OCCURRENCE DETAILS SUBMITTED TO RAAUS: The lesson consisted of circuits with the student flying low passes along the runway without landing to practice directional control. The runway in use was 02 and the wind was <5kt with no crosswind component. The student was demonstrating greatly improved directional control of the aircraft as the lesson progressed. On the last landing to full stop the instructor told the student the instructor would fly the final approach and landing but asked them to follow on the flight controls to experience the feel of the transition from stabilised flight above the runway to landing. The approach was ideal and the main gear touched the runway at ~65kt 50 metres from the threshold. At this point the student suddenly pulled the stick back hard and the instructor was unable to counter this quickly enough with their right hand. The aircraft rose about a metre into the air again but with insufficient airspeed to maintain flight landed heavily on all gear, damaging the nosewheel oleo under the vertical force with consequential prop tip damage from runway contact. Braking action arrested the roll on the runway centreline at taxiway Bravo. RAAus interview with the instructor indicated there was no expectation of this input from the student and they have debriefed with the CFI about ensuring clear communications with students about who is in command during nominated phases of the training.

During flight training in forced landings, the undercarriage was not deployed. The aircraft landed on gravel runway. The CFI advised the focus was on maneuvering for the simulated emergency and normally for simulations a go around is conducted at 500; AGL. As the location included a private strip, the exercise was continued to the ground and the pre-landing checks were overlooked. The CFI has implemented a requirement for pre-landing checks regardless of any simulation.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Wind shear on final for Runway 03 at YLTV. The landing was the second attempt after an initial go around due wind. There was lots of lift and sink. The aircraft touched down initially before bouncing up to three times, before the nose gear failed and caused a prop strike. The aircraft came to a stop near the centre of the runway.

While descending to Dubbo at approx. 15nm, an aircraft was observed on TCAS, 1-2nm northwest of the 05/23 centreline at approx. 1000 ft AGL. Shortly afterward, ML CTR advised that an unidentified VFR aircraft was operating in the circuit and unconfirmed callsign. The RPT made numerous calls on CTAF and one call on ML CTA freq. to establish contact with the aircraft but no reply was received. The RPT diverted to the northeast and held 2000 ft above the runway (1000 ft above the Jabiru) to overfly for a wide crosswind leg Rwy 05 (W/V on AWIB 080/16). Additional circuit calls were made to alert Jabiru. Visual contact was made with the aircraft from early downwind position, and they observed that the Jabiru was positioning for a close-in final Runway 11. They continued to fly the circuit while maintaining visual contact with the Jabiru through its landing and until it vacated Runway 11. The aircraft temporarily entered Runway 05 to allow it to complete a 180 deg turn prior to backtracking Runway 11. The RPT was at approx. 600 ft on final approach, when this occurred and poised for a missed approach until it became clear that the Jabiru had cleared Rwy 05 and was established on its backtrack… They landed on Rwy 05 without conflict. After landing and while backtracking Rwy 05, the Jabiru was heard to report “clear of all runways”. This was the only transmission heard from the Jabiru. RAAus interviewed the Jabiru pilot who advised of a radio issue he was trying to correct. We discussed the CASR 91.245 requirement to confirm the radio was transmitting and receiving and the pilot will make use of AWIS and AFRU to confirm the radio is serviceable for the future.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft experienced an engine failure soon after take-off at about 60’above runway with gear just retracted. The pilot judged there was just enough runway remaining to land so glided onto runway with the gear still retracted. The aircraft slid along then stopped on the runway from which it was soon removed with the help of Aviation Centre personnel at Tyabb.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Aircraft was preflighted and refuelled for a private flight. On take-off roll the Portside fuel cap fell from the aircraft onto the runway. During the flight the fuel level was monitored and no loss occurred. At the conclusion of the flight the starboard side fuel cap was inspected and found to be firm. Upon reflection it would appear the loose cap was not noticed during the check at the conclusion of preflight. Procedures have been implemented to minimise the risk of this occurring again.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Student was undergoing a pre-certificate test flight in their aircraft. Final circuit was a glide approach onto RWY 26, with a considerable cross wind from the south. The winds exceeded the aircraft's (and the student's) crosswind capability, so the landing was poor, resulting in a couple of bounces with some lateral load on the front nose leg. They went around and landed uneventfully into the wind on RWY 17. The instructor didn't consider that the landing was particularly hard until the student called to their attention that the nosewheel was bent.