OCCURRENCE DETAILS SUBMITTED TO RAAUS: An aircraft (VH registered) called 10NM to the north inbound stating that they were going to join on extended base leg for runway 07. It was advised by an Instructor supervising a solo student that there were several aircraft in the circuit. The VH aircraft then called approx. 6NM out stating that they were positioning to join extended base for runway 07. It was then advised by a Cessna 172 that they were late downwind for runway 07 and the instructor then radioed that a Tecnam was early downwind for runway 07. When it was late downwind the instructor made another position call as they did not have visual on the VH aircraft, and they advised that they were turning long final (was further than 3NM out). The VH had earlier made a call stating that it was happy to orbit just outside the circuit area and join on long final - it was advised by the instructor on the ground that, with the amount of traffic in the circuit, it should conduct an overhead join and another Instructor on the ground informed them that there were also 3 aircraft at the holding point. The VH continued with its approach despite the multiple calls advising of the potential risk (and angst to solo students) by joining on extended base (it in fact joined on a long final in contradiction to the ERSA and their radio calls). The VH aircraft ended up having to do a go-around due to the high traffic situation.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On a dual navigation exercise, planned to track via the Melbourne coastal route and then onward towards Melton (YMEL) aerodrome, a climb towards 2500 ft was mistakenly commenced in the 2000 ft LL class C airspace around 1 mile towards to the northwest of Laverton BOM Tower. Melbourne center made contact with the aircraft and advised that they had climbed a little too early, and they subsequently vacated the controlled airspace, making a safe landing at YMEL. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On returning to Port Pirie the pilot flew to the north of the airfield to position for a midfield crosswind join on runway 26. At 1000ft and approximately 0.5 - 1.0nm to the north of the field they called to join midfield crosswind and about 10 seconds later observed a C172 flying a reciprocal heading directly overhead with approximately 500ft vertical separation. Being aware of the traffic in the area the pilot of the CTLS called joining downwind, base and final for runway 26. While on base the C172 called downwind for the same runway. At this time, they had no reason to believe the C172 was not aware of their presence in the circuit.
On late final the pilot of the CTLS heard a radio call from the C172 stating they were joining final, at the time of the call the pilot of the CTLS recalls thinking they must be fairly close behind them and almost immediately they turned in front of the CTLS. The CTLS pilot estimates there was approximately 50-100 metres of separation at that time. At this point they commenced a non-standard left turn to avoid a collision and increase separation and then climbed to rejoin a midfield crosswind leg and rejoin the circuit. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was going to go for a short fly around the Tooraweenah area and was using the marked taxiway to enter the runway, there was an old drainage pipe underneath the taxiway which was broken and had formed a hole around 30cms wide and around 15cms deep. Taxiway had been freshly moved and hole was not visible and encountered a propeller strike with the ground. Because of the broken pipe the front wheel went into the hole and front wheel just stopped causing the prop strike. Front of plane was lifted off the ground when the propeller hit the ground. |

On the first landing, the aircraft bounced and the aircraft descend on the nosewheel which collapsed. This matter was reviewed with the student and the CFI at the flight training school. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight training, while recovering from a practice forced landing, the lesson drifted into restricted airspace (R234). After analyzing OzRunways and FLARM data, it is estimated the incursion was approximately 330 meters and up to 30 seconds in duration.
OUTCOME: No reported issues from Gawer Airspace (RAAF). Instructor to use VFR markers to ensure a breach does not happen again in the future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The flight was a dual training exercise returning to Gawler airfield. On re-joining the circuit, the aircraft veered slightly to the right whilst aiming to join crosswind on runway 31. Whilst doing so, the flight path may have crossed slightly into Restricted Airspace before joining normal circuit pattern.
OUTCOME: Instructor and student de-briefed by airspace officer and CFI on the possible breach of controlled airspace. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Just after take-off on a second circuit the pilot was alerted that the cylinder head temperatures were rapidly rising. The pilot immediately turned early downwind, throttled back to idle power and conducted a glide approach and landing onto the runway.
They taxied back to the apron, shut down and exited the aircraft. On examination they discovered a considerable amount of engine coolant on the fuselage.
The following day the aircraft was checked by an L2 who found the coolant hose had come off the inlet port of the radiator allowing the coolant to escape.
Further examination revealed the radiator inlet port had been deformed by excessive tightening of the coolant hose clamp allowing the hose to come loose. The aircraft had only 16 hours of flight time since being serviced which included the 5 year hose replacement required by Rotax. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The first lesson of the day was circuit training, after initial take-off just before the turn onto crosswind, the student indicated that there was smoke entering the cockpit, the instructor was looking out the right hand window clearing the turn at the time, they immediately noticed the same thing occurring. The instructor took control of the aircraft and entered a left hand turn for a close downwind. They advised the tower they needed to make a full stop and that they had smoke in the cabin. The aircraft landed immediately on runway 14R and taxied off the runway to the apron and the aircraft was shut down, the instructor and immediately exited the aircraft. Airport fire trucks were on the scene immediately to assess the situation with aircraft deemed safe not long after. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Instructor and student in a Bristell conducted a dual flight training at Orange airport. The Bristell made an initial taxi call for RWY 29 and a subsequent entering and backtracking call. At the time the AFRU was noted and both transmissions were made on the correct CTAF frequency (119.00).
After backtracking RWY 29 for approximately 800m the student and instructor both noticed a VH registered Piper aircraft on short final for RWY 29. The instructor immediately attempted to contact the aircraft to which there was no response. The pilot of the Piper then proceeded to initiate a go-around and simultaneously changed to the correct CTAF frequency and announced intentions to conduct a go-around.
The instructor in the Bristell asked if the pilot of the Piper had been monitoring 119.00, to which the pilot responded negative, they were accidently monitoring 119.90.
Both aircraft proceeded without any further incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During landing mechanical turbulence gust from houses to left of runway during landing roll lifted the left wing despite full aileron application and the RH wingtip and propeller contacted the ground. TAF reported 18015G25 but crosswind component within operating limits. Following this occurrence the member conducted a dual circuit lesson with an instructor to confirm crosswind technique. Members are reminded of the importance of correct control management, particularly in windy/gusty conditions. Pilots should also be aware of possible mechanical turbulence caused by buildings and other terrain in the local area. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a cross country flight which departed Boonah Airfield to travel to Luskintyre Airfield, when at approx. 17 miles approaching Kempsey Airfield from the North the engine started vibrating badly with a loss of power then stopped completely. The pilot trimmed the aircraft then started looking for a suitable place to land. During subsequent emergency landing the nose wheel has sheered off with nose leg ploughing into the ground stopping the aircraft in approx. 10-15 meters stopping short of a creek and tree line about 15 meters further on. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After departing No1 Private on property grass strip to pick up a passenger on No 2 private grass strip the aircraft touched down and struck a foreign object on the strip (later found to be a cut branch from a job completed a few days before). In doing so, the port side leg gave way and as the pilot applied brakes but the hydraulic line had burst. The aircraft came to a halt within 100mtrs without any further damage.
OUTCOME: Pilot to complete strip inspection before landings to try determine suitability before touchdown which may have prevented this incident. |
