Doing a precautionary forced landing on the deserted beach at low tide the pilot did a touch and go. It was not a hard touch and the aircraft had heaps of power and straight and level attitude for the go. On touching the ground, the tail boom on the drifter failed bending the tail boom giving slack on the rudder cables and severing the elevator control rod. With no elevator and rudder control the aircraft crashed onto the beach from low altitude. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After dual flight training the CFI sent the student solo. The CFI was observing the student’s circuits from a hangar that overlooks runway 06. On the students second circuit, they saw the student turn base and then final. At this stage while the student was on final, the CFI observed a combination of tug and glider starting to roll on 06.
The three aircraft went past the displaced threshold together with the glider tug combination climbing under and to the right of the Tecnam. The student carried out a go-round and climbed straight ahead as per a normal circuit. The glider combination was then observed on downwind still climbing on the inside of the Tecnam. The Tecnam was then seen to make a turn right, to remain clear of the glider combination who continued to climb on the inside of them. The student continued on with their circuit and landed to a full stop and returned to the CFI a little shaken.
The airport has high traffic levels and a variety of traffic including fixed and rotor wing, glider launching, flight training and private operations. Alerted see and avoid and vigilance by pilots is a critical aspect of operations at non-controlled airports. |

On return from a flight with a friend damage to the prop was noticed, further inspection found that an exhaust manifold stud and nut had shared off hitting the prop and going through both sides of the sail of the trike. This has caused a rip/tear along the stitching of about 60mm on the bottom and some 75mm on the top surface of the sail. A bestso test has been completed to find the sail still in safe usable order. The owner is not sure if the sail can be repaired at this stage. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On arrival to Redcliffe aerodrome, the Tecnam joined the circuit overhead completing a crosswind downwind base and final for runway 07 making radio calls on all legs of the circuit. The pilot is aware helicopters do a smaller circuit pattern and saw a helicopter cut in-front of the Tecnam doing a combined base and final manoeuvre. The helicopter saw the Tecnam and conducted a go-around and the Tecnam landed safely on runway 07. The Tecnam was aware of the helicopter and in the circuit and believed they were going to give away as the conducted a go around. Radio call were made, but this is a good example of pilots ensuring a good look out is continued through all phases of flight, particularly when in the circuit at an airport. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The training aircraft had just landed after a dual training lesson of circuits. On backtracking down the runway to the exit taxiway, another aircraft landed straight towards them. The instructor pulled off the runway outside the gable markers but the second aircraft pulled up abruptly on the runway, conducted a 180 degree turn, returned to the threshold of the runway and departed the airfield, by which time the instructor had cleared the runway. At no time were any radio calls received to advise of the presence of this second aircraft and its intentions and it did not respond to calls from the training aircraft.
The pilot of the second aircraft was contacted and advised the radio was intermittently not working. Pilots are reminded of the requirement under Part 91.245 and Manual of Standards Chapter 10.d to ensure the radio and other equipment aboard if working correctly prior to flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was tracking north bound climbing through 8000' for 8500' and failed to realise they had broken into the step for Coffs Harbour airport. The pilot self reported and notes they fly in the area regularly however were distracted due to adverse weather including mountain waves and moderate turbulence. The turbulence had resulted in a decision to climb higher than normal for the area to avoid turbulence, which resulted in flight at a higher level than normal for the area.
The pilot will complete a BFR focusing on CTA avoidance and decision making prior to departure, to review weather forecasts and plan the avoidance of CTA more carefully. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst on final approach to runway 24 at Clifton, a VH registered aircraft did a low level turn in front of an RAAus aircraft and cut them off. The VH registered aircraft then landed long and overran in the end section of the runway. The RAAus pilot commenced a go around and landed. The RAAus pilot confirmed their radio was operating correctly and reports that they did not hear the incoming aircraft. As this occurrence involves a VH registered aircraft, it has been forwarded to CASA for review.
RAAus reminds all pilots of the importance of maintaining standard circuit procedures as well as the use of radio calls where required and maintaining an active lookout at all times. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: As the aircraft approached the airport from the east the pilot gave 10m and 5m calls and joined midfield crosswind for runway 03, diverting for parachutes. The pilot re-joined long downwind 30 and landed and gave calls for joining downwind, joining base and joining final. When the aircraft was back in the hangar the pilot was advised by an instructor that there had been two other aircraft in the pattern but they were using runway 21. The other aircraft had attempted to call but the pilot did not answer. It was then the pilot discovered they had not changed to the CTAF frequency when returning from a local flight.
The pilot has now initiated corrective actions to ensure the correct frequency is selected when returning to an airport and will not assume there is no traffic just because there are no calls. |

At this point a dust devil formed and passed through the right wing initiating a right hand yaw and incipient ground loop. Power was reduced and full left rudder and back elevator applied however the nose continued to pitch forward and a prop strike occurred. The engine shut down on strike. |

The aircraft was enroute between Yarrawonga and Kyneton via Seymour at altitude 6500ft, it had passed Mangalore and was approaching turning point at Seymour when the engine suddenly ran rough and started vibrating. The pilot turned back for Mangalore. The vibration became severe so closed the throttle and engine stopped completely. The pilot made a mayday call on the Mangalore CTAF and informed anyone on that frequency that they were intending to make a landing on runway 36. At the time of the engine failure they were about 7 to 8 nautical miles south of Mangalore so within gliding range. The aircraft made an uneventful if somewhat rough landing on 36.
The owner has since recovered the aircraft and checked cause of failure. The initial failure appears to be a fatigue failure of the #5 exhaust valve. The #5 piston was completely shattered and the connecting rod buckled. The crankcase has been distorted by the internal debris. |
