OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst performing a short field take-off the pilot raised the tail excessively and then made incorrect inputs in an attempt to correct by reducing power rather than bringing the stick back. The propeller impacted the ground and the engine will be assessed and repaired in accordance with the maintenance manual requirements for a propeller strike. The pilot will undertake additional training to prevent a recurrence. |

The flight was being conducted as part of a tail wheel endorsement. On the third touch and go the instructor pilot commented a slight oil smell after take-off. Just after this comment the CFI of the training school, who was watching the flight from the clubhouse veranda, called on a hand held radio that smoke was trailing from the aircraft. Given the possibility of a fire or impending engine failure the instructor elected to land as soon as possible. As landing straight ahead was not an option a modified circuit to the cross runway was flown. Oil pressure and temperature remained within normal limits during the approach and landing. The aircraft was shut down with no further incident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Failed landing attempt, touch and go with a severe take-off stall. The left wing impacted the ground tearing the wing off the aircraft which then continued to rotate before impacting the ground inside the airfield boundary. Aircraft attained a height of approx. 50 feet AGL before the stall occurred resulting in a very heavy impact. |

The landings were all conducted on runway 04. This final landing was gentle, but with a slight gust from the right. The aircraft began a ground loop to the right, resulting in the right undercarriage leg collapsing. |

The aircraft suffered a partial engine failure on climb out during take-off. On initial indication of rough running, the pilot commenced turn back to reciprocal runway, (whilst making a radio call ‘landing back on’) and landed diagonally across the verges of the runway.
Once stopped, a radio call was made to aircraft in the area that they were on the ground, safe and clear of operational areas. Initial inspection of the aircraft indicated no apparent damage so taxied back to the apron.
Closer inspection found that the R/H MLG leg was deformed as a result of the heavy landing.
In discussing the incident with the CFI, the pilot recounted that whilst they were doing pre take-off checks, they had inadvertently turned both fuel taps to the off position.
The pilot noted for the future they would ensure greater focus on adherence to Check List requirements, and the importance of line-up checks. |

Whilst returning to airfield after a local area flight, the engine hesitated. Temps checked and were all OK. Rpm started to drop, the boost pump was turned on but made no difference. The throttle was opened slightly and the engine started to run down. Reducing throttle slightly, the engine continued to run down then gradually stop. A field was selected for landing, and an attempt at restart the engine was made, it turned the engine easily but no response. A Mayday call made (CTF frequency) but no response was heard. The aircraft landed safely in a field, the pilot made a radio call advising the safe landing but again heard no response. The pilot phoned a friend at the airfield who confirmed hearing Mayday call but not the "landed safely" call. Later when the recovery crew arrived, the engine systems were inspected including fuel level/contamination check of fuel bowls. Everything appeared satisfactory except cracks were noted in the carburettor rubber sockets. The engine was started Ok but a little roughness was noted. |

While climbing out after utilising runway 11 and at approx. 900 feet AGL a loud bang was heard along with a substantial vibration from the front end of the aircraft. At that time power was reduced and a gentle northerly 180-degree turn was made without losing altitude back to Orange airport. A local emergency was declared on the CTAF and a glide approach was made for runway 29 followed by a successful landing without further incident.
Inspection of aircraft was carried out at the hanger and it was discovered that the left hand side air intake for cylinder cooling was cracked. The top cowling cover was removed and green nylon fabric was tangled around the propellor shaft inside the cowling area.
Prior to the flight the aircraft owner conducted a thorough preflight inspection however the pilot conducted their own as they always do and the aircraft was deemed safe to operate.
It is possible the aircraft taxiied over a rope or tiedown material and this became entangled around the propellor.
The engine and propellor are undergoing post propeller strike actions prior to returning to flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual instruction, on final approach with approx. 8kts crosswind right to left. The student maintained a stable approach with crosswind technique crabbing into wind. A strong gust increased the crosswind upon touch down and forced the aircraft further to the left creating an undesirable state. All of this occurred in an extremely short space of time. Possible rotor from nearby buildings may have contributed to the accident. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft touched down on a private airstrip, then veered suddenly to the left and the pilot was unable to correct in time. The aircraft hit rough ground, a fence and steel post with the left wing and left wheel. The aircraft rolled and came to rest inverted. The pilot turned off all electrics and vacated the aircraft.
Pilots are advised to exercise caution when landing on private strips as the surface, size and surroundings may not be within the aircraft or pilot capabilities or may be subject to micrometeorology or other weather issues. |

The pilot was seen to approach the airstrip, flare too high and as a result, pushed the nose of the aircraft down and bounced twice. The aircraft suffered undercarriage damage. It was revealed the pilot did not hold the required endorsements to operate the aircraft in that location and they have been referred to their local CFI for retraining as required.
Pilots are also encouraged to read Recreational Aviation advisory Publication (RAAP) 1 - Type Training to ensure they understand the considerations when operating a new aircraft type they are unfamiliar with.
Operations of aircraft without the appropriate training presents a possible safety issue and if the pilot does not hold the correct endorsements is a breach of Section 20AB of the Civil Aviation Act (1988) as an offence of strict liability. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An Instructor and student on a navigation flight spotted a Savannah aircraft to their left approximately 100 metres away converging on their track. (It was 90 degrees to their aircraft). The Instructor reduced power to idle and dived right. They attempted to contact the pilot however there was no answer.
Pilots are reminded of the importance of lookout when operating at all times. |
