OCCURRENCE DETAILS SUBMITTED TO RAAUS: While on final approach to runway 35 at Cessnock, a go-around was initiated due to an aircraft conducting low level circuits, appearing on the runway ahead of the RAAus aircraft. When the throttle was advanced to conduct the go-around, the aircraft engine began to run roughly. Climb out was impossible and the plane was landed in the last 400 metres of the runway without incident. At this stage the other aircraft was stationary in the middle of the runway about half way down the strip. Examination of the engine has not yet revealed a cause for the rough running but the problem is reproducible on the ground and still being investigated. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Aircraft entered airspace from incorrect waypoint as clearance was to proceed northbound west of the motorway. Instead, the aircraft crossed to the eastern side of motorway 1nm before clearing YBSU airspace. The instructor had been distracted, focussing on student patter. The instructor has acknowledged and determined that future patter should be restricted when operating during critical phases of flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The left carburetor drip tray was found to be broken away from the 2 (tight) securing bolts. The cause appears to metal fatigue. The tray is only secured at one end. Bending of the side wall of the tray suggest that the tray had been subject to vibration and bending before the metal fractured. Early signs of bending were also observed on the right carburettor tray, suggesting that re-occurrence of the fracture is possible.
Initial cracking at the fastener holes, led to failure through vibration induced by an out of balanced propeller. Owner has subsequently had the fuel trays repaired and the propeller balanced. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot completed a touch and go, when on upwind (at approx. 50 to 100ft), there was a bang and vibration from the engine. The engine was still running with some power at low revs. There was enough power remaining for the pilot to complete a circuit and land.
Engine cowls were removed and it was found that two through bolts on the number one cylinder had fractured. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Student was conducting subsequent solo under direct supervision of senior instructor. Circuit and approach were normal, flare height correct, however a slight balloon during the hold off led to the student attempting to force the aircraft onto the ground. The resulting bounce did not result in a go-around and the aircraft bounced 3 more times before the prop struck and the aircraft stopped.
OUTCOME: Operations have reviewed this report and after consultation with the CFI determined no further action required by RAAus. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The instructor and pilot combination did not correct when the aircraft deviated from expected tolerances on landing which resulted in the aircraft impacting a runway marker at this LA.The decision was made to conduct an immediate "go round" suspecting undercarriage damage. A decision was then made to return to the originating AD when the instructor determined that elevator control had been compromised by the impact but successfully landed the aircraft at the training base. Changing cross wind conditions, reduced attention by instructor and delayed decision making were likely contributors to this incident.The instructor has arranged to undertake further instructor development training following this incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student requested start clearance for circuits, taxi clearance and downwind call for a touch and go, however broadcast using the wrong call sign. Despite the student making perfect practice calls in the cockpit prior to transmitting, when the broadcast was made, these errors occurred. The student was required to develop a clear process for ensuring the same call was made during practice and actual transmission. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was conducting a navigation command building exercise, on the way back to Moorabbin the weather had changed to crosswind variable 12-15kts. After attempting to land the pilot made a decision to do a go around because they didn't feel it was safe to continue with the landing. After this happened the pilot attempted to land 4 more times which all resulted in a go around. On the last attempt the CFI was on the radio and gave a bit of a guidance for landing in this wind conditions which resulted in a successful landing. The CFI has successfully mitigated risk of recurrence . No further action required by Operations at this time. |

The pilot became confused and entered a runway without clearance, stopping across a hold point. The CFI has provided confirmation to the pilot of company policy regarding correct exit procedures and will monitor into the future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flight, approx. 3 NM to the NW of Robina at 1000' AGL the Lane A warning light started flashing intermittently. The pilot immediately turned toward Southport airstrip whilst completing all usual troubleshooting checks. At approx. 1 NM south of Heck Field the aircraft entered into a decent, after 5 seconds the engine began to run rough and stopped within 3 seconds. The pilot made a PANPAN call that the aircraft had suffered an engine failure and was intending to conduct forced landing at Heck Field. The aircraft landed safely and the pilot cancelled the PANPAN call.
Determined Outcome: A failed electrical join at the fuse box caused the engine pump to fail. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After a 45 minute local flight from the airfield, the pilot encountered wind shear on landing. Due to the runway surface the plane deviated off the right side of the runway with one wheel on bitumen and the other on sandy bush land. The right side wheel sheared off and the plane skidded to a halt after 100m. Control force difference and lack of familiarity with the type, which was newly purchased, contributed to this accident.
The pilot also indicated a go-around would have been a better response.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The student carried out a smooth successful landing, however the right undercarriage leg of the aircraft snapped. The undercarriage leg in question had completed just 12 cycles before failure. Engineering analysis was carried out on the metallurgical properties by an independent engineer. The professional opinion is that the heat treatment during manufacture was inadequate resulting in the failure. |
