A Sling aircraft was manoeuvred out of the hanger and onto the grass. There was not enough room left for aircraft to taxi on the taxi line. Upon start up and beginning of taxi, a Jabiru wing lightly hit the rudder of the Sling aircraft that was parked off the TWY. The rudder on the Sling moved to let the wing tip to pass without any damage.
OUTCOME: The pilot involved had moved the Sling in order to access the Jabiru for flight Operations and subsequent to this event the school has implemented revised starting and surface movement clearance requirements for all operations. |

During a practice forced landing the student flared high. As the aircraft was slowing down the student pushed the control column full forward and the aircraft was too low to recover from the ensuing high descent rate. The nose wheel and prop struck the ground.
OUTCOME: The local CFI has conducted any required remedial training with the student. No further action required by RAAus. |

5 touch and go landings had been completed without incident whilst conducting circuits. They were good landings and the aircraft was operating normally. On the 6th touch and go the rudder failed during a touch and go take-off. The pilot elected to continue with the take off as they believed that full power torque rolling effect and slight cross wind was not conducive to a safe take-off abort. Once off the ground it was very clear that no rudder control was available. The aircraft continued to crab as it climbed and gained height to 2,500ft. Aileron was used to keep heading which felt uncomfortable and unbalanced. Further testing on controls indicated that the rudder was not responding and that the nose wheel spat had no effect on turning. The pilot commenced a landing with a wide circuit, soft control inputs and a long final approach with a short field landing. The landing was uneventful. The aircraft touched down with minimal airspeed. On touch down the nose wheel gave steering authority and the pilot was able to keep it on the RWY.
Outcome: This issue has been referred to CASA Airworthiness and Engineering Branch. |

Runway incursion: The aircraft landed without a clearance.
OUTCOME: The degradation of task management occurred. In this case active listening and compliance with airways clearance whilst performing instructional functions. The division of attention between management of the aircraft, communications and other non-technical tasks and the delivery of instructional must be constantly assessed and managed by instructors and situational awareness must be maintained at all times, particularly whilst operating in controlled environments. |

The pilot requested a transit clearance through Sunshine Tower to transit D airspace as a result of adverse weather. The pilot misread the VTC and commenced descent to YCDR at the 40 DME Brisbane boundary instead of the 8 DME Sunshine Coast boundary, therefore the descent commenced approx. 2DME prior to the 8 DME Sunshine Coast boundary.
DETERMINED OUTCOME: Pilots are reminded that if they are not appropriately qualified to access controlled airspace, if an in-flight emergency is experienced or they encounter weather which is not forecast, they should declare an emergency with ATC on area frequency using the phrase PAN, PAN or MAYDAY as appropriate. Assistance will be provided which may result in them being granted access to CTA. |

The aircraft made a precautionary landing in a paddock after experiencing abnormal engine indications. After touching down in the paddock the nose wheel dug into a sandy culvert and the front of the plane dug down into the soft ground where it slid along for about 15 metres before eventually flipping over onto the roof.
Subsequent investigation revealed this amateur built aircraft had used parts from Cam-It making it non-compliant to Jabiru requirements. Fuel exhaustion was considered a possible factor. |

Whilst conducting circuits the pilot noted the oil pressure gauge was sitting close to zero and decided land to ascertain the cause. During landing the aircraft bounced twice, on the second bounce the front end collapsed resulting the prop to strike the ground. The pilot will complete a flight review once the aircraft repairs are complete |

The aircraft was destroyed in a hangar fire and another aircraft that was parked next to this aircraft was heat damaged and written off. There was also smoke damage to the hangar.
Investigation showed fire started in the engine bay. It is suspected that the voltage regulator failed. All Jabiru aircraft owned by the Club have now been fitted with solenoids, to isolate the battery from the electrical system. |

Landing on a grass runway, the hydraulic line to the aircraft brakes was severed, resulting in "no brakes". The aircraft rolled through the end of the runway, across the adjacent bitumen runway and through an earth gutter on the other side of the bitumen. In crossing the gutter, the low fibreglass fairing around the front wheel split.
Determined outcome: Possible cause was a small stick or stone being thrown up on landing and impacting the hydraulic line. |

A loud clattering noise in headset during radio call on final approach. The engine was discovered to be making the noise. The pilot carried out successful landing with engine idling and as the aircraft rolled across the RWY intersection to a TXY the propeller suddenly stopped in the 11 o'clock position. The crew disembarked and found the engine seized solid so pulled aircraft down TXY to the hangar for examination.
Later investigation revealed the circlip holding the gudgeon pin was no longer in place. |

A Nav training flight was being conducted between Bendigo, Maryborough, Ballarat and back to Bendigo. The aircraft was running well with no issues evident. Ballarat was a full stop landing to have a short break. On restarting the engine an unusual noise was evident and the decision was made not to continue.Further investigation revealed a number of the flywheel bolts had broken. |

A student was conducting their second solo in a Jabiru, a Tiger moth cut in front of them when they were on base.
The student thought that there was enough room to allow the Tiger moth to land and get off the RWY, but misjudged. This resulted in the student landing the tiger moth was still on the RWY.
The student also miscalculated the distance between the two aircraft and couldn't stop in time, requiring them to veer off the RWY to get around the Tiger month. There was no damage to either aircraft.
Determined Outcome: The CFI has debriefed the student involved and RAAus is satisfied that no further action is required. This report has been forwarded to CASA Sport. |

An aircraft was conducting circuits and noted another aircraft taxiing onto the active runway, however did not hear any calls. The aircraft conducted appropriate avoidance actions and as a result, no conflict occurred. Pilots are reminded of the requirement to check the radio is on the correct frequency and receiving prior to flight.
To each pilot: Pilots are reminded of the requirement to ensure the aircraft radio is transmitting on the correct frequency and confirm the radio is receiving as part of the first flight checks. Vigilance in the circuit using "see and avoid" and appropriate avoidance actions resulted in no serious conflict. "see and avoid" can be augmented to "alerted see and avoid" if radios are set to the correct frequency, checked for receiving quality and all appropriate calls are made. |
