OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot clipped the corner of Sunshine Coast Airspace at A025 when the lower limit was A015. They knew the airspace was there and briefed on it before departure. When airborne, they became distracted with the student and failed to descend. Subsequently, the instructor instituted a thorough review process with any students prior to navigation exercise to ensure CTA avoidance actions were clearly understood and briefed. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A VH registered aircraft has reported that whilst they were conducting operating circuits at YBNA a Jodel taxiied for the runway. Runway in use was runway 24. The Jodel's radio was unreadable. As the VH aircraft turned base they called turning base runway 24 for an inspection run. They noted the Jodel sitting at holding point A.
As they turned final the Jodel now called entering runway 06. Ballina Information called Jodel to notify them the VH aircraft were using runway 24 and had just turned final. The Jodel then taxiied off the end of runway 06 onto the grass in the overshoot. Significant wheel tracks were observed as the VH aircraft completed the low pass and continued onto upwind.
As the VH aircraft turned crosswind they noted the Jodel commence take-off roll on runway 06 - an unreadable radio transmission was heard. During the take-off roll the aircraft appeared to almost experience two runway excursions. As the VH aircraft turned downwind they made a radio call advising they were downwind runway 24 for another inspection pass. No call from Jodel received but aircraft noted now on late upwind and commencing turn onto downwind - head to head. Car 1 made call to notify aircraft of collision risk - traffic departed on opposite runway now departing head to head with circuit traffic. The VH aircraft widened out and passed port to port with departing aircraft at <1/3nm at approximately 200' separation.
The pilot of the Jodel has been required to complete remedial training for operations at non-controlled airports, radio procedures and circuit procedures. |

The engine was flown the previous day with no issues. On doing the daily inspection the following day, loss of compression was detected on one cylinder. The Low compression was identified from #1 cylinder and it was stripped down revealing the exhaust valve seat had separated from the cylinder head. The engine is running on PULP 98. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Partial engine failure during a circuit lesson on upwind of RWY 29L at 500ft. Whilst conducting circuits the RPM dropped from full power (5600rpm) to 2600rpm for 30 seconds, with the aircraft shaking. The PIC turned back, declared a mayday and landed in the opposite direction on RWY 11C. The oil Ts and Ps were in the green. Aircraft had full fuel with both tanks open. The Aircraft landed safely without incident and the plane was grounded. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting solo flight training, the pilot inadvertently infringed upon the airspace of R234. The incident occurred at a height of 2900ft, wherein R234 was 'ACTIVE' and designated LL 1500.
The FTS responsbile for hiring aircraft to pilots in this location has implemented fitment of FLARMs to aircraft along with a process of retraining and review of any airspace violations with pilots. |

Witness noticed the cover was burning and ran out to the aircraft to remove it. The cover was removed from the aircraft and the fire was extinguished. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Pilot was seen to use runway 05 practicing circuits, and it became obvious that they were having difficulties, landing long, in light winds, late go arounds.
After about 5 circuits, and noting that the pilot was having some difficulty in putting the aircraft down, a Senior Instructor radioed the pilot and "talked them down" by reminding them to reduce power fully to land. After doing this, the pilot landed normally and taxied back to the hangar.
The Senior Instructor spoke to the pilot after the incident and then advised the CFI about the incident.
The CFI will review the events and corrective actions with the pilot. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On approaching the runway (23) the pilot of the FK9 saw a tug and glider in the distance and believed they were on the grass to the side of the runway. The pilot knew the gliders were operating from about the mid length and in 23 direction. The FK9 pilot called entering and the glider replied that they were to take off in 3 minutes which gave the FK9 time to take off before they rolled. The FK9 is a taildragger so the pilot did not see them over the nose until they were airborne when they judged the safest course was to continue the take-off. The FK9 pilot was surprised to see the glider was on the runway and not the grass.
Lack of effective communications between the glider launch operation and the pilot of the FK9 was determined to be a causal factor. |

Whilst flying at 4500 ft, the pilot sensed a smell of burning wire and shortly after cabin started to fill with smoke. The landing was satisfactory with no injuries or damage to plane. No fire occurred in cabin. As soon as landed all electrical systems and engine were turned off. After the engine stopped the battery was disconnected. The smoke dissipated. The pilot called the airport coordinator at Albany airport and explained what had happened, and they alerted responsive parties including police. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Upon completion of flight instruction work at the Aerodrome, the instructor boarded their aircraft with the intention of ferrying the aircraft back to their base aerodrome.
A Standard Operating Procedure established for some time was to survey the aerodrome parking area before starting the aircraft to ensure there were no obstructions such as other aircraft or ground vehicles towing gliders etc. The instructor noticed an aircraft had commenced taxiing towards the South Eastern runway so they delayed start up with the intention of not causing themself excessive taxi and waiting time.
The instructor heard no taxi call from this aircraft which proceeded to the holding point of runway 16 and stopped. The aircraft then made a left turn and proceeded back to the parking point in which it had come from. The instructors assessment of this unusual back track may have been the result of a student reassessing their departure protocols.
As the instructors aircraft had been recently flying and the motor was still warm they started up, prepared the aircraft for take-off, taxied to the holding point of runway 16 for an intersection entry to the runway followed by a downwind departure to the north .
The Instructor was informed that there was a Flying Doctor aircraft approaching their departure runway from the South once they became airborne, they did not hear any radio calls from this alleged aircraft nor did they notice the approach of it but upon receipt of this information they immediately made a 90 degree left turn and continued to fly their crosswind and downwind legs uneventfully . |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student and instructor had started a lesson and were holding at the taxiway for a FlyDoc aircraft that they had heard on a 5 mile final for runway 34. While they were holding a Jabiru aircraft taxiied around them on their right hand side and appeared to begin to to backtrack runway 34. At this point, FlyDoc called on the radio for a one mile final runway 34.
This was when the instructor noticed that the speed of the Jabiru was beyond taxi speed, and they were in fact taking off on runway 16 - into opposite the oncoming path of the FlyDoc aircraft.
At this point, the student and instructor had taxied back onto the apron in anticipation of the FlyDoc aircraft needing extra space to vacate the runway, and there was nothing they could do but watch as the Jabiru made an avoidance turn to the left to clear out of the way of the oncoming traffic.
Pilots are reminded of the importance of listening to the radio and establishing any relevant traffic prior to entering a runway. |
