OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a normal landing with instructor and student on board, the L main tyre deflated. The aircraft was taxied to a stop on the runway, then pushed outside the gable markers and made safe. OUTCOME: Flat tyre cause unknow. No damage to aircraft and no injuries to instructor or student.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting circuit training on runway 05 at Gawler, the aircraft breached the western boundary of Edinburgh airspace for about 35 seconds and intruded by about 300 meters. The instructor completed a be-brief with the CFI in relation to the incident and was reminded to use ground based references to ensure that future breaches are avoided.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual circuit training, down-wind calls were given. On final a blue mower was operating a westerly direction, having entered the runway area since they took off. It was mowing directly adjacent to the bitumen. Due to the location of the mower, a go-around was initiated. The same occurred on the next circuit. Later when they returned, the mower had situated its self outside the gable-markers and the aircraft landed without further incident.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the take-off run on runway 23, at rotation a loud noise was heard with a severe vibration. Take-off was aborted, power pulled back and the magnetos to off. After exiting the aircraft, it was discovered that part of 1 blade of the propeller was missing, and another was damaged. No other visible damage.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A flight was planned for training purposes to be conducted from YSCB to YTMU. The aircraft pre-flight was conducted with no issues found (including fuel dip and drain) and run ups showed no evidence of any issues. After passing approx. 300ft AGL, after take-off checks were conducted. Flaps were retracted, landings lights extinguished, and the auxiliary fuel pump was turned off. At this point, a low fuel pressure warning was noted, and the tower was contacted with a requirement to immediately return to the field. After a short discussion with the tower, a normal circuit was conducted (a little tighter than usual to account for a possible total loss of power) and a landing was conducted without further event. No emergency services were required or requested. After landing, an investigation of the fuel system revealed a fouled fuel filter. Back washing of the fuel filter revealed substantial foreign material to be present in the fuel filter (see attached images) Subsequent drains of the tank and examination of fuel remaining in the tank revealed no further contamination. The fuel system was flushed, and the filter was replaced and the low fuel pressure issue was rectified.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Partial engine failure during climb from 2000ft to 3000ft, rough running with vibration at reduced power. Aircraft was successfully landed back at departure aerodrome. The engine was removed from the aircraft and returned to Jabiru for repair. Inspection revealed that the cause of the partial power loss was as a result of a broken valve spring. Engine has since been repaired and returned to service.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During circuit training, the instructor allowed the student to extend the downwind leg to show what happens when they get distracted or absorbed in their downwind checks, without realizing themselves that they had flown into restricted airspace. Instructor reminded to use ground based reference markers to ensure that the aircraft does not enter restricted airspace. Instructor was de-briefed by CFI in relation to incident.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The electric aircraft was conducting circuit operations when during the climb for the 4th circuit at 600ft AGL, an instrument display warning appeared 'Power control lever failure' which resulted in complete loss of power. The pilot pitched for a glide and a turn back, attempted a reboot cycle of all switches off, pulled the power control breaker, powered on all systems in sequence but the same warning appeared. They repeated this one more time, but power control was not restored. The pilot carried out a successful emergency landing in a field to the north of the runway. DETERMINED OUTCOME: Upon close and detailed inspection of the power controller circuit and connectors on the aircraft, it was identified that the white wire pin in the power controller Molex connector J104M at the main controller PCB was not seated correctly in the Molex connector and was making intermittent contact. This connector had not been handled, serviced or disconnected at any time since the aircraft was delivered from the manufacturer. The pin has now been correctly clipped and seated in the Molex connector to make secure and consistent contact. A tension test was conducted on all pins in the main controller PCB and found no other pins to be loose or 'unseated'. A detailed visual inspection of the power lever PCB and mechanism was completed and no anomalies were found. Initial run-up tests after these actions were taken, including shaking the wiring looms by hand during low-power tests, indicate the loose connection has been resolved.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Nose wheel deflated after landing leading to vibration and heavy steering. Aircraft stopped and removed from runway for repair. OUTCOME: Aircraft repaired and returned to service.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst taxiing the pilot made a taxiing radio call prior to crossing the runway. They turned the aircraft so they could see all the way down the runway and the pilot and passenger both checked to make sure there were no aircraft on final. The pilot then straightened the aircraft and proceeded to cross the runway when they noticed a trike landing which just missed the tail of the aircraft. The pilot continued on their flight. On return to the airfield the pilot made a 10 mile call and also made a downwind call. As they were rolling out from their landing they noticed that the trike was landing on a crossing runway. No calls were heard from the trike pilot. RAAus has reviewed this occurrence with all pilots involved. Failure to make neccessary radio calls is a breach of the Civil Aviation Act and may result in a significant degradation to safety. All pilots are reminded of their requirement to adhere to legal requirements relating to mandatory radio calls to minimise the risk of mid-air collisions.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During flight, the pilot noticed that the aircraft was flying right wing down which they tried to correct with control inputs. Shortly after, they noticed that one tank was dry and the other had 30 litres. As the right wing was low the fuel was below the fuel pick up point and the engine ran rough, then stopped momentarily. The pilot then decided to land in a paddock. A possible issue with fuel tank vents not allowing fuel to equalize during the flight has now been corrected. It is likely that the failure to maintain level flight resulted in a fuel inbalance followed by a subsequent fuel starvation.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst on a training flight the student was taxing and started to drift left towards taxiway edge. The instructor went to takeover, but the student still had some pressure on pedals reducing the authority of the instructors input. The instructor quickly shut down the engine and coasted onto the grass. No damage and no injuries. The instructor was reminded to provide a clear brief to the student in relation to who has control of the aircraft and when. This would have allowed the instructor to state "I have control" and the student should have released all the controls allowing the instructor to make the appropriate correction.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot of a gyrocopter backtracked from mid-field of a runway whilst another aircraft was waiting at the hold. The aircraft waiting at the hold believed that they were first for departure and as such, entered and taxied around the gyrocopter. The aircraft then decided to take off in front of the gyrocopter. This occurrence was reviewed with both pilots. The pilot of the aircraft has been educated on legal rights of aircraft on runway whilst the member of gyrocopter has been educated in relation to courtesy of conduct when operating with other aircraft at the hold.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During initial climb, the engine ran rough and the pilot returned the aircraft to the airfield. The aircraft was grounded and inspected by an L2 maintainer who checked and replaced multiple items including fuel system and engine components. Possible cause was a smaller than usual fuel return orifice that has now been rectified. Aircraft has completed numerous test flights without further issue. Aircraft has now been returned to service.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A Student had completed a pre-solo check with their instructor. The student was then briefed for their first solo which included the conduct of one circuit only and the go-around procedure in the event of an unstable approach. The student completed a nice take-off and circuit spacing/approach, however bounced during landing, so conducted a go-around. The instructor had a hand-held radio so talked the student through two further attempts at landing which resulted in a safe landing. The student conducted a de-briefed by the instructor and CFI on the first solo and go-around.
