OCCURRENCE DETAILS SUBMITTED TO RAAUS: A pilot arriving to Sunshine Coast Airport was requested to maintain A015. On arrival into onto the downwind leg the pilot was observed by ATC to be at A011 without clearance. The pilot was then given a visual approach and landed without further incident. The pilot has undergone retraining on the use of controlled airspace and has subsequently complete a passenger endorsement. The procedures as the FTS are also being reviewed to help mitigate a similar incident occurring in the future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was taxiing to take off and was back tracking RWY 12. One emu was on the port gable markers and two emus were on the starboard gable markers, all well clear. The single emu attacked aircraft without warning (probably protecting other two emus) The pilot turned the aircraft away from the attacking emu but it ran into the rear of the propellor and was instantly killed. The engine stopped/stalled and the aircraft was towed back to hangar for inspection. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft experienced a loss of control on take-off. The pilot was unable to input right rudder.
OUTCOME: Exact cause of accident was unable to be determined. The pilot lost control of aircraft during the take-off phase of flight. This may have been due to gusty conditions at the time of the incident with improper or insufficient rudder input or as a result of a foreign object limiting the moment of the rudder pedals during this stage of flight. The member has conducted further flight training with a qualified instructor. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While an A320 was approaching runway 06 to land at Ballina Byron Gateway Airport, a Jabiru J230-D was tracking south toward Evans Head Airport, NSW. About 22 km south-west of Ballina Byron Gateway Airport, the two aircraft's flight paths intersected, with a vertical separation of approximately 600'. The A320 traffic collision avoidance system (TCAS) traffic advisory provided an alert to the A320 crew, however this has been determined to be a separate issue.
RAAus members are reminded of the importance of ensuring transponders are calibrated and operating with Alt when in flight and encouraged to review the CASA Be Seen, Be Heard, Be Safe guidance material, in addition to CAAP 166 when operating in an around busy non-controlled airports like Ballina. OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had 55 litres in one tank and nil in the other and both tanks are connected. The pilot had both tanks selected on. When the aircraft banked away from the empty fuel tank air was sucked into the fuel line and got a fuel pressure warning and the engine lost power. The aircraft landed in a paddock. Both Pilot and Manufacturer have subsequently taken actions to ensure further fuel mismanagement does not occur. The manufacturer has published an updated POH to advise a minimum of 10L of fuel must be carried in each wing tank. |

During the conversation, the instructor was told that they had conducted one of the circuits in the wrong direction. The non compliance with the airways clearance has been addressed satisfactorily by the CFI and Safety controller for the school. Attention to changes in ATC circuit direction clearances and noting them in flight can assist in mitigating the "expectation bias" than can occur when operating in CTA. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a CFI Flight assessment in the Bribie designated training area the cabin began to fill with white smoke. Both pilots initiated emergency procedures and proceeded to conduct an emergency landing at Caloundra AD. The cabin smoke was focused on the left side and venting was managed as best as possible by both pilots- ongoing checks were conducted for any incapacitation between the pilots and while uncomfortable no deterioration in consciousness occurred.
All electrics and comms were lost and it was identified as a probable battery failure as it had been replaced prior to the flight with a temporary borrowed battery.
A successful straight in approach was completed on RWY 05 in between aircraft operating on RWY12. At no time was engine power lost but rough running was observed at lower rpms possibly due to air filter damage.
Determined Outcome: Inspection revealed the aircraft's Voltage regulator was faulty, charging at 13.8 volts at idle but rising to over 18 volts at full power. This was not a lithium battery compatibility issue. No other systems were found to be damaged as a result of the over voltage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst taxiing to the runway the right main-wheel tyre deflated suddenly. The aircraft was steered in the direction of the deflated tyre until it was clear of the taxiway. On examination of the tyre, the hub-bolts were found to have sheared, but it was not possible to determine whether this was due to the tube failing first, of if the bolts had failed before the tube failed. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot landed outside the area marked by gable markers into resulting long grass, adjacent to runway 27, which caused the nose wheel to dislodge from housing and the prop striking the ground. The pilot has discussed the incident with the local CFI and will ensure all repairs are to standard, including propeller and crankshaft assessment of the engine prior to returning to flight. |
