OCCURRENCE DETAILS SUBMITTED TO RAAUS: After landing runway 17 at Tyabb without incident the aircraft was taxied off the runway and proceeded to taxi to the parking area. After approx. 100m of taxiing the aircraft came to a sudden stop. Following the aircraft being pulled to a hangar with a dolly under the stuck wheel and the spat being removed it was found that the rim had ‘sheared’ into two. The reporter has contacted the manufacturer who stated this was a common occurrence in this type of wheel over 25 years old (these wheels are 26 years old) so it is likely due to metal fatigue. It appears this cracking or fatigue was missed at the annual inspections over time.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While undertaking an Area Solo training flight and approx. 5 miles East of the airport, the student inadvertently entered cloud. The student-maintained level flight but did not want to descend below 1600 feet AMSL because they had heard aircraft performing 1500 feet circuits. After being in cloud for about a minute the student made a Mayday call on CTAF advising they were in cloud at 1600 feet. The supervising instructor immediately called the student and advised them to keep the wings level and reduce power to begin a descent. The aircraft broke clear from cloud about 5 seconds later at about 1550 feet and the student was able to report visual and aware of their position. The student has returned for further training including solo training. The instructor has been advised to be more vigilant when send students on T/A Solo activities with low cloud or poor visibility.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On the 02/10/23 PIC Checked the weather in the morning and prior to flight Departed YGAW (Gawler Airport) for Truro Flat and then conducted a touch and go and continued to the East to look at the Murry River, as the winds were starting to pick up the PIC started the return trip back to YGAW. On arrival the pilot conducted two go-arounds due to the gusty conditions on to RWY 31 as the winds were predominantly from the West. On the Final touch down, the aircraft landed to the right of the centre line and once the nose wheel touched a strong gust of wind turned the aircraft to the left causing it to depart the runway, applying maximum breaking the aircraft came to a stop just prior to the taxi way then taxied back to the hangar where they found the right tyre had deflated due to picking up a 3 horned Bindy.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: At 5500ft on cruise there was a slight reduction in power so the pilot turned the fuel pump on and pulled carby heat on. The RPM was reduced to 1200 and remained at that. A station was to the left, so a landing site was selected. The engine stopped at around 1000ft on descent. The landing was successful along a graded fence line and about 2 km from the homestead. Fuel was dripping on to the spat of the nose wheel. After closer inspection fuel was overflowing from the carburettor. A mechanic was notified, and the problem rectified. Most likely the cause was particles caught in the needle valve and flooded the engine. The aircraft proceeded without any problems after the incident. OCCURANCE OUTCOME: Aircraft was repaired and flown back to base. Further investigations into the reasons why the failure occurred continue with the manufacturer.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While conducting circuits, the student was flying an approach to RWY 35 and the approach became unstable. The CFI attempted to apply power for a go around and the student was holding the throttle lever in the closed position and did not realise the CFI was trying to add power. The landing was completed successfully however, the throttle arrangement prevented the CFI from applying go around power. Resolved after interview with the CFI by ensuring a clear briefing about the requirements to relinquish the controls if called for by the CFI.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After a successful landing on the student’s first solo flight whilst braking and preparing to stop and backtrack, the student lost directional control of the aircraft and the starboard main wheel collided with a runway marker damaging the wheel spat.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Following runups, the pilot called entering and backtracking on 31. While backtracking on the runway a buggy towing a glider entered the runway without making any radio calls and cut the aircraft off. OUTCOME: The individual towing the glider has been de-briefed on the incident and received further training to ensure an appropriate level of lookout and listen out is completed before entering or crossing all runways at all times.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot of a Jabiru was flying circuits using runway 05 following an aircraft in front of them. On this particular circuit, the Jabiru was positioned downwind, abeam the 05 threshold in a landing configuration. The pilot in front then announced on final that they would complete a full stop landing. Whilst extending the circuit to provide more room for the landing aircraft in front the Jabiru pilot inadvertently entered Edinburgh Military airspace, before turning base and final. The Pilot has debriefed the occurrence with an instructor and completed additional ground training to ensure the circumstance does not happen again.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Jabiru aircraft was taking off from a private LA in which horses owned by the pilot and passenger were unfenced from the airstrip. The operators had conducted significant flights from the same location with the horses unrestrained on all previous occasions and they were not perceived as a threat. During take off roll, a single horse, who's position was separated from the herd, galloped towards the aircraft at speed, impacting the left wing and pilots door area resulting in the aircraft becoming inverted with significant damage and injuries to the pilot. The horse also sustained significant injuries and was later euthanised by the owners. This accident is a another example of normalisation of deviance, where clearly known threats were dismissed by the pilot based on previous animal behavior at the location. The pilot will amend all future operations to mitigate this risk by restricting access by livestock to the airstrip when operating.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst downwind on with a student, a call was heard from another aircraft (returning from a flight and inbound from the north I heard the inbound call from him and notified our location in the circuit.) notified overhead the field with intention to join " midfield downwind", we notified our position on downwind and requested position of the joining aircraft and to confirm they were number two in the circuit and requested confirmation they had us visual. He said he did not have visual, at which I gave my location downwind turning base, there was no reply, we had just turned base leg and across from our left shot the joining aircraft at which I transmitted "you have just flown past us and you have crossed out path". There was a pause and a response by the pilot of the joining aircraft acknowledging my transmission. We continued on our descent and landed. They landed shortly after. RAAus reminds members that the risk of collision at non-controlled aerodromes is greatly increased if pilots do not confirm visually where other traffic is, or use radio to provide effective separation. Training aircraft with students aboard may fly wider circuits at slower speeds than more experienced private pilots, but regardless, circuit positions and right of way requirements must be maintained regardless of the usual circuit size and pattern flown. Radio must be used to confirm traffic position and if a visual position cannot be confirmed, pilot should consider alternative actions, make additional radio calls and ensure they do not fly inside or conflicting positions to other established traffic.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was going to go for a short fly around the Tooraweenah area and was using the marked taxiway to enter the runway, there was an old drainage pipe underneath the taxiway which was broken and had formed a hole around 30cms wide and around 15cms deep. Taxiway had been freshly moved and hole was not visible and encountered a propeller strike with the ground. Because of the broken pipe the front wheel went into the hole and front wheel just stopped causing the prop strike. Front of plane was lifted off the ground when the propeller hit the ground.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual flight training, while recovering from a practice forced landing, the lesson drifted into restricted airspace (R234). After analyzing OzRunways and FLARM data, it is estimated the incursion was approximately 330 meters and up to 30 seconds in duration. OUTCOME: No reported issues from Gawer Airspace (RAAF). Instructor to use VFR markers to ensure a breach does not happen again in the future.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The flight was a dual training exercise returning to Gawler airfield. On re-joining the circuit, the aircraft veered slightly to the right whilst aiming to join crosswind on runway 31. Whilst doing so, the flight path may have crossed slightly into Restricted Airspace before joining normal circuit pattern. OUTCOME: Instructor and student de-briefed by airspace officer and CFI on the possible breach of controlled airspace.

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The first lesson of the day was circuit training, after initial take-off just before the turn onto crosswind, the student indicated that there was smoke entering the cockpit, the instructor was looking out the right hand window clearing the turn at the time, they immediately noticed the same thing occurring. The instructor took control of the aircraft and entered a left hand turn for a close downwind. They advised the tower they needed to make a full stop and that they had smoke in the cabin. The aircraft landed immediately on runway 14R and taxied off the runway to the apron and the aircraft was shut down, the instructor and immediately exited the aircraft. Airport fire trucks were on the scene immediately to assess the situation with aircraft deemed safe not long after.
