Airspace infringement: The Archerfield Tower reported that the aircraft had infringed CTA. Outcome: The aircraft static pressure readings have been check as serviceable and the pilots have stated that they were operating at 2000FT AMSL. It is unclear why ATC monitored their transponder as infringing on airspace. At this point in time RAAus will not be investigating further.

During solo circuit training the student flared on landing but was too high and hit the strip a bit hard as a result. The aircraft rose again, so the student decided to go around. The second attempt to land was a replica of the first, the student again decided to go around. The third attempt was heavy as well however the student managed to land the aircraft and return to where the instructor was waiting. DETERMINED OUTCOME: RAAus Operations has discussed this incident and any follow up action taken with the CFI and is satisfied that all protocols were adhered to. No further action required at this time.

On take-off, at approx. 100ft the engine failed instantly without any warning resulting with the aircraft contacting the ground heavily. OUTCOME: Owners self identification: "Checked fuel pump inlet line from filter to pump bingo there it was a piece of clear plastic tubing photo attached to this email ,it had obviously had softened enough and had travelled down the line to the pump and closed the orifis in the pump shutting off fuel supply to both carbies."

After experiencing turbulence from a nearby headland the pilot decided to out land safely on the sand dunes at North Shelley Beach. This resulted in minor damage to the prop. OUTCOME: The pilot is to apply more extensive weather assessment with local flights with respect to wind conditions and potential turbulence and is independently having the integrity of the PPC checked in addition to other mechanical repairs.

An aircraft was conducting circuits and noted another aircraft taxiing onto the active runway, however did not hear any calls. The aircraft conducted appropriate avoidance actions and as a result, no conflict occurred. Pilots are reminded of the requirement to check the radio is on the correct frequency and receiving prior to flight. To each pilot: Pilots are reminded of the requirement to ensure the aircraft radio is transmitting on the correct frequency and confirm the radio is receiving as part of the first flight checks. Vigilance in the circuit using "see and avoid" and appropriate avoidance actions resulted in no serious conflict. "see and avoid" can be augmented to "alerted see and avoid" if radios are set to the correct frequency, checked for receiving quality and all appropriate calls are made.

An aircraft was conducting circuits and noted another aircraft taxiing onto the active runway, however did not hear any calls. OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: The horizontal shaft of the rudder control tube cracked at the point of the rudder pedal shaft resulting in loss of rudder control. DETERMINED OUTCOME: This issue has been identified and rectified by the reporter. This is an isolated issue and no further action is required by RAAus on this matter.

During circuit flight training a VH registered aircraft joined the circuit and descended on the ACTIVE side of RWY 12, cutting in front of the 3 training aircraft already established in the circuit, one of which was between mid to late downwind and conducting a glide approach.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: After take-off from YMMB RWY31L the pilot noticed they were unable to apply any forward trim. The Trim indicator was now also showing full deflection to up position. After landing safely back at YMMB, they tried several times to get the trim to re-position unsuccessful until it finally started to respond. OUTCOME: RAAus have spoken with the Australian agent. It appears that an electrical connector for the trim system may have failed. In discussion with the Australian sling agent a modification to remove the suspect connector will be completed and a letter from the manufacturer will be supplied in due course. RAAus will review and investigate with the engineer and ask that any other model with the same type of connector be inspected and if required have a change completed.

After flying from West Sale to Latrobe Valley, the pilot of a Jabiru made a 10nm inbound call, and noted that there was at least one other aircraft in the YLTV CCT conducting circuit training. The other pilot was extremely hard to understand on the radio. The Jabiru broadcast their position more frequently than normal due to the perceived language barrier (at 5nm and 3 nm to YLTV). After the 3nm call, the Jabiru pilot heard a call that someone was on final for RWY 03 (the active RWY) and assuming it to the aircraft they heard previously, made a mental note of their position and broadcast as such. The Jabiru pilot then proceeded to turn downwind, once again making a call. As they turned from downwind to base, an aircraft (with the same accent) reported that they were turning downwind. The Jabiru pilot assumed it to be this same aircraft having done their touch and go and now conducting another circuit. The Jabiru then turned base, configured the craft for landing and then commenced to turn onto final after a lookout and broadcasting their intentions, when the pilot noticed a Cessna directly under them, no more than 100ft below presumably also on final. The Jabiru then powered up and conducted a left orbit on the base arresting their descent. The Jabiru made a call stating that they were conducting a left orbit to avoid the Cessna, however there was no response from any aircraft in the circuit. The Jabiru then proceeded to land. Referred to CASA Sport due to the other aircraft being VH registered.

During a routine flight on a remote property the pilot called to station personnel that the aircraft was not responding as normal and he was planning an emergency landing. During the approach to landing, the aircraft became uncontrollable and subsequently impacted with terrain. The pilot was seriously injured, but managed to extricate himself from the wreckage. Outcome: During the subsequent investigation it was revealed the left rudder cable had been rubbing on the split pin securing the cable in the pulley system, to the extent it failed completely, resulting in the loss of control. A Safety Alert was issued by Aeroprakt reminding aircraft owners and maintainers to carefully inspect all cables at 500 hours or 5 years. https://members.raa.asn.au/storage/sa-a-22-15-inspection-of-rudder-cables.pdf
