OCCURRENCE DETAILS SUBMITTED TO RAAUS: After landing runway 17 at Tyabb without incident the aircraft was taxied off the runway and proceeded to taxi to the parking area. After approx. 100m of taxiing the aircraft came to a sudden stop. Following the aircraft being pulled to a hangar with a dolly under the stuck wheel and the spat being removed it was found that the rim had ‘sheared’ into two.
The reporter has contacted the manufacturer who stated this was a common occurrence in this type of wheel over 25 years old (these wheels are 26 years old) so it is likely due to metal fatigue. It appears this cracking or fatigue was missed at the annual inspections over time. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While undertaking an Area Solo training flight and approx. 5 miles East of the airport, the student inadvertently entered cloud. The student-maintained level flight but did not want to descend below 1600 feet AMSL because they had heard aircraft performing 1500 feet circuits.
After being in cloud for about a minute the student made a Mayday call on CTAF advising they were in cloud at 1600 feet. The supervising instructor immediately called the student and advised them to keep the wings level and reduce power to begin a descent. The aircraft broke clear from cloud about 5 seconds later at about 1550 feet and the student was able to report visual and aware of their position.
The student has returned for further training including solo training. The instructor has been advised to be more vigilant when send students on T/A Solo activities with low cloud or poor visibility. |

The tail wheel disintegrated on landing resulting in debris on the runway. The tail wheel weakened over time due to exposure to UV radiation. This item was noted on the maintenance record however was not replaced. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Returning from training area with a student onboard, a Piper aircraft which had been instructed to follow their yellow Bristell aircraft had made an early turn which resulted in the Piper cutting in front of their aircraft. Both the student and instructor identified the conflicting traffic coming from their right. The instructor maneuvered the aircraft to avoid the traffic. Tower contacted the Piper aircraft and instructed them to go around. The instructor also contacted tower to let them know they were maneuvering to avoid the traffic.
OUTCOME: Pilot visually identified conflicting traffic within controlled airspace and took appropriate action to avoid a collision. |

On arrival from the Northwest the PIC was unable to confirm the location of two other traffic and chose to over fly to the East. It was at this time he chose to make a right hand turn to track for a 4nm final onto RWY 36 yet found himself on final for RWY 05 and landed with Nill Incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On the 02/10/23 PIC Checked the weather in the morning and prior to flight Departed YGAW (Gawler Airport) for Truro Flat and then conducted a touch and go and continued to the East to look at the Murry River, as the winds were starting to pick up the PIC started the return trip back to YGAW. On arrival the pilot conducted two go-arounds due to the gusty conditions on to RWY 31 as the winds were predominantly from the West. On the Final touch down, the aircraft landed to the right of the centre line and once the nose wheel touched a strong gust of wind turned the aircraft to the left causing it to depart the runway, applying maximum breaking the aircraft came to a stop just prior to the taxi way then taxied back to the hangar where they found the right tyre had deflated due to picking up a 3 horned Bindy. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: At 5500ft on cruise there was a slight reduction in power so the pilot turned the fuel pump on and pulled carby heat on. The RPM was reduced to 1200 and remained at that. A station was to the left, so a landing site was selected. The engine stopped at around 1000ft on descent. The landing was successful along a graded fence line and about 2 km from the homestead. Fuel was dripping on to the spat of the nose wheel. After closer inspection fuel was overflowing from the carburettor. A mechanic was notified, and the problem rectified. Most likely the cause was particles caught in the needle valve and flooded the engine. The aircraft proceeded without any problems after the incident.
OCCURANCE OUTCOME: Aircraft was repaired and flown back to base. Further investigations into the reasons why the failure occurred continue with the manufacturer. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On landing on RWY 23, the tail wheel bracket broke in half and wheel came off aircraft. In the process, a small hole in the fabric in the rudder. The pilot could hear there was an issue and proceeded to stop the aircraft and vacate the runway as soon as possible. They conducted a shutdown and inspected the damage.
There was no loss of directional control and no injuries. L2 is currently inspecting damage and failure of component.
This is an amateur built aircraft, and it is unknown the grade of metal used for the tail wheel spring. Usually this is made from a high-grade spring steel as it is is susceptible to high loads and metal fatigue over time. The tail wheel spring was replaced, and the fabric was repaired with nil defects found. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Incident occurred during landing at home field after a cross-country flight. The aircraft was set up for a longer than usual final approach with two stages of flap, maintaining 60 knots on short final descent and steadily reducing thereafter (over the trees immediately before the threshold). Because of the reduced runway length the pilot was aiming to land at the very start of the runway. Touchdown was heavier than the pilot was expecting. The nosewheel also came down sooner than they had intended so they immediately raised the nose, holding it up for the remainder of the rollout. It was only while pushing the plane back into the hangar that they noticed damage to the nose wheel.
OCCURANCE OUTCOME: CFI contacted, pilot debriefed and aircraft inspected, repaired and returned to service. |

A student was on a solo navigation exercise and became lost, they falsely identified their position as lake Somerset where it was most likely Lake Wivenhoe. The student communicated with two senior instructors by way of mobile phone texting. Unable to confirm the location of the student the SI directed the student to contact Brisbane Centre for assistance. The student was directed to a controlled airport and landed safely. There was considerable smoke in the area from local bush fires reducing visibility. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While running up the engine the nose gear fork collapsed.
Engineer reports metal fatigue as the most likely cause. It is noted that this model aircraft has the same fork as earlier models which have much less weight on the nosewheel. Later similar models have a heavier fork. Aircraft agent has been notified about this and will pass this onto the factory. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While conducting circuits, the student was flying an approach to RWY 35 and the approach became unstable. The CFI attempted to apply power for a go around and the student was holding the throttle lever in the closed position and did not realise the CFI was trying to add power.
The landing was completed successfully however, the throttle arrangement prevented the CFI from applying go around power.
Resolved after interview with the CFI by ensuring a clear briefing about the requirements to relinquish the controls if called for by the CFI. |
