OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was commencing the first flight of the day and first flight after minor maintenance. Immediately after take-off on RWY10 and on initial climb out at a height of 100ft and approx. 60kts the pilot heard a loud bang followed by some engine vibration. The throttle was closed and the pilot attempted a landing back on the remaining RWY. However, there was insufficient RWY length and as the aircraft appeared to be within normal control parameters in straight and level flight, the pilot decided to maintain altitude of around 100ft and commence a wide left hand turn towards RWY18. The aircraft behaved normally in all axis of flight however the pilot was concerned as to structural integrity and elected to land as soon as possible. The landing was carried out successfully and the aircraft was brought to a halt using normal braking and no other damage was incurred. The aircraft was taxied under normal power back to its hanger.
The noise appeared to have come from a cowl that was unfastened. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: At the threshold the aircraft floated above the runway and did not seem to settle to land. It then dropped suddenly to a hard landing, bounced and as it came back down seemed to settle on the mains. However the nose wheel must have touched and the propeller contacted the runway. After the second bounce the pilot powered on and went around to complete a successful landing. After taxiing back to the hangar and shutting down the damage to the propeller became evident.
Pilots are reminded of the importance of managing unstable or unusual approaches by executing go-arounds as required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual TW instruction, after completing some taxiing runs along the RWY the controls were operating normally and the decision was made to take off. Full power was applied, a little forward elevator control was applied and the TW left the ground. Then a little up elevator was applied and the aircraft left the ground. It reached an altitude of a couple hundred feet and an airspeed of 30 knots. No excessive elevator control was applied. The aircraft then suddenly began to lose altitude with no change in the engine noise. The student released the control column and the CFI took over maintaining a straight line ahead. The aircraft narrowly flew over a fence and a road and nosed down into a paddock just outside the airfield. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During take-off roll, it was noted that airspeed was not functioning. Take-off was aborted, the throttle closed and brakes applied before lift-off. Brakes grabbed unevenly and aircraft left the RWY onto grass until it came to rest. Upon investigation it was discovered that an insect was blocking the Pitot probe preventing airspeed indication. steering post application of the brakes was hampered by an imbalance in the adjustment of the brake pads. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the landing phase at a private strip surrounded by trees, wind shear and mechanical turbulence resulted in the aircraft bouncing. The nose wheel stuck the ground heavily resulting in its collapse. The collapse of the nose wheel caused the propeller to strike the ground and the right wing tip to scrape the grass strip. The pilot will conduct remedial training with a RAAus CFI. Pilots are reminded to consider go-around actions prior to landing in these circumstances as a plan for management of micro meteorology. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Following evidence of coolant loss in flight the pilot made the decision to reduce power and configure the aircraft clean to extend the glide back to the aerodrome to ensure they made the runway. On short final they lowered the undercarriage without use of flap. During subsequent throttle adjustments the engine stopped. The pilot focused on continuing a safe glide approach and was not aware the gear was not fully extended and locked at touchdown. Audible alarms only enunciate when gear is up and flaps are extended in this aircraft.
Onsite investigations by the pilot and maintainer revealed the radiator return hose separated at the top radiator cap attachment, No other hoses were identified as incorrectly secured.
Subsequent to the accident the pilot has been made aware of Rotax operating provisions that allow the engine to be operated for limited periods in emergencies without coolant. |

However, when the aircraft was taxied back to the hard stand it was noted that it was leaning towards the port side. An inspection found the port undercarriage had partly cracked at the top close to the fuselage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student was on an approved solo training navigation exercise. During the navex the pilot became unsure of their position and whilst making track corrections inadvertently climbed into controlled airspace.
An investigation was undertaken with both the pilot and supervising instructor with a focus on assessment standards and navigational competency and a review of remedial training actions was undertaken. The pilot undertook appropriate retraining and was subsequently assessed as competent by an independent examiner for issue of cross country endorsement privileges. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On a dual training Navex the pilot did not recognise the increased sink rate during the final stage of the approach and despite late intervention by the instructor the aircraft struck the tail as a missed approach was commanded by the instructor. Contributing factors were identified as a reclined seat position changing the pilots normal eye line referencing and delayed intervention by the instructor. It was also noted this was the first dual flight with the student instructor combination and the landing was not at the schools normal operating location.
Schools and Instructors are urged to attend and review the resources currently being provided by RAAus at Professional Development seminars on "Control Management for Instructors" Effective intervention strategies are vital to safe training outcomes while preserving maximum learning opportunities for pilots under training. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During approved training circuits at Bankstown whilst under a control clearance the RAAus registered Bristell and a C152 lost visual reference in circuit as the C152 conducted a simulated glide approach from crosswind resulting in the Bristell overtaking the Cessna in the circuit.
Both aircraft continued operations without further incident. The primary cause was loss of visual awareness of the C152 by the instructor in the Bristell. Contributing factors were a lack of communications by the controller in advising an approved non standard circuit had been granted to the C152 when the Bristell was asked to follow the preceding C152.
RAAus continues to recommend that training operators undertake joint aerodrome safety meetings with all affected stakeholders to raise awareness of issues such as these and devise procedures and agreements to assist safe and harmonious operations at all training aerodromes. These meetings and devised outcomes can form part of any schools risk management plans and Safety Management System. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During take-off the airspeed indicator was slow to show speed increase. After take-off the airspeed indicator kept increasing through to >100kts even though the aircraft was in climb/low airspeed. The pilot deduced the pitot head must have a blockage. On downwind the pilot requested a full stop, tower asked if emergency services were required which was declined. The aircraft taxied back to base, where an engineer examined the pitot head and cleaned it, dried mud was found inside the pitot tube. |
