The aircraft veered to the left after the wheels touched down. When the aircraft exited the RWY the propeller and wings both impacted the grass.
The pilot has discussed this occurrence with the CFI and has also completed a successful flight review since. |

After take-off, at approx. 250 feet AMSL the engine start running rough. Carby heat was applied, the electric fuel pump turned on and changed from left tank to the right tank. The engine was still running rough and RPM dropped to 3800 RPM with full power. A call was made to air traffic control returning back for landing on RWY 29R. The aircraft landed uneventfully. Possible Carburettor icing. |

The student conducted an flight with no approval from the CFI. On landing an unstable approach occurred which resulted in loss of control on landing. The student was required to undertake training to ensure aircraft management was understood and to competency along with remedial training for issue of Pilot Certificate and endorsements and sign a Declaration of Honorable Conduct to commit to operating within RAAus and CASA requirements. |

The aircraft had entered the RWY so it could backtrack for take-off, when it lurched. The pilot thought they may have run over a pothole in the rough taxiway. There was no evidence of any abnormality on normal pre-take-off run up etc. The aircraft flew back to YGAW with no abnormalities noted, however there was found to be a chip out of the tip of one prop blade (carbon fibre) when preparing the aircraft for hangar.
A following aircraft reported that the aircraft ran a main wheel over a wire-framed, cloth-covered runway marker as it entered RWY 30. |

The pilot started the aircraft after several attempts, they had the foot throttle held down part-way and when it started, it revved suddenly and lurched forward. The pilot pressed the brake pedal and the throttle trying to stop the aircraft, however before they could recover and release the throttle and the left wing made contact with the hanger door post.
The student has undertaken significant training in conversion to Group B aircraft but believes some primacy of toe brake operation from 3 axis GA aircraft may have led to inadvertent foot throttle operation against the brakes. The student will continue with training with the CFI further addressing the differences between Group B and 3 axis aircraft. |

The aircraft departed on RWY 31 using both fuel tanks, and after take-off the pilot turned off the right tank in order to balance the tanks (45L in the left, 20L in the right). On return, after turning downwind, the pilot commenced the landing checks. As part of the fuel check, the intention was to turn the right fuel tap back to the ON position. The pilot mistakenly turned the left fuel tank to the OFF position, therefore turning off the fuel altogether. After turning base, the engine began to run roughly and cut out. The pilot immediately realised what had occurred and switched both fuel taps to the ON position and the engine started again. The aircraft landed without incident. |

During a solo training flight, the pilot was too high on approach so lowered the nose to reduce height resulting in an increased airspeed. Flared late causing nose wheel to hit the ground first, this action resulted in 3 bounces, the third of which bent the nose wheel and reduced sensitivity of rudder controls. Went around and was intending to try again but was directed to the training areas while circuit cleared then came in to attempt landing on RWY 23. The first attempt resulted in a go-around, the second attempt was successful however the aircraft ran off the RWY strip due to compromised rudder control.
The pilot reviewed appropriate approach speed management and techniques verbally and practically with a Senior Instructor |

OUTCOME: Pilot identified a number of contributing factors based around his actions and specifically bouncing on landing and choosing only 1 stage of flap combined with the effects of wet grass on this 360m landing strip. |

Whilst on very short final for runway 31R, a flock of birds took off from the ground and flew across in front of my aircraft. One impacted low on the front facing radiator below the propeller. A full stop was made. No damage was visible.
Outcome: RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |
