OCCURRENCE DETAILS SUBMITTED TO RAAUS: An instructor and student were conducting dual flight training circuits at Riddells Creek Airfield. When turning off a left base onto final the instructor identified a VH registered aircraft slightly to their left flying directly at them and climbing. The instructor immediately took command of the aircraft by lowering the nose and breaking away to the right and continued the turn until they sighted the aircraft.
The instructor transmitted to the pilot of the other aircraft stating who they were and asked what were their intentions and requesting their registration. The VH registered aircraft replied with their registration and that they were leaving the area. The instructor and student only heard one radio call prior to this event which said 'leaving Riddell'.
This occurrence has been forwarded to CASA for review. All pilots are reminded of the importance of maintaining standard radio procedures and maintaining an active lookout to avoid near miss occurrences. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A converting pilot was completing solo practice under supervision of CFI. Three landings were completed satisfactorily. The fourth landing was observed as normal then the aircraft was slowly departed the runway onto the grass, ran over a cone marker and hit a star picket before coming to a halt. The pilot has completed additional training. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The occurrence took place during a student training flight at Wagga Wagga Airport. During the taxi out to the runway, a construction vehicle was observed travelling at speed across a grass access road towards taxiway Charlie. The vehicle did not have a rotating beacon or hazard lights on display. No radio call was made that the vehicle was going to cross the active taxiway. The instructor advised the student that the vehicle was on a collision course with them and be prepared to stop if the vehicle didn't start to slow down and stop. It became obvious that the driver had not seen them and was going to enter the taxiway so they started to brake to prevent a collision. The driver suddenly braked heavenly and stopped just short of the taxiway.
This information has been forwarded to Wagga Airport Operations for their review which may assist with awareness and improvements in safety. This occurrence highlights the importance of maintaining a good lookout during taxi to avoid obstacles on the ground. If members identify a potential hazard at an airport or experience a similar occurrence, RAAus encourages reporting within our occurrence management system so that awareness and safety can be improved with airport operators. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student and instructor were conducting a navigation exercise. Upon arrival at the first aerodrome there were several aircraft in the circuit and a significant cross wind was noted which required the instructor to prompt the student. The aircraft landed heavily and bounced, the instructor then initiated a go-around and flew back to base. A crack in the undercarriage leg was noted on arrival. RAAus Flight Operations staff mentored the instructor on personal minimums with students and the challenge implicit in letting the student operate to the extent of their experience to assist in learning and intervening in time to protect themselves and the aircraft. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was on the ground after landing and experienced a strong 25 knot wind gust which resulted in a ground loop and impacted the wing with the ground. The pilot was not seriously injured, but the aircraft was damaged and moved to a hangar for repairs.
While the airport did not provide an ATIS service the passenger jet which landed just prior had confirmed thermal activity and a strong wind change had just occurred. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Foxbat was conducting a go around procedure on RWY 29L in the southern circuit at 200 FT AGL. While going around it came into close proximity with a helicopter flying at 500 FT AGL conducting the choppers south procedure. The pilot tried to make a go around call but there was significant frequency congestion. The Foxbat pilot climbed while the chopper descended to avoid conflict.
RAAus will review the helo procedure at the airport and liaise with ATSB and the tower to determine if any changes to procedures are required. |

The pilot had completed a number of familiarisation flights and practice landings prior to the ferry flights. The home airport was approached at normal speeds for landing, however the aircraft bounced and came down, tearing off the nose wheel and bending back the nose wheel leg. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was flying towards Lilydale 7.5 miles out when it experienced a partial engine failure. This then became a total engine failure and the pilot completed a forced landing on a private airstrip. Review with the owner has indicated it could have been carburettor ice however the fuel pump and the 2 fuel caps have been replaced as a precaution. No further concerns have been observed since this occurrence. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On a morning flight south of Holbrook, NSW, in the vicinity of the hamlet of Mullegandra, the engine intermittently lost power. Switching on the electric fuel pump had no effect. Opening and closing the throttle also had no effect. The pilot then decided to find a suitable paddock to land the aircraft in rather hilly country. A paddock was sighted not far from the Hume Highway and a landing was effected. Unfortunately, the paddock contained a harvested crop of lucerne and a rough landing was experienced because of the lumps of lucerne left after harvesting. When the aircraft stopped the pilot ran up the engine to full power. The engine performed as normal. An inspection of the aircraft after landing found no obvious damage. The passenger decided to walk to the Highway leaving the pilot to fly the aircraft back to Holbrook after assisting with the take-off. A search of the paddock found a possible take-off path and with full power, half flap and the passenger holding back the tail, a very short take-off was effected and the aircraft flew back to Holbrook without experiencing any further problem.
The pilot and the passenger, an experienced pilot, agreed that the problem appeared to be carburetor icing. Further review of the icing probability chart demonstrated conditions on the day were favourable for icing. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the roll out after landing the tail on the tail wheel aircraft was lowered to the three point position. As the tail wheel made contact with runway a gust of wind pushed the aircraft to the right. The pilot was unable to correct the swing and departed the runway towards the soft sand. The left wheel stuck into the sand causing the left wing to dip and made contact with the ground. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the landing in gusty wind conditions, the aircraft landed as usual however veered to the left and at slow speed left the runway, stopping just off it. The pilot noted lack of recency and operations in gusty wind conditions may have contributed.
RAAus members are reminded of the importance of setting personal minimums and as required, to take an instructor with them for a refresher flight to ensure management of flights in challenging conditions is conducted safely. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A single point failure occurred in the nose wheel leg assembly. At the top of the gear leg is a boss/bolt and nut welded to the gear leg. The boss bolt failed causing movement in the gear leg to the point the steering was unavailable. After landing, a student was unable to steer the aircraft and excited the runway into a grassed area at slow speed.
RAAus has communicated with the manufacturer who was aware of this occurrence and confirms no similar cases have been reported. It is possible that this failure occurred due to a previous unreported heavy landing. RAAus reminds members of the importance of reporting heavy landings and ensuring a heavy landing inspection is carried out to assess an aircraft for any potential damage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting pre-test with student, after successful upper air work and practice forced landing the student and instructor returned to the airfield for the circuit part of the pre-test. They started with a normal 3 point CCT with between 3-5kts of crosswind on runway 21 which the student managed with no issues, moving on to wheeler landings. Upon touch down the aircraft bounced slightly, the Instructor told the student to bring the "stick forward" as done with wheelers (once on the ground) but the aircraft had already become airborne because of the bounce. The stick was then pushed forward and due to the timing of the instruction the nose of the aircraft was pushed down which subsequently resulted in a hard landing with a nose down attitude. The landing was completed and the aircraft came to a stop. There was some unusual vibration from the engine. The motor was shut down immediately and it was noticed that the ends of the propeller had been damaged. The aircraft was towed back to the hangar.
The Instructor noted internal review of training techniques and use of language by the instructor has been completed to prevent this type of occurrence from happening again in the future. |
