OCCURRENCE DETAILS SUBMITTED TO RAAUS: During take-off full throttle was applied and the aircraft accelerated until 50-60 knots was reached before rotating. The pilot experienced the nose drop at this speed when they would normally expect lift. The pilot then let the aircraft try to gain further speed. The second pull back on the yoke initiated lift however it was sluggish and felt like they were only a few feet off the ground. At this point the pilot thought the lift wasn't adequate and pulled off throttle and applied the brakes. Trying to maintain a straight course. The aircraft veered slightly left and at the end of the strip nosed into a ditch and embankment before the landing on its roof. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft lost directional control on take-off and ran off the RWY and made contact with a concrete drain. The impact cracked the right hand undercarriage and snapped the nose wheel off when it came to a stop damaging the front of the aircraft.An investigation with the pilot was conducted and the primary cause related to foot misplacement on the pedals where one foot slipped during take off roll. The owner pilot is discussing improvements with grip on the metal surface and will ensure positive control and traction of the rudder bars in future. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: After completing an hour and a half flight, the aircraft had just landed when it started veering to the left. The pilot applied hard right rudder but it continued to run off the RWY. The pilot applied some power and lifted the left wing and wheel to clear a landing strip light, and after clearing this obstacle, continued to apply hard right rudder, however the plane continued to turn left. Brakes were applied to no avail, the aircraft ran off the strip and through a barb wire fence, coming to a standstill near some trees. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On final approach the pilot closed the throttle but pulled back too hard and over rode the idle stop. As a result the engine stopped. This occurred close to the ground. The PIC tried to restart the engine and in doing so was distracted from concentrating on the landing.
This resulted in the aircraft entering a PIO situation damaging the nose gear and propeller. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot of a Jabiru was on mid downwind for RWY 14 and observed two microlights enter and roll on runway 32. The Jabiru pilot immediately commenced a right hand orbit to avoid conflict with the other aircraft on 32. No radio calls were noted from the microlights.
Pilots are reminded of the importance of a good lookout when operating in the circuit and encouraged to reference CAAP 166-1(4) for operational guidance at non-controlled airports. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The student pilot was under supervision and requested to conduct 2 solo circuits. During the initial line up the student followed a preceding aircraft which had chosen to roll adjacent the main designated centerline of this grass runway. During the subsequent landing on the same surface the student was unable to control the bounce from the landing resulting in a pilot induced oscillation (PIO) which resulted in nose wheel damage that was identified post flight. The student had conducted recovery from these scenarios prior to solo but the school has refocused attention on this area to mitigate future events of this type. Further education and awareness of the varying runway surface at Lilydale is to be undertaken by the school CFI. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During final approach for a full stop landing the pilot received a request via radio from the Gliding Club tug pilot requesting the landing point be shifted so the landing would not interfere with gliders parked on side of the RWY. The pilot immediately applied power, adjusted attitude and identified a new aim point as requested. The tug pilot broadcast a further request for the pilot to extend their landing later on the runway. This transmission was received at approx. 150' and the pilot again took Immediate action. The pilot then set up for landing, however this point of landing was now significantly further down the RWY than the pilot was used to and was adjacent a large clump of trees on the Eastern side of strip. Due to the crosswind from the East at approx. 50ft the aircraft encountered a serious downdraft caused by the trees. Power was immediately applied but the aircraft impacted the RWY from a height of approx. 20-25ft. The nose wheel collapsed and prop struck ground. Brakes and ailerons were applied to maintain the aircraft upright and stop it.
The pilot advised he should have conducted a go-around.
The tow pilot should not have requested the pilot to change their landing point and this has been referred to the Gliding Federation of Australia for further followup.
RAAus has also investigated a possible amendment to the ERSA entry for the airport in question to alert pilots to potential wind shear issues. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had been assessed as in airworthy condition. The front nose wheel was inspected prior to start up and nothing unusual was noticed. The aircraft was flown previously by a student, doing touch and go’s, and hadn’t been through any hard landings. After taxiing to southern end of the field and after TEMPFISCH checks/Run Up checks, the plane was taxied towards the beginning of the runway where the nose oleo leg failed without any prior symptoms. . Estimated speed at impact was a walking pace so no injuries were sustained. The propeller had hit the ground and had come to a stop in a diagonal position no longer touching the ground. The aircraft systems were shut off including the fuel cut off valve and the aircraft was evacuated.
It was found that the lower nose bungee attach fitting bolt was missing, either failed and fell out or the nut had detached and the bolt worked its way out. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The student had successfully completed one circuit and landing with one stage of flap, they had done so without any need for the instructor to come on the controls. The second landing (a touch and go) was flapless, the student had landed a little long. The grass RWY surface was a little rough and as the aircraft exceeded 60 knots, the student hadn’t rotated. Initially the instructor thought the student may have delayed rotation due to it being a flapless take-off and the instructor was concerned that the nose leg was being pounded. The instructor reached for the flap control and asked if the student required flap, the student responded by closing the throttle and saying that there was no aileron control. The instructor checked the control yolk which came away in their hand. The instructor aborted the take-off and rolled through the remaining RWY and taxied to the hangar.
