OCCURRENCE DETAILS SUBMITTED TO RAAUS: CFI requested engine out glide approach for an RPC test. Tower advised glide approved. The engine was shut down once landing clearance was received and aircraft landed safely. The CFI then restarted engine once on the ground and taxied to park. Tower request to be contacted in regards to this, so the CFI contacted the tower. The tower advised that they would prefer our engine out glide approaches not to be done in class D airspace.
OUTCOME: A review of training procedures was conducted with the flight school and the communications with ATC at the time of the occurrence were also reviewed in conjunction with the CFI and the duty Airservices controller.
Whilst section 3.02 of the RAAus Operations Manual permits the practice of "engine off" operations under controlled circumstances by a CFI or higher approval holder for emergency training, this was not clearly understood in the communications by the ATC controller.The CFI has modified his training practices to exclude this operation at a controlled aerodromes in future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the third session of circuits the student rounded out too high and despite being prompted by the Instructor, experienced a heavy landing, resulting in damage to the aircraft.
The FTS has implemented a policy of a minimum height of 200'AGL to conduct the exercise and to take account of the student workload and ability to manage this exercise after a complete circuit session. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While flying from Derby WA to Bundaberg QLD to get routine maintenance done a Jabiru, by the pilots’ calculations there was enough fuel to reach Bundaberg but there was concern about going into the reserve. The pilot made the decision to divert to Mundubbera which was approx. 12nm W of Auburn State Forrest where the aircraft eventually completed an emergency landing.
The pilot side fuel tank on the wing gauge still showed a good amount of fuel however the PAX side tank viewed empty on the wing gauge The engine started to splutter and shut down. With carby heat on and pushing throttle in and out the pilot managed to restart it but this only lasted momentarily. The pilot had approx. 45- 60 secs of flight left when they saw what was thought to be a road through the forest however as they got closer it was a single phase high tension power line with a service road under it. The decision was made to land in the forest approx. 30-40m away from power lines with the sole intention of putting the plane between the trees to take the wings off and slow the speed of the decent/landing. As the aircraft came into the trees the pilot had to make two slight deviations, one to the left and one to the right, the main two trees the aircraft hit were not directly opposite each other. The aircraft then clipped some smaller trees, the pilot with the stick full back in the stall position tried to belly it into the trees.
Fuel was running from the wings once the aircraft came to rest. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While warming the engine prior to completing an oil change and due to the pilot's agitated state of mind as a result of issues encountered during setting up the aircraft, the throttle was selected to full power and the aircraft overrode the brake system. The aircraft moved forward and struck a person providing assistance to the pilot, resulting in minor injuries.
Pilots are reminded of the importance of human factors such as distraction and frustration affecting decision making and aircraft management.
The pilot is to be commended for submitting the report. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the second circuit of a solo check lesson, a fuel cap was identified by another aircraft on the runway, unsure of whether it belonged to our aircraft we conducted an immediate full stop. On inspection after shutdown, the fuel cap was identified to be missing on the left hand wing.
The fuel caps were inspected visually on the ground in the initial pre-flight of the aircraft and appeared to be secured. However an inspection by hand was not carried out which lead to this incident. The instructor acknowledges that all inspections must not only be visual but require to be inspected by hand. This not only pertains to fuel caps, this extends to all components of the aircraft as a preventative measure to such an incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The student vacated 31L then onto Foxtrot before calling ground for the apron. They did not ask for a clearance to use 31L.
OUTCOME: This was the student's first solo flight. The student was grounded, retrained over four subsequent flights and released to flying. Since then, student has completed further 3 solo flights, with no further occurrence. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During pre-flight procedures the student had the fuel caps loosened, waiting for the aircraft to be refuelled. The instructor decided there was enough fuel onboard to continue with the sortie and the pair departed. Whilst in the run-up bay the fuel caps fell off. The aircraft proceeded to take off while the company found the fuel caps on the ground. The aircraft returned to land without issue.
The Instructor did not double check the fuel caps were tightened and student tried to help the refueller by leaving fuel caps loose. In future, the instructor will be vigilant to conduct pre-flight checks after student has done so. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Stalls and practice forced landings were being conducted in the Bankstown training area near Camden airport. The engine started running rough and the PIC made the decision was made to return to Camden airport for a Full stop landing. Reporter stated that they suspected water in the fuel was the causal factor as the aircraft had been washed directly prior to flight. Abnormal pitch and roll attitudes during stalls and simulated engine failures could possibly send water ( heavier than fuel) to the fuel tank sump intermittently. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: A 'kink' developed in fuel line resulting in fuel starvation on takeoff. A new fuel filter had been tested, installed and engine run-up performed on ground to test, which indicated normal operations. However, on take-off at approximately 50 feet AGL engine noticeably dropped revs. The pilot conducted a landing by closing the throttle, lowering the nose and selecting full flap on remaining 500 metres and taxied back to the hangar.
The new filter had been connected without noticing that the fuel line at the electric pump end had kinked due to it being enclosed in the center console and therefore not being visible. The new filter was marginally larger than the old one. Some fuel was getting through but not enough to satisfy normal takeoff requirements.
The pilot elected to shorted the fuel line from electric fuel pump to fuel filter by approximately 10mm thus straightening line and removing the kink. After conducting fuel flow tests the unit proceeded to producing maximum flow. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Student was conducting circuits outside of tower hours as CTAF operations and came in a little faster than usual in an effort to avoid a Jetstar A320 departing shortly. Due to excessive airspeed during the approach and flare the student touched down hard with a higher than usual airspeed. This resulted in a large bounce, with incorrect recovery technique, pushing the control stick forward and pitching down. The aircraft pitched down very steeply with a high rate of descent and impacted the runway on the nose wheel which collapsed. OUTCOME: The student will undertake remedial training. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst flying from Redcliffe QLD to Narromine NSW tracking via Gatton and Wee Waa, the aircraft was on autopilot for most of the flight. Conditions were hazy even close to Brisbane. Beyond Warwick the pilot couldn’t discern a distinct horizon, however abeam Moree they could still see the large grain silos next to the airport from 25km away. As the aircraft got to Wee Waa it became progressively hazier. Visibility over the engine cowl was negligible and to maintain a sense of the aircraft’s attitude required looking at ground features to the side of the aircraft. Abeam Gilgandra at 4,500’ it quickly got worse – almost like the aircraft flew into a denser cloud of dust within the thickening haze. At this point the pilot couldn’t see Gilgandra airstrip about 7km out to their left - and then lost sight of the ground. This was a scary moment and being on autopilot didn’t allay the pilots’ fears as much as you would probably expect. The pilot could only stare at the artificial horizon on the EFIS while they cancelled Altitude Hold on the autopilot, throttled back and descended. Within 30 seconds or so they started to regain visual attitude references, and flew the remaining short distance under clearing conditions to Narromine at 2,500’.
The loss of visibility was due to smoke from farmers burning off. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While conducting circuits the aircraft radio failed. The pilot wasn't aware of the failure until another aircraft came in to land. As soon as the pilot became aware of the problem he landed and ceased flying. The radio failure was identified as a wiring issue in the PTT and was rectified.
RAAus reminds pilots of their obligation to ensure the radio of an aircraft is operational and functioning correctly as part of serviceability checks prior to aircraft movement. |

The pilot received remedial training and provided a formal apology to the airport operator and passenger airline.
Pilots are reminded to ensure they are familiar with the airport ground taxiways markings and consider the safety of uninformed participants when operating at unfamiliar airports. |
