OCCURRENCE DETAILS SUBMITTED TO RAAUS: The Instructor was undertaking a Stalls lesson with a student. Approx. 50 minutes into the flight at a location approx. 15NM north of YGSD and 3500'AGL, after conducting a stall, the student attempted to recover using power but the engine only revved up to 2300 RPM and ran lumpy. The lesson was immediately terminated and returned to YGSD by the shortest route. After engine recovery steps made no impact on the engine performance, removing carb heat and then cycling the throttle made the engine recover power as per normal. The aircraft was landed without further incident and the aircraft was taxied back to the hangar.
Inspection of the engine found that the idle mixture adjustment screw had worked loose to 3 1/2 turns from closed (should be 3/4 of a turn), and the O ring on the needle was worn which would have reduced the friction on the needle allowing it to rotate when the engine was running. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot completed the usual preflight, runup and pre-take-off checks. Taxied to RWY 04 and rolled out. The take-off was normal until they completed their ATO checklist at 300ft. It was then the aircraft lost 500-700RPM and the engine was running a bit rough. The pilot enacted their engine failure procedure and that was when they forgot to pump the electric pump on. They switch both Carby heat and fuel pump on but no improvement, so turned them off.
The pilot levelled out at 500 ft. and pulled the power back to reduce the strain on the engine then turned crosswind and declared a Mayday. The pilot made the decision to land on RWY 14. The pilot followed the normal landing procedure and landed on RWY 14 and broadcasted they were safe on the ground.
DETERMINED OUTCOME REVIEW: A remote investigation was conducted and determined that lack of recency and reduced discipline to pre take off fuel checklists contributed to the partial starvation event. No mechanical faults or issues were identified.
Pilots are reminded of the importance of checklist disciplines- to verbalise, identify and confirm all checklist items and be slow and deliberate in these checks, especially if extenuating factors such as rushing, normalised deviance or distraction are prevalent. |

With multiple aircraft operating in the circuit pattern including multiple callsigns in the same type aircraft C172, the SA (Situational Awareness) of the Jabiru pilot was lost with another training aircraft on final until a broadcast was heard, The Jabiru pilot immediately stopped descending and turned to the right of runway centreline and saw an aircraft pass below to their left maintaining approach to the runway. The Jabiru pilot instigated a go around and broadcast this action. Once established on downwind a broadcast turning base 23 touch and go is heard. The Jabiru pilot saw an aircraft on extended downwind commencing a base turn. They completed a few more touch and go's before landing. Each time observing the 172 aircraft were flying an extended downwind before turning on base. The Jabiru pilot had the opportunity to discuss this incident with an instructor soon afterwards covering issues of situational awareness, multiple callsigns on same aircraft type and circuit patterns. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft took off from YTFA tracked east to the river Murray then tracked south following the Murray at 1000 feet AGL. Just before Wongulla the pilot saw pelicans to left of their flight path. As they flew past them pelicans from above started diving across in front of the aircraft, one of them hitting it and breaking the windscreen. after the shock of the impact the PIC elected to conduct an emergency landing into a field.
RAAus continues to monitor bird strike and wildlife reports at aerodromes to ensure correct ERSA/NOTAM information is in place and to enable reporting to the aerodrome proprietor. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot of a Jabiru chose runway 05 for landing, this may have resulted in a tail wind landing (approx. 13kts). an Instructor on the ground made a broadcast referencing the into wind RWY instead. The pilot acknowledged to land on runway 23, joined relevant circuit and competed landing. No other aircraft where airborne at the airfield at that moment. In the future the PIC will take more care when choosing the preferred RWY. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While demonstrating turns to a student, north-east of Freeling at approx. 3,500 feet, the instructor lost orientation within the training area and proximity to RAAF Edinburgh's restricted area R234. The aircraft wandered in by about one km and conducted the training activity. When the instructor looked back at Freeling, they realised they’d never seen it from that perspective and knew that they had strayed into restricted airspace. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot of a Jabiru called inbound at 10nm and heard a couple of gliders and the tug in the air. When the pilot got to Gawler Dam, they made another inbound call as per the Adelaide Soaring Club By-Law 7, at this time the tug had landed, one glider had landed and another on short final. When the pilot gave the call over Gawler Dam, they stated their intentions to track for the dead side of runway 23 to join midfield crosswind. They got no response over the CTAF. As they turned crosswind, they spotted a glider directly over the aerodrome 500ft above circuit height, spotted the pilot asked what their intentions were and got no response. The pilot tracked behind the glider and when they called that they were clear of them they responded with a “thanks” so the pilot knows they had a functioning radio. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot had just conducted a practiced forced landing and was climbing up to 3500ft, tracking north from Lauke Mill, towards Greenock, in the Gawler Airfield training area. Around 2000 or 2500ft, a yellow and white aircraft appeared suddenly on a converging heading at what appeared to be the same altitude. The pilot turned to the left to avoid a possible collision and the yellow and white aircraft appeared to turn to the right. The next time the pilot caught sight of the aircraft, it was at a lower altitude, tracking south west. The pilot had transmitted their location and intentions on 126.5 prior to conducting the air work. No radio calls were heard from the other aircraft.
OUTCOME: Near miss recorded by RAAus after receiving OMS. Pilot completed acceptable levels of look-out to identify other aircraft in close proximity. Pilot reminded to be extremely vigilant when completing Airwork, especially for aircraft transiting the training area. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On Approach to land on runway 17, Coldstream, winds were variable 08 knots. Good approach, and just on flare to land all headwind stopped and the aircraft landed slightly hard due to the sudden lack of headwind. On touchdown the aircraft veered slightly off to the left of the bitumen runway and onto the grass. The left wheel went into a shallow ditch besides a taxi turn off point. This caused the noise to pitch down and impact the ground, breaking the wooden prop and damage to the from wheel. There was no property damage, and no injuries. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After landing runway 17 at Tyabb without incident the aircraft was taxied off the runway and proceeded to taxi to the parking area. After approx. 100m of taxiing the aircraft came to a sudden stop. Following the aircraft being pulled to a hangar with a dolly under the stuck wheel and the spat being removed it was found that the rim had ‘sheared’ into two.
The reporter has contacted the manufacturer who stated this was a common occurrence in this type of wheel over 25 years old (these wheels are 26 years old) so it is likely due to metal fatigue. It appears this cracking or fatigue was missed at the annual inspections over time. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While undertaking an Area Solo training flight and approx. 5 miles East of the airport, the student inadvertently entered cloud. The student-maintained level flight but did not want to descend below 1600 feet AMSL because they had heard aircraft performing 1500 feet circuits.
After being in cloud for about a minute the student made a Mayday call on CTAF advising they were in cloud at 1600 feet. The supervising instructor immediately called the student and advised them to keep the wings level and reduce power to begin a descent. The aircraft broke clear from cloud about 5 seconds later at about 1550 feet and the student was able to report visual and aware of their position.
The student has returned for further training including solo training. The instructor has been advised to be more vigilant when send students on T/A Solo activities with low cloud or poor visibility. |
