear any calls. The aircraft conducted appropriate avoidance actions and as a result, no conflict occurred. Pilots are reminded of the requirement to check the radio is on the correct frequency and receiving prior to flight.
To each pilot: Pilots are reminded of the requirement to ensure the aircraft radio is transmitting on the correct frequency and confirm the radio is receiving as part of the first flight checks. Vigilance in the circuit using "see and avoid" and appropriate avoidance actions resulted in no serious conflict. "see and avoid" can be augmented to "alerted see and avoid" if radios are set to the correct frequency, checked for receiving quality and all appropriate calls are made. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: After take-off from YMMB RWY31L the pilot noticed they were unable to apply any forward trim. The Trim indicator was now also showing full deflection to up position. After landing safely back at YMMB, they tried several times to get the trim to re-position unsuccessful until it finally started to respond.
OUTCOME: RAAus have spoken with the Australian agent. It appears that an electrical connector for the trim system may have failed. In discussion with the Australian sling agent a modification to remove the suspect connector will be completed and a letter from the manufacturer will be supplied in due course. RAAus will review and investigate with the engineer and ask that any other model with the same type of connector be inspected and if required have a change completed. |

After flying from West Sale to Latrobe Valley, the pilot of a Jabiru made a 10nm inbound call, and noted that there was at least one other aircraft in the YLTV CCT conducting circuit training. The other pilot was extremely hard to understand on the radio. The Jabiru broadcast their position more frequently than normal due to the perceived language barrier (at 5nm and 3 nm to YLTV). After the 3nm call, the Jabiru pilot heard a call that someone was on final for RWY 03 (the active RWY) and assuming it to the aircraft they heard previously, made a mental note of their position and broadcast as such. The Jabiru pilot then proceeded to turn downwind, once again making a call. As they turned from downwind to base, an aircraft (with the same accent) reported that they were turning downwind. The Jabiru pilot assumed it to be this same aircraft having done their touch and go and now conducting another circuit. The Jabiru then turned base, configured the craft for landing and then commenced to turn onto final after a lookout and broadcasting their intentions, when the pilot noticed a Cessna directly under them, no more than 100ft below presumably also on final. The Jabiru then powered up and conducted a left orbit on the base arresting their descent. The Jabiru made a call stating that they were conducting a left orbit to avoid the Cessna, however there was no response from any aircraft in the circuit. The Jabiru then proceeded to land.
Referred to CASA Sport due to the other aircraft being VH registered. |

During a routine flight on a remote property the pilot called to station personnel that the aircraft was not responding as normal and he was planning an emergency landing. During the approach to landing, the aircraft became uncontrollable and subsequently impacted with terrain. The pilot was seriously injured, but managed to extricate himself from the wreckage.
Outcome: During the subsequent investigation it was revealed the left rudder cable had been rubbing on the split pin securing the cable in the pulley system, to the extent it failed completely, resulting in the loss of control.
A Safety Alert was issued by Aeroprakt reminding aircraft owners and maintainers to carefully inspect all cables at 500 hours or 5 years. https://members.raa.asn.au/storage/sa-a-22-15-inspection-of-rudder-cables.pdf |

A lesson was conducted with a student which included a preflight inspection. No damaged noted by student or instructor. The flight was conducted flight with video recording through the whole lesson. There was no impact with anything noticed during taxi or flight by instructor or student and no impact evident in video footage. The instructor and student disembarked and commenced debrief in office. Approx. five minutes later, the following student started their preflight checking aircraft and noticed the wing tip was damaged and alerted the instructor.
OUTCOME: This incident highlights the importance of pre and post flight inspections. No further investigation required. |

While conducting flight training of stalls upon returning to CFS airport requested to join overhead at 2000 to conduct a glide approach RWY 03. The instructor was training on simulated engine failure while descending from 2000 AGL upon late downwind they commented to the student not to leave the turn go too late as the headwind was approx. 15kts. After turning base at approx. 600ft at the threshold of 03 it soon became evident that they would not make the airfield, the instructor applied power and the engine did not produce power and vibrated (suspect carb icing). The instructor elected to take over at 400ft and landed the aircraft beside the railway line as they would not be able to glide to the RWY. The landing gear was destroyed and the wing hit a small tree which damaged the leading edge.
Outcome: This occurred as a result of poor planning and inadequate crew resource management by the instructor as provides a good example to pilots to consider. RAAus operations has investigated the matter and provided remediation as neccessary. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During a 100 hourly inspection of a Sling 2 LSA a number of fractures were discovered in the exhaust system. Also, the bracket that located the exhaust muffler to the engine mount had broken. Proper inspection of the stainless steel exhaust muffler (canister) was made difficult because the shroud used to collect hot air around the muffler for direction to the Cabin Heat system was riveted to the muffler rather than held on by removable/reusable clamps. The exhaust system had been previously inspected during the preceding 50 and 100 hourly inspections and no defect found.
OUTCOME: This incident has been referred to the manufacturer for development and release of a service bulletin. |
