OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had landed and was coming to a full stop. During the roll/taxi along the runway the left wheel struck an lip in the runway surface. This uneven section of runway stuck the left wheel and caused the left wheel spat to crack and become dislodged from the aircraft. There was no other damage to the aircraft or any persons and the spat was removed from the runway. RAAus contacted the aerodrome operator and spoke with the ARO who confirmed the incident. The ARO advised that the runway was due for major rework in January 2023 which would resolve the surface issue. it was advised that the aerodrome issue a NOTAM if possible in order to warn other users until surface repairs were complete. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flight the pilot commenced a climb in order to avoid cumulus cloud. As a result, the aircraft inadvertently infringed controlled airspace. Contact was established with air traffic control (ATC) who issued a clearance for the aircraft to transit and subsequently depart controlled airspace. Pilots are reminded of the importance of maintaining awareness of controlled airspace boundaries when navigating. Pilots are urged to contact ATC in the event that they are unable to remain clear of controlled airspace in order to remain clear of cloud. In such circumstances ATC will be able to assist pilots in maintaining safe navigation. Pilots should always avoid the temptation to press on into cloud with VFR into IMC a known contributing factor in approximately 1 in 10 fatal accidents in Australia. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the second flight of an experimental aircraft the oil temperature was indicating to be outside acceptable parameters. The aircraft landed immediately, with assistance of ATC, without further incident.
The in flight cowling air flow was found to be inadequate. Modifications to cowling have been implemented and tested with engine temperatures now within the manufacturers specifications. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flight at 2500 feet an unusual noise developed in the aircraft and got progressively worse over a period of approximately three minutes. Power was reduced and a severe vibration commenced. A call was placed to Brisbane Centre that the aircraft was experiencing difficulties and would be immediately commencing a precautionary search and landing. During landing the left wing of the aircraft struck multiple kangaroos. This placed the aircraft into a sliding rotation and the right wing struck an embankment. The impact with the kangaroo's caused the nose-wheel to collapse and the left main gear to be damaged in the table-drain. The pilot shut down all systems and exited the aircraft uninjured. Further review identified that the aircraft contained a substantial amount of contaminated fuel, which is likely to have caused the aircraft engine to suffer a loss of power. The amount of water contained in the fuel tank would have resulted in a fuel drain showing no distinction in color. This may have lead the pilot to believe that the fuel in the main tanks was not contaminated as there was no change in color between the water and fuel. Pilots are reminded of the importance of conducting a thorough fuel check prior to flight. |

After conducting simulated partial engine failure and forced landing training, a non-simulated loss of rpm and rough running occurred in the YBAF southern training area. After circling over suitable landing areas to troubleshoot, the pilot returned to YBAF in a shallow climb to increase options in the event of a total failure. The pilot conducted an upwind join and close circuit to land uneventfully RWY 28L. Engine cylinder pulled after incident and it was identified that value had deformed. Sent to Rotax for investigation. New cylinder fitted and Nil defect evident since. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Radio problem after departure from Canberra. On ground radio was working fine and clearance was able to be obtained without issue. After departure passing 1000' as Canberra tower was handing aircraft off to approach the pilot noticed some crackling in the radio and didn't make out all of the handoff call. Readback was carried out but was said to be unreadable from Canberra tower. Subsequent call was made by the pilot asking for a return to the field and enough of this transmission went through that tower was able to coordinate them. Subsequent transmissions from the aircraft were completely unreadable however inbound transmissions were still received at readability 3. Aircraft made a safe landing and was grounded pending inspection.
OUTCOME: Aircraft has been repaired by a L2, including removing replacing a faulty cable and solder joints. Aircraft radio has since been tested both on the ground and in the air and found to be working correctly. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was conducting circuits at YBAF when a partial engine failure occurred. No issues were noted during run up. On first circuit, there were no issues, all went as per a normal circuit.
On take-off for the second circuit, the aircraft initially was performing normally. At approx. 400 feet, serious vibrations were felt, and the engine would not reach max climb power (max rpm noted was 4995rpm), normal climb power is 5200-5300rpm. The engine issue/incident was reported to the tower, as aircraft was approaching downwind, it stabilized at 5000PM before landing uneventfully.
Outcome: No conclusive cause was identified. However, no further defects were evident after replacing the following with new - Coils, carburettors, rubber hose replacement, fuel pumps, and leads. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting a Nav flight with a student, the student conducted a supervised pre-flight on the aircraft and there was sufficient oil and fuel on board for the exercise. A touch and go was completed and the aircraft climbed out at 65 knots with full power. At 300ft AGL the engine started to sputter (multiple times). The Instructor noticed that the RPM was fluctuating and there was a vibration (similar to a wheel shimmy but in flight) in the aircraft.
The instructor then took over control from the student and tried to get as much height possible for a turn back to the field. Close to the ground the engine sputtered again.
After an extensive investigation, it was found that the right-hand fuel tank contained foreign matter which would intermittently block the fuel pick up port. The tank was removed, cleaned and reinstalled with no further defects evident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On climb out after take-off at approx. 150' the aircraft started losing power and vibrations could be felt through the airframe and control column. RPM decreased from approx. 5200 RPM to 4800 RPM initially and then gradually continued to decrease from there. The aircraft was able to maintain height at around 300' with partial engine power and the instructor opted to turn back on to the reciprocal runway. The aircraft was landed safely without further incident and the aircraft was subsequently grounded pending LAME inspection.
Determined Outcome: Borescope of number 1 cylinder found that the exhaust valve had failed. Valve renewed with nil further defects. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Instructor and student noticed unusual nosewheel shimmy during a lesson of touch 'n go circuits. They elected to call for a full-stop landing and to end the lesson. Once the aircraft had landed on the active runway, lack of directional control noted by the instructor who took control of the aircraft. The instructor established control and taxied clear of the runway.
Determined Outcome: Once the aircraft was shutdown it was noted that the front nosewheel tube had deflated. The aircraft and the rim of the tyre sustained no damage. |

After fueling at Hughenden, the aircraft left for the return flight to Donnington. The pilot landed at Pentland for a break and there was no indication of fault of aircraft or engine.
After following the shutdown and removing their headset the pilot heard a very slight trickle/creak type sound. Being that it’s hard to look fully into the back of plane whilst seated and because they felt they should move with haste the pilot got out. Leaning through the door and looking into the back and pulling out the headset at the same time the pilot caught a glimpse of a flash up close to the back of the seat. Knowing that fuel lines run across the base of the seats in back they immediately got away from the plane and only was able to take the headset and bag from the passenger seat with them. Nothing seemed to be happening and the pilot thought if it’s a small fire they could use dirt to put it out. They then approached the back door and opened it but then could see that they couldn’t stop it. It looked like the area behind seat was alight. Emergency services were called to extinguish the fire. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot didn’t realise the aircraft was descending slowly, it then touched the water and stopped. It then sank in the mouth of the Proserpine River.
RAAus conducted a follow up interview with the pilot and suspended the RPC until a BFR with a nominated CFI was completed to a satisfactory standard.
The pilot was also cautioned about conducting landings at locations which were not approved for aircraft use. |
