OCCURRENCE DETAILS SUBMITTED TO RAAUS: During landing on a gusty day, as the aircraft was about three quarters of the way along the runway, a gust of wind picked up the right wing, the aircraft veered off into the scrub and the pilot was not successful in keeping the aircraft straight. The nose wheel made contact with a clumpy bush, caused the spat to jam the wheel and stop turning. The aircraft flipped vertically as a result and ended up on its back.
Once the aircraft was repaired the pilot completed a Biennial Flight Review with RAAus Flight Operations focusing on ensuring stabilised approaches and airspeeds and go-around assessments. |

All pilots are recommended to conduct alerted see and avoid and make calls as recommended in CAAP 166 when in the vicinity of an airport due to the higher likelihood of additional traffic in a circuit. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst navigating from Caboolture to Bundaberg direct, the aircraft entered the western edge of Brisbane Class D airspace. This was the first time the pilot had flown following the changes to Brisbane airspace changes. The pilot realised their error and exited immediately, notifying ATC. The pilot will put a range of procedures including, updating the aircraft GPS, paper maps and become more familiar with ground based features prior to operating around this airspace again. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a circuit lesson with an ab-initio student, the aircraft was established on final on RWY12 approximately 20ft above the runway when a flock of Ibis's took flight in our flight path resulting in a bird strike with one or two birds.
RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place. |

The Instructor was conducting In Flight Adjustable Propeller endorsement training with a pilot in their own aircraft which had just been fitted with a new electric constant speed propeller. The owner and Instructor completed a preflight check with 30 litres in the right tank and 15 litres in the left tank.
The departure was to the south to complete some upper airwork with the new propeller system and then return to conduct two circuits with no issues. On the 3rd circuit, the turn onto base leg was made with Manifold Pressure (MP) set at 17". Not long after turning base the MP started dropping along with the RPM. The instructor then looked at all gauges and the throttle position and noticed the owner had moved the lever forward with still reducing MP. They realised at that point there was an engine issue and turned towards the runway whilst running through all emergency checks, with no result.
At this point RPM was still falling, speed was getting low and they recognised they were not going to make the runway, successfully conducting an emergency landing in a paddock. The aircraft owner ensured control of the aircraft was managed by reducing the Angle of Attack (AoA) and turning left into an open area. At this time the engine had come to a complete stop, no propeller rotation. All terrain obstacles were avoided and a successful landing was carried out into a paddock.
The issue was later determined to be a failure of the woodruff key in the crankshaft. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DETAILS: After full daily inspection, taxi and run up, a take-off was commenced on RWY 35 for solo circuits. Ts and Ps were all good on climb out and levelled off at 1000ft AGL. The pilot started a base turn with closed throttle and 1 stage flap selection. The temperature warning light illuminated with the needle entering red zone. The pilot decided immediately to divert to RWY 27 due their position, extra length and lack of obstacles. They retracted the flap and conducted a high speed descent to allow maximum airflow over engine whilst maintaining safe decent profile for landing. Uneventful landing whilst switching mags off immediately. An on-site L2 attended and found the hose between the rear of the engine and firewall had detached with a hose clamp that appeared to have insufficient tension. After cooling down the hose was re-attached and the coolant replaced. The engine started immediately and ran perfectly.
Members are reminded of the importance of checking hose connections after maintenance and during daily inspections. This is a commonly reported occurrence type which is often avoidable through thorough checks. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After a normal take off and climb to approx. 2500ft AGL a pop sound was heard, then a gap of about 25mm was noticed along the side of the bubble canopy followed by a draught of outside air entering the cockpit.
The pilot reduced engine power and began a slow descent and gentle bank towards the airfield followed by a call announcing the need to land back on the runway. With a clear visual approach, the pilot proceeded to land then taxi to the run up bay where they successfully locked the canopy and was able to recommence the flight.
It was found that a poorly adjusted passenger harness allowed a slack shoulder strap to obstruct the canopy lock. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was on late final over the threshold and had a stable airspeed of 53 knots, lower than the accepted approach speed of 60 knots. The student left the flare action late and required instructor intervention. The intervention included application of full power and firm back-pressure to arrest moderate rate of descent. The aircraft touched down initially on main 2 wheels and then as a result of the full power applied, a pitch up moment was observed which scraped the tail skid on the runway.
The aircraft was inspected and no additional maintenance action was required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After conducting a full stop landing at the completion of a circuit lesson, while exiting the runway and taxiing to the apron it was noticed that taxiing performance had deteriorated, a flat left main tyre was suspected. After shutting down on the apron, it was confirmed the left main tyre had deflated.
Corrective actions to prevent a recurrence included assessment of the tyres and the side walls to ensure excessive wear was not evident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flight the pilot noticed a metal on metal burning smell, they asked the passenger if they noticed it, they said it smelt electrical. At this point they were over water, within gliding distance of the land, so the pilot turned the aircraft toward the land. As they turned onto a westerly heading the aircraft lost both the Tacho & Oil Temp. there was a good oil Pressure reading but lost the temp. It was at the point the pilot chose to undertake a precautionary landing at Tumby bay. The aircraft was high at the current location so the pilot chose to do a descending turn and come out on final for runway 33 Tumby Bay. As the aircraft came over the threshold at about 20ft the pilot retarded power and begin to transition into a flare, as they did this about 20ft above the runway the engine totally ceased.
The investigation identified the flywheel bolts had failed. Repairs were undertaken and the aircraft was ready to fly, however a storm came through and flipped the aircraft upside down. OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft engine failed during take-off just as nose was rotated. As the pilot was taxiing, they noticed one tank was fuller than the other so turned the fuel cock off for that tank. During the Pre-Take-off Vital actions (PTVAs) the pilot elected to turn the fuel cock back on for take-off, but inadvertently turned the wrong fuel cock off, stopping all fuel flow. Sufficient fuel was in the system to line up and accelerate to rotate speed before the engine starved of fuel and shut down. The pilot lowered the nose wheel back onto the runway and aborted the take-off roll exiting on a taxi way.
The pilot undertook a review with a CFI and noted the correct fuel tap directions for future operations. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student pilot was conducting solo supervised circuits when the pilot of another training aircraft on early downwind became concerned with the track and proximity of the solo student aircraft on crosswind.
The instructor debriefed student pilot after landing and the student confirmed that aircraft on downwind was sighted and student was widening out circuit to avoid prior to ATC instruction to widen out circuit.
The student was advised to ensure communication with the other pilot, ATC and actions were prompt to assist with situational awareness in the circuit. |
