OCCURRENCE DETAILS SUBMITTED TO RAAUS: During approach for landing, just before touch down a sharp cross wind gust from the east hit and the pilot over corrected resulting in the aircraft landing with one wheel of the strip on the grass/dirt. Due to the exceptional rain and wet ground the aircraft main wheel entered a very muddy area and was pulled off the strip. This wheel dug into the mud causing the aircraft to turn to the left and tip over onto the nose.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During take-off the wind changed direction resulting in a tailwind which reduced the rate of climb of the aircraft. The instructor took control of the aircraft and attempted to lower the nose slightly to increase speed and achieve the maximum rate of climb which had no effect. The aircraft encountered tree tops with the wings impacting small branches, slowing the aircraft down prior to making contact with the ground in a nose down attitude of approx. 45 degrees.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: The fuel tank drain valves are fitted with cork washers by the factory. The cork washer failed after a period of time and fuel tank leaked fuel from around the faulty cork washer. The manufacturer has removed the cork gasket and replaced it with a fit for purpose type that is made from a different material and is fuel resistant.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Following a routine take-off the engine revs started declining after reaching a maximum altitude of about 2,000ft. After trying the fuel pump and carby heat to no benefit, the revs dropped to less than 1000 rpm. A stubble paddock was selected and an uneventful forced landing was completed. Subsequent investigation revealed a small amount of fuel line had lodged in the carburetor causing it to flood.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft started rolling for take-off and as it accelerated to rotate speed the pilot heard a radio call from another aircraft. The pilot was unsure of their location and made the decision to abort take-off so pulled back on the power and applied the brakes. The aircraft moved to the right so the pilot then corrected with left rudder and left brake. As they did this the aircraft moved to the left and exited the RWY to the left side into a grassed area. The pilot thought that the aircraft would decrease speed naturally as they were in a cleared area with no obstacles. Unfortunately, there was a ditch/ water diversion drain hidden in long grass that the pilot didn’t see and the nose wheel dug into it as the aircraft left the RWY.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was on the RWY amid touch and go when the Tower advised full stop due 'possible nose wheel damage'. The pilot rejected take off and taxied to the node on the apron. There was no visible damage although 3 screws were noted as missing for nose wheel fairing. The pilot called the tower by phone to ask about damage they sighted. The Tower said that the nose wheel was shimmering a lot. The aircraft was reviewed by engineers and the missing screws replaced.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Upon turning crosswind at 500'AGL, RPMs dropped to lower level than usual. Typical is 5300RPM full power on climb; dropped to between 4810 and 5050. This also resulted in reduced climb performance to 200'/min (typically at same conditions 500-700'/min). At 900' the pilot set cruise attitude and aircraft was very slow to accelerate and only to 90KIAS at full power (typical cruise is 95 KIAS with fast cruise at 5500RPM 105KIAS). As the pilot was still in the circuit, they continued downwind and landed. Noting the engine seemed to run more smoothly at lower power setting.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft experienced a total loss of power at 1400ft, 1.5 NM to the West of YBRS. The pilot made an uneventful emergency landing into a paddock. Aircraft maintenance facility advised fuel exhaustion cause for engine failure.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: When landing, on round out, a small amount of wind shear was experienced due to trees surrounding strip. Recovery efforts by the pilot were insufficient, and the aircraft experienced a heavy landing from 6ft collapsing undercarriage.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An Instructor was conducting circuit work with a student on RWY 32 (Strathalbyn) and had been for the previous 45mins giving base calls (126.7) for touch & go each time. No other aircraft were operating from Strathalbyn that morning and no calls had been received from any aircraft in the local area. After a touch & go the student powered on and climbed off the centre line reaching 500 feet just after the end of the RWY and commenced a climbing turn on to cross wind and as 600 feet was reached an unidentified aircraft flew underneath the training aircraft travelling from east to west at around 500ft. Coming in from behind them neither the student or instructorsaw the aircraft until it passed under them. The Instructor gave an immediate call on 126.7 asking for a response from the aircraft that just flew past the Strathalbyn Airfield but was not answered. The aircraft continued to head west at the same low altitude and disappeared from sight. RAAus reminds all pilots to monitor the correct frequency for aerodromes which are not assigned a nominated CTAF frequency, but do appear on aeronautical charts.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight training on take-off the aircraft developed a vibration. The instructor took control and landed without incident. The instructor then conducted a second run up and check at 3800rpm, the engine ran smooth and the ignition checked normal. They conducted a full power check; upon increasing RPM beyond 4000rpm and the engine performance decreased and rough running occurred at maximum 4400rpm. Upon retarding the throttle engine performance would increase before resuming normal smooth running and RPM decrease. The aircraft was taxied back and shut down for inspection. A small blockage in one of the carby fuel jets was identified

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight training at Hervey Bay, the student identified RWY 29 was the active RWY. As the instructor was unsure a radio call was made to the RPT aircraft on the ground for its intentions and which RWY it was going to use. The instructor did not hear a response, therefore allowed the student to join mid field crosswind after descending on the dead side of RWY 29. The student turned to close to the RWY not giving themselves space for the base leg which led them to not having space for finals (too high). After turning for finals the instructor looked at the wind sock and realising it was a tailwind, took the controls to demonstrate that even with full flap they would not make it. Eventually they proceeded to conduct a go round, making a go around call and departing the circuit back to Caloundra. Upon landing at Caloundra, it was determined the captain of the RPT aircraft had raised concerns about the communications. Further investigation by the CFI determined that the student had decreased the volume on the radios, which was not apparent to the Instructor and caused miscommunication and the inability to hear other calls correctly at Hervey Bay. This was corrected after departure and the rest of the flight the instructor was able to hear radio calls.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was directed to join circuit for landing with a 15 kt crosswind. The pilot completed touch down successfully but when the nose wheel contacted the runway the aircraft veered off toward right (into wind) and came to a stop on the grass. The pilot then added power and taxied off the RWY successfully. Actions were undertaken internally with the flight school to address this RLOC incident.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On completion of an annual inspection the aircraft was taken up for two circuits. Upon landing on RWY 05 with a 20 knot northerly wind the pilot ballooned the landing and bounced on the second landing. The aircraft was taxied back to the hangar and found the front nose spat damaged. There was no other damage to the aircraft. The damaged spat was removed. Pilot has recognized the need for recency and currency in the prevailing conditions.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: During 100hr inspection the stabilator bearing was found to be worn. The stabilators were removed from the aircraft for access to replace the bearings. Once removed a crack was very noticeable on the RH empennage skin just forward of the stabilator attach fitting. A closer inspection found the crack and one rivet found loose, the crack seemed to be coming from the skin strengthening bend relief hole just forward of the rear bulkhead. Investigation: Apparent defect in the manufacture of the longitudinal stiffener at the point of attachment with the vertical stabilizer, cracking started at the radius of a bend in the metal that was not de-burred at manufacture. Cracking amplified by worn bearings in the stabilator. Manufacturer notified of defect report.
