OCCURRENCE DETAILS SUBMITTED TO RAAUS: On approaching the runway (23) the pilot of the FK9 saw a tug and glider in the distance and believed they were on the grass to the side of the runway. The pilot knew the gliders were operating from about the mid length and in 23 direction. The FK9 pilot called entering and the glider replied that they were to take off in 3 minutes which gave the FK9 time to take off before they rolled. The FK9 is a taildragger so the pilot did not see them over the nose until they were airborne when they judged the safest course was to continue the take-off. The FK9 pilot was surprised to see the glider was on the runway and not the grass. Lack of effective communications between the glider launch operation and the pilot of the FK9 was determined to be a causal factor.

Whilst flying at 4500 ft, the pilot sensed a smell of burning wire and shortly after cabin started to fill with smoke. The landing was satisfactory with no injuries or damage to plane. No fire occurred in cabin. As soon as landed all electrical systems and engine were turned off. After the engine stopped the battery was disconnected. The smoke dissipated. The pilot called the airport coordinator at Albany airport and explained what had happened, and they alerted responsive parties including police.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Upon completion of flight instruction work at the Aerodrome, the instructor boarded their aircraft with the intention of ferrying the aircraft back to their base aerodrome. A Standard Operating Procedure established for some time was to survey the aerodrome parking area before starting the aircraft to ensure there were no obstructions such as other aircraft or ground vehicles towing gliders etc. The instructor noticed an aircraft had commenced taxiing towards the South Eastern runway so they delayed start up with the intention of not causing themself excessive taxi and waiting time. The instructor heard no taxi call from this aircraft which proceeded to the holding point of runway 16 and stopped. The aircraft then made a left turn and proceeded back to the parking point in which it had come from. The instructors assessment of this unusual back track may have been the result of a student reassessing their departure protocols. As the instructors aircraft had been recently flying and the motor was still warm they started up, prepared the aircraft for take-off, taxied to the holding point of runway 16 for an intersection entry to the runway followed by a downwind departure to the north . The Instructor was informed that there was a Flying Doctor aircraft approaching their departure runway from the South once they became airborne, they did not hear any radio calls from this alleged aircraft nor did they notice the approach of it but upon receipt of this information they immediately made a 90 degree left turn and continued to fly their crosswind and downwind legs uneventfully .

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student and instructor had started a lesson and were holding at the taxiway for a FlyDoc aircraft that they had heard on a 5 mile final for runway 34. While they were holding a Jabiru aircraft taxiied around them on their right hand side and appeared to begin to to backtrack runway 34. At this point, FlyDoc called on the radio for a one mile final runway 34. This was when the instructor noticed that the speed of the Jabiru was beyond taxi speed, and they were in fact taking off on runway 16 - into opposite the oncoming path of the FlyDoc aircraft. At this point, the student and instructor had taxied back onto the apron in anticipation of the FlyDoc aircraft needing extra space to vacate the runway, and there was nothing they could do but watch as the Jabiru made an avoidance turn to the left to clear out of the way of the oncoming traffic. Pilots are reminded of the importance of listening to the radio and establishing any relevant traffic prior to entering a runway.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Bird Strike of right wing tip shortly after take-off (100/150ft AGL). Flew a circuit and returned to land without issue. RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a dual flight, the Brumby was on mid final runway 03 at approximately 300 ft AGL for a fullstop landing. Another aircraft was sighted conducting a departure on runway 21, 1nm ahead passing 200 ft AGL. No radio calls were heard from the departing aircraft. A go around was initiated with a right turn to the east. The conflicting aircraft was seen to also conduct a right turn to the west. Attempts were made to contact the aircraft with no response received.