OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft engine failed during take-off just as nose was rotated. As the pilot was taxiing, they noticed one tank was fuller than the other so turned the fuel cock off for that tank. During the Pre-Take-off Vital actions (PTVAs) the pilot elected to turn the fuel cock back on for take-off, but inadvertently turned the wrong fuel cock off, stopping all fuel flow. Sufficient fuel was in the system to line up and accelerate to rotate speed before the engine starved of fuel and shut down. The pilot lowered the nose wheel back onto the runway and aborted the take-off roll exiting on a taxi way.
The pilot undertook a review with a CFI and noted the correct fuel tap directions for future operations. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student pilot was conducting solo supervised circuits when the pilot of another training aircraft on early downwind became concerned with the track and proximity of the solo student aircraft on crosswind.
The instructor debriefed student pilot after landing and the student confirmed that aircraft on downwind was sighted and student was widening out circuit to avoid prior to ATC instruction to widen out circuit.
The student was advised to ensure communication with the other pilot, ATC and actions were prompt to assist with situational awareness in the circuit. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The student was back-tracking runway 23 from the intersection after completing a solo flight when they realised that an aircraft was on final for 23 at around 400 ft AGL. The other aircraft called "going around" and the student then reduced power but as the runway was a bit damp they weren’t able to slow the aircraft sufficiently. The student was also taxiing too fast and didn't vacate at the exit point for 23, instead running onto the grassy run off area at around 8-10kts. The student turned left onto bumpy grass and the prop made contact slightly with the grass and the engine stopped.
The engine was reviewed by maintainers to ensure no gearbox damage was incurred and the CFI reviewed taxi speeds, clearing of approach paths and actions when taxiing with the student. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Flying along the coast of the north end of North Stradbroke Island at 2000ft, 45 minutes into a one hour flight and the engine cut out. Initial problem solving did not provide a resolution so the pilot called Mayday and made an emergency landing on Amity Point Beach. A successful landing was made without any aircraft damage or injury to 2 POB. The flight commenced with 50L of fuel (25L each tank) for a planned one hour flight (20L/hr). Once on the ground, the tanks were switched and the engine fired up, indicating that the likely cause was not switching the fuel tanks prior to the right tank running empty. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An instructor and student were established in a circuit pattern on a runway in a RAAus training aircraft when they heard two other aircraft taxiing. One formation of two aircraft, with whom they were in contact, who were taxiing for departure from runway. There was no conflict with the formation as the two aircraft fitted in with the training aircraft operations. However, the other aircraft taxiing at the time which had announced they were going to use a nominated runway, did not acknowledge the training aircraft, yet they had acknowledged the taxiing formation aircraft.
An investigation identified the pilot of the taxiing aircraft was unaware of the communications from the training aircraft but had visually cleared entry into the alternative runway and was not aware of any conflict. A flight review was requested to be undertaken by the pilot of the privately operated aircraft which was completed successfully. |

During the 100H inspection, corrosion was found on the bolts holding the oil cooler to the firewall. These bolts are obscured from view by a piece of sponge/rubber between the inner side of the oil cooler cover and the firewall. Failure of one of the bolts may result in the oil cooler becoming loose. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On a flight from Caloundra to Bundaberg, the pilot was on a track towards Gympie and became a little concerned about some low scattered cloud and slight mist. The pilot unintentionally strayed to the right of their track at about 3200 feet which put them into controlled airspace. The pilot was contacted by Brisbane Centre and immediately descended and turned away from the controlled area. The pilot had some difficulty hearing the requests from ATC and gave some incorrect responses while still descending. The pilots' last response was they were at 2800 and after that heard no more from ATC. The pilot continued north until low cloud banks and mist patches caused them to return to Caloundra taking extreme care to remain well away from the controlled airspace. The pilot reported this to their CFI on arrival home, who sat and debriefed the situation with them. The pilot now has measures in place to avoid this controlled area in future, including a procedure to conduct a 180 degree turn and ensure constant headings are managed. |

LHS shock long strut snapped on landing roll out. The aircraft had just landed on runway 05 left (grass), normal 3-point attitude approx. speed was 20mph when the pilot heard a bang and immediately felt the left wing drop. They then thought they had done a bungee cord. The pilot managed to stop engine and brake within 25m. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Jabiru was undertaking circuits at Chinchilla airdrome with one other RAAus aircraft in the circuit. On late final on runway 14 just before touchdown the pilot of the other RAAus aircraft notified the Jabiru pilot that there was a third aircraft on late final for runway 32. The Jabiru pilot saw the other aircraft, just below the tree line at the opposite end of the airstrip, just as the call came through.
Both aircraft performed avoidance manoeuvres.
The Jabiru pilot changed frequency to 126.7 and contacted the third aircraft (VH registered) to inform them they were on the incorrect frequency and provided the correct frequency (126.35) to them. The pilot apologized and all continued flying. |

Lack of steering resulted in the aircraft running off the left hand side of the runway. No damage or injuries occurred and the student was able to taxi back onto the runway and return to the hanger. |

Increased power but severe vibration still present and unable to maintain sufficient power for level flight. At an altitude of 3000ft the pilot selected a suitable landing area ahead, transmitted a mayday call on area frequency, and carried out an uneventful forced on a property west of Turkey Beach. On initial inspection there is no visible damage to propeller as first suspected. The engine cover was removed and there was no visible damage, however when turning the propeller, no compression was felt in engine, suspected internal engine damage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While warming up, the pilot received a notice on the G3X touch that there was a hardware fault with the engine interface module. After a short pause the error cleared itself, they assumed it was from low battery voltage which was also flagged at the time. The pilot conducted an extended run up and there were no further warnings. They departed RWY10 and climbed north toward Red Rock using the autopilot in heading mode and IAS climb mode. Above Sapphire Beach, about 3NM from the airport the pilot had the warning for hardware failure again, and lost engine instruments from both screens. They also had no trim indication so decided to turn and return to the airport and announced their intentions on the CTAF. The pilot pressed the autopilot disconnect and started to turn when they noticed that the autopilot was fighting them and the lights showed that it was still engaged. The pilot tried to disconnect the autopilot again but the controls were still fighting their inputs. They pulled the autopilot circuit breakers and flew back to RWY10 where they performed a normal landing and taxied back to the hangar. |