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Aircraft suffered a significant loss of power while taking off. This occurred at ~150-200 ft altitude AGL. Throttle was closed and aircraft landed with half flap on the remaining runway without incident. Engine continued to run at idle and subsequent run up to full power on the apron appeared to be functioning normally. The upper cowl was removed, inspection revealed nothing. Subsequent run up appeared normal. Five circuits were then flown without incident. This aircraft has a long history of suffering from carburetor ice. Relative humidity at the time was high 90s and sky was 3-4 octas of cumulus with bases at ~3500 ft. Under such conditions this aircraft is always operated with carb heat ON and that was the situation this morning. Start up, warm up, run up and take off were all conducted with carb heat ON however there have had previous similar events where the carb ice heat system was insufficient to prevent ice. In those instances the engine just ran a bit rough after take-off however in this incident the power loss was such that flight could not have been maintained.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An Evektor Harmony departed YSHL and tracked East towards the coast. The pilot heard an RPT call “approaching from the South West”. They then heard them call "Overhead the airfield and commencing the NDB Alpha approach. Being unfamiliar with the NDB Alpha approach, the Harmony pilot replied directly to the RPT stating they were 5 miles to the East at 2,000 feet and turning to track South. The RPT replied "Roger, thanks for that, we'll be at the inbound turn tracking for runway 34" The Harmony is fitted with ADSB In and Out. The pilot observed the RPT approaching from the right on the Dynon Skyview screen and saw the aircraft visually 3 miles to their right and well above their height. The Harmony continued to track South and descended slightly. The pilot observed the RPT pass in front of them from right to left at what appeared to be at least 500 feet above them. Shortly after the RPT had passed and commenced a turn to join downwind Harmony turned left 180 degrees and tracked back to the North. The RPT made no further calls to their Harmony after the initial "Roger, thanks" to indicate there may be any conflict. The Harmony pilot observed the RPT make their approach and heard all their remaining CTAF broadcasts. The Harmony pilot was unaware at the time or afterwards that there had been any traffic conflict between the RPT aircraft and themself.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While an instructor was performing right-hand circuits on Runway 34 with a student pilot, a VH registered Piper Warrior transmitted on the radio indicating their descent on the "deadside" of Runway 34. Due to the non-standard phraseology, the instructor requested clarification from the pilot flying the VH aircraft regarding their location and if they were on the western side of the aerodrome (which is the non-active side of the circuit for runway 34). The pilot flying Piper Warrior responded, stating they were to the east of the aerodrome at overfly height. Seeking further information, the instructor requested the pilot flying Piper Warrior to confirm their altitude, to which they reported 2,600 feet. At this moment, the instructor visually identified the aircraft executing a left-hand descending turn on a late right downwind position for Runway 34, contrary to the established circuit direction. The Piper Warrior’s trajectory posed a conflict with the prescribed circuit direction. Immediately recognising the potential collision risk, the instructor alerted the Piper Warrior about the instructors’ aircraft position relative to theirs. Subsequently, the Piper Warrior initiated a climb and passed overhead the instructors’ aircraft on a northerly heading. They informed the pilot of the Piper Warrior that the prescribed circuit direction for Runway 34 at Cootamundra Aerodrome is right-hand, to which they acknowledged. The Piper Warrior then departed the Cootamundra on an approximate westerly heading.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Defect: Both attach points of the wing have elongated holes and the wing was loose. OUTCOME: After discussions with L2 and the LSA manufacturer, it was determined that this aircraft is the only one built incorporating an eccentric bushing to adjust the wing camber. It appears that one of the bushings may have rotated and become loose. Affected parts will be replaced, and the aircraft manufacturer will collaborate with L2 to ensure this issue does not recur. Further, RAAus technical support has now provided the LSA Brumby maintenance manual to the RAAus maintainer.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The flight was to transfer the aircraft from the current hangarage to the new hangarage location. Take off and fight to height was uneventful, the pilot made a 5miles inbound call to land made, approx. 2 miles south of Ozler, when the engine stopped. The pilot checked fuel and attempted restart the aircraft however, this failed so they declared Pan Pan. The pilot was looking for landing spot (the old Flinders highway, now unused and unkept), none could be seen so they were aiming for possible marsh landing beyond any trees and bushes. When approx. 150ft AGL the pilot declared second Pan Pan, unfortunately they didn't have enough airspeed or height to clear the last row of chine apples and mulga bushes, they avoided a fairly large tree on right but hit in the middle of dense bushes and spun the aircraft 180deg and impacted trees, bushes and the ground.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Instructor was undertaking a Stalls lesson with a student. Approx. 50 minutes into the flight at a location approx. 15NM north of YGSD and 3500'AGL, after conducting a stall, the student attempted to recover using power but the engine only revved up to 2300 RPM and ran lumpy. The lesson was immediately terminated and returned to YGSD by the shortest route. After engine recovery steps made no impact on the engine performance, removing carb heat and then cycling the throttle made the engine recover power as per normal. The aircraft was landed without further incident and the aircraft was taxied back to the hangar. Inspection of the engine found that the idle mixture adjustment screw had worked loose to 3 1/2 turns from closed (should be 3/4 of a turn), and the O ring on the needle was worn which would have reduced the friction on the needle allowing it to rotate when the engine was running.
