OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft departed YLIL at 0512z tracking north to the approach point GBR (Glenburn) on climb to cruise altitude of A045. The pilot made two calls of their intentions and level on climb to, first on departure then again on late downwind as they were departing the circuit. They reached A045 at approximately 0520z and changed frequency to 135.7 (MEL CEN) at 10NM YLIL. At approximately 0523z and witnessed a C152 in a right banking turn less than 0.5NM and 100ft from their track. Thankfully the C152 saw them first. but I do question them being at A045 when they were on a heading of (best guess) 160.
The pilot did not hear an inbound call to YLIL from the C152 but had changed to the CEN freq. They did not hear a safety alert or traffic from CEN but it was on a FIA boundary so that complicates things. The aircraft has a TT21 with 1090 ADSB out fitted and had Ozrunways traffic going as well but the pilot did not see the other aircraft via those means either.
Outcome: As the C152 has remained unidentifiable, no further investigation can be undertaken. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A Cessna aircraft decided to enter 06 at holding point Charlie and conduct an intersection departure with a strong tailwind while a Tecnam aircraft was late downwind for 24 and in the process of turning base. Despite appropriate radio calls being made, the Cessna aircraft continued with the take-off and turned right off the end of runway 06, which is not the standard left hand circuit pattern. RAAus has reached out to the Chief Pilot of the company who operates the Cessna aircraft to discuss the safety concerns regarding the reports being received. The chief pilot agreed to discuss these matters with his line pilots to try prevent these occurrences from happening again in the future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Jabiru aircraft was taking off from a private LA in which horses owned by the pilot and passenger were unfenced from the airstrip. The operators had conducted significant flights from the same location with the horses unrestrained on all previous occasions and they were not perceived as a threat. During take off roll, a single horse, who's position was separated from the herd, galloped towards the aircraft at speed, impacting the left wing and pilots door area resulting in the aircraft becoming inverted with significant damage and injuries to the pilot. The horse also sustained significant injuries and was later euthanised by the owners.
This accident is a another example of normalisation of deviance, where clearly known threats were dismissed by the pilot based on previous animal behavior at the location. The pilot will amend all future operations to mitigate this risk by restricting access by livestock to the airstrip when operating. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flare on landing, the aircraft became unresponsive to the elevator controls. The nose wheel hit ground and collapsed. The aircraft then came down heavily on mains and canted to the left wing tip which contacted ground. The prop struck ground and engine stopped.
Additional information was sough from the pilot, who advised they were not paying sufficient attention to the airspeed and the aircraft stalled approximately 6 ft above the ground. The approach speed was too low and there was insufficient airflow over the elevator, rendering it ineffective. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The brake rotor screw worked loose and was striking the calliper. The pilot could hear and feel this issue on landing. They immediately pulled to the side of the runway, shut down, exited the aircraft, and physically inspected the undercarriage. Nothing visible was detected at this time. The aircraft was restarted and very slowly taxied back to hanger. Loctite will be applied to the effected screws during future installation in order to prevent reoccurrence. |

They encountered significant traffic particularly approaching and leaving YBSS from the north and some leaving YMEL. at similar heights and below their flight path. Whilst trying to get a visual on the traffic the pilot noticed they had climbed above 3500ft and immediately descended. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Flight was a Trial introductory Flight to Barton training area. Aircraft conducted run-ups as part of standard pre-flight with nil issues observed when cycling propeller control.
On departure from runway 30 no issue was observed, and no environmental conditions were a factor. Upon leveling out, as per standard procedure the propeller lever was exercised to set 5300RPM. Upon selection of this setting a loud ‘pop’ was heard followed by a clear and audible RPM increase. Upon observing the tacho, the RPM at that moment in time peaked at around 7000RPM. The pilot attempted to reduce RPM using prop lever to no effect. Actions followed was to reduce power until tacho read under the maximum permissible RPM and set 5500 approximately for return to Canberra. The pilot elected to make an immediate turn to return to the field and was instructed to join a right circuit runway 30. Landing was made with no further issues.
Upon inspection of engine bay, it was observed that the jack screw had dislodged from the electrical motor that drives it. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst downwind on with a student, a call was heard from another aircraft (returning from a flight and inbound from the north I heard the inbound call from him and notified our location in the circuit.) notified overhead the field with intention to join " midfield downwind", we notified our position on downwind and requested position of the joining aircraft and to confirm they were number two in the circuit and requested confirmation they had us visual. He said he did not have visual, at which I gave my location downwind turning base, there was no reply, we had just turned base leg and across from our left shot the joining aircraft at which I transmitted "you have just flown past us and you have crossed out path". There was a pause and a response by the pilot of the joining aircraft acknowledging my transmission. We continued on our descent and landed. They landed shortly after.
RAAus reminds members that the risk of collision at non-controlled aerodromes is greatly increased if pilots do not confirm visually where other traffic is, or use radio to provide effective separation. Training aircraft with students aboard may fly wider circuits at slower speeds than more experienced private pilots, but regardless, circuit positions and right of way requirements must be maintained regardless of the usual circuit size and pattern flown.
Radio must be used to confirm traffic position and if a visual position cannot be confirmed, pilot should consider alternative actions, make additional radio calls and ensure they do not fly inside or conflicting positions to other established traffic. |

Of 3 aircraft in our fleet, all 3 aircraft have suffered corrosion in the same location. This corrosion developed from the inside out, due to the aft section of the longeron being exposed to the atmosphere. The RHS was corroded more than the LHS. |

On departure to the Training Area, An RPT aircraft gave their 30nm inbound call from the south and an estimate via an IFR waypoint. As the RPT aircraft was inbound from the south and the training aircraft were tracking to the north they didn't think there would be any conflict. The Training aircraft gave a departure call and another call once established in the Training Area with their duration and operating altitude. Another RPT Aircraft was departing YPMQ to the South, and stated on CTAF they would level out at 5000' until they had the RPT aircraft from the South sighted. The Inbound RPT aircraft replied with intentions to join the circuit via an IFR waypoint, not in plain English.
The training aircraft continued to fly doing steep turns when they noticed they had strayed more to the west than intended, they turned back towards the east. Now tracking East they saw the RPT aircraft in their 2 o'clock and high, now tracking southbound to the airport. The Instructor didn't think at the time they were in any danger of collision but they did trigger the TCAS alert in the RPT aircraft. Both aircraft landed with no further incident. |
