While taxiing a very low speed to the parking area, the LH gear leg fell into a deep rabbit hole that was not visible from the pilot seat. The impact sheared the gear leg retaining bolt, the gear folded back and the aircraft fell down on its LH wingtip and elevator.

The pilot completed a full pre-flight including control check whilst stopped facing 90 degrees to RWY. They entered RWY12 and lined up and control was handed over to the front seat pilot. They applied full power and right rudder pressure to keep aircraft tracking straight, then tried to apply forward stick to raise the tail. Stick and/or elevator seemed jammed completely. Neither pilot could move the stick. Before they could close the throttle, the aircraft became airborne in the 3-point attitude. Maintaining right rudder pressure, the aircraft veered up and to the right, impacting on the right wing, before the nose struck the ground and then the undercarriage collapsed. A FOD problem between pre-flight checklist and line up is a possibility. Any evidence of such would have been thrown clear on impact. No cause for the jam has been identified. |

During a dual flight the instructor was demonstrating a landing at the flare. The power was brought to idle and required left rudder to keep the nose straight for landing. When the instructor applied left rudder, it was unresponsive with no rudder effect. A go-around was initiated. On the downwind leg following some trouble-shooting it was found that only the instructors side left pedal was unresponsive and the student had full rudder authority. On the downwind leg a radio call was made to alert the tower as a precaution and that no further assistance was required. The student made the landing and taxied back to the company apron with no further assistance. |

The member operates from a private strip under Pearce airspace in WA. The member contacted the controllers for Pearce and discussed a proposed departure, however as the airspace was active, is not compliant with the requirements of CAO 95.32 to operate in active military restricted airspace. As a result, the pilot should not have been operating at the time. Subsequent to his departure, on return to the private airstrip, the aircraft entered R155 airspace without a clearance.
The pilots has completed a flight review and ground based review of the requirements of operating in active military airspace. |

The pilot was attempting to land and taxi at the first available taxiway, resulting in a hard landing. The aircraft developed pilot induced oscillation which resulted in damage to the nose gear.
OUTCOME: The pilot acknowledged for future operations, the aircraft must not be forced onto the ground, but landed using usual landing technique. |

The aircraft had departed Archerfield and climbed to 2000ft OCTA when it was noted that the oil pressure was low and there was oil on the windscreen so the pilot turned back to the airfield. Visibility was poor due to the oil on the windscreen and light showers. ATC provided guidance back to runway.
Determined Outcome: Upon inspection a crankcase screw was found at the bottom of the cowling. This screw is removed during 100 hourly inspections to check the clutch and was not torqued correctly when reinstalled. The aircraft had only completed 1.2 maintenance hours since the last 100 hourly. |

The pilot was going through their pre-flight checklist and was at “Park brake on”, they checked the brake toggle visually to confirm it was in the on position but did not release it then reapply it as they would do normally. The next step was to call “clear prop” then start the engine. As soon as the aircraft started it began to roll forward. The pilot immediately released the park brake and attempted to reapply it, however was unsuccessful and the aircraft continued to roll forward.
The pilot realised they were getting too close to another aircraft parked in front so they applied full left rudder in an attempt to avoid it however the right wing made contact with the parked aircraft on its left elevator and then continued forward so that the right wing also contacted its left wing tip.
OUTCOME: The pilot acknowledged that a number of small but significant contributing factors added up to this accident outcome including HF elements of passenger management distraction and the differing complexity of brake application procedures for variants of this type of aircraft used at the school. The pilot immediately identified the correct initial actions but ergonomics and functionality of the brake system delayed effective intervention with the limited space the pilot had accepted prior to start up. |

On arrival into Moorabbin airport the pilot mistakenly descended to 1000ft instead of 1500ft as advised by ATC. Once advised of the error the pilot quickly followed the further instructions given by the ATC and completed a successful landing.
After landing on RWY17L and vacating the runway on TWY A4, the pilot called the ground controller for taxi clearance to the apron. The pilot was instructed to hold short of active RWY17R. The aircraft was second on the TWY A4. The pilot then thought they were given clearance however was advised by the ground controller they had crossed the active RWY without a clearance. The pilot apologized and continue to taxi. After a briefing with the CFI the pilot realised that they would have avoided this situation if they had Read Back the clearance and the ATC would have corrected them if there was a mistake. |

The aircraft was flying at 2000ft in clear calm conditions over wooded terrain. The pilot gave a 3 mile call for the airfield and pulled back the throttle to begin their descent to enter the circuit. As the aircraft was passing 1800ft the pilot felt and heard a muffled bang and approximately 3 to 5 seconds later the engine began to lose power, there was no response from the throttle. The pilot performed an emergency landing into the tree canopy and felt 3 small impacts on the way in and then hit a tree about 30ft from the ground at about 25 to 30kts. The aircraft slid down the tree landing nose first and resting against it in a vertical position. The pilot sustained minor injuries.
Determined outcome: Possible latent conditions for icing to occur. |

As the aircraft was slowing toward the completion of landing, it began to veer toward the left of the RWY. Right brake was applied heavily with little effect, and the aircraft continued to veer progressively more to the left, slow in speed, and skid across grass surface.
OUTCOME: The grass runway surface was very wet and a possible crosswind contributed to the directional control issue. |

A Jabiru aircraft entered the RWY whilst an aircraft conducting Regular Passenger Transport (RPT) operations was on approach.
Due to possible misunderstanding regarding the type of approach conducted (straight in approach), the Jabiru rolled through the taxiway holding point, but stopped short of the runway itself. The RPT aircraft landed but did not respond to any calls until in the parking area.
The pilot noted the RPT aircraft was lower on approach than expected and was reminded of the importance of "alerted see and avoid" when using radio at a Certified aerodrome where radio is required. |

After gaining a clearance to taxi, the radio had a tone in the background that was still a 3-4 readability reported to the pilot by ATC. The pilot was told to "line up and wait runway left" by ATC. Due to radio issues not yet known, the pilot heard a broken "cleared to take off " for another aircraft on a different runway. As the traffic on the runway left was vacating the pilot thought it was their take off clearance and commenced the take-off. As they got airborne ATC informed them that they had taken off without a clearance. Arriving back into YSBK the radio was playing up and was transmitting on carrier wave only. It was later confirmed that the battery had died at some point, the alternator was still working hence normal instrument reading in the cockpit and radio issues.
The CFI reviewed troubleshooting practices and cockpit management with the student to prevent a recurrence. |

Pilot was interviewed regarding environmental conditions and control management during the landing. Members are reminded of the importance of managing the aircraft with regard to prevailing weather conditions as Runway Loss of Control continues to be a major factor in runways accidents. |