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The flight was a return leg of a cross-country flight which originally departed from Maitland, to Port Macquarie, then back to Maitland. The pilot had originally planned to leave Port Macquarie at 0635Z, which meant they were due to arrive at Maitland by 0735Z. The departure had been delayed due to two departing and one arriving RPT aircraft and then another departing aircraft after that. This delay pushed the departure time to 0645Z. The pilot still had planned to arrive exactly 10 minutes before last light. After noticing the aircraft not reaching points as quick as it should have, the pilot recalculated and found their ETA to be 0755 for the circuit at Maitland, they then requested tracking through Williamtown restricted airspace to ensure they could reach Maitland in a timely manner. The pilot received help from Williamtown centre controller who assisted with vital information and support to get them on the ground safely. The pilot had the established visual on Maitland aerodrome and made the call for a straight in approach, runway 23. Their touchdown time was approx. 0758Z. The pilot was appropriately qualified to operate in active military restricted airspace which requires current CASA qualifications. Pilots are reminded of the importance of ensuring adequate time is allowed to account for delays, particularly when intending to arrive close to last light on longer navigation flights.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: At early downwind on the fourth circuit at a controlled airport, the pilot received no response despite making several radio calls. They tried troubleshooting actions including confirming the volume level, confirmed the frequency was correct and checked their headset and got no response. The pilot assumed a radio failure had occurred. As traffic was clear, they landed on runway 06R, taxied off the runway and called ground 3 times. The pilot checked for traffic, crossed 06L and taxied back to the hangar. The pilot was debriefed by a RAAus CFI and realised they should have included the phrase "transmitting blind" and checked for light signals from the tower.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During a dual instructional flight, on upwind following after take-off checks the aircraft experienced a significant vibration above 4200rpm. They immediately reduced power to 4000rpm and advised tower they were returning for an immediate landing. The aircraft had sufficient power available to conduct a normal circuit. Changing fuel tanks and switching electric fuel pump on made nil difference. They made an uneventful full stop landing and engineers checked aircraft and endorsed to fly again.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the check flight prior to the students’ solo flight the instructor went over Engine Failure after Take Off (EFATO), short field landings, go arounds and glide approaches. All circuits were demonstrated by the student to a very high standard. The wind was pretty much directly down the runway at no more than 5 knots. The student had completed about 3 to 4 circuits with no issues but on the final landing, bounced the aircraft. Instead of applying full power and going-around, the student closed the throttle and tried to control the aircraft. The aircraft bounced 3 times and on the fourth bounce struck the propeller, breaking the nose wheel landing gear. The student will be retrained on correct techniques for managing bounces, go arounds, and effective decision making.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft (a Foxbat A22LS) was inbound to Bankstown via Prospect (inbound reporting point), flying a heading to establish on a downwind leg. A short time after an IFR twin reported they were also inbound via Prospect. The twin was given instructions to pass Foxbat A22LS on the left. They were unable to do so and passed the Foxbat on the right. During this time the pilot of the Foxbat missed a traffic alert and received a safety alert from ATC to inform them of the twins position. When they did respond, they acknowledged that they had traffic in sight, and the twin overtook them on the right. Subsequent interview with the instructor revealed they were distracted at the time, interacting with the student. As a result of this event the school has enacted a sterile cockpit process when inbound and outbound at high traffic areas such as Prospect, in addition to during circuit training lessons.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight instruction in a busy training area associated with a Class D airport, the aircraft inadvertently entered the Camden CTR. The instructor noted they were focussed on ensuring the student selected an appropriate landing location for PFL and didn't realise how close to Camden airspace they were. Corrective actions introduced include use of ipad or similar to confirm airspace boundaries and remaining situationally aware of the aircraft location.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student was completing solo touch and go’s. As they lined up on their final approach they decided to go around as they weren’t comfortable with their set up. As they lined up for the 2nd approach, they were happy with their configuration and line up and proceed to land with 2 sets of flaps extended. The student felt their flare was smooth and they were lined up in the middle of the runway, the wheels touched down smoothly and then the aircraft glided momentarily before the nose lifted then suddenly dropped. This happened twice before it came to a stop. The CFI will conduct additional training in go-arounds and decision making with the student.
