OCCURRENCE DETAILS SUBMITTED TO RAAUS: On taxi to the runway, the brakes failed approaching the holding point. The pilot reduced the power to help bring the aircraft to a stop before the holding point. This was not possible and the pilot diverted off the taxiway into grass to avoid breaching the active runway. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot flew from Mudgee to Singleton at 7500ft then cruise decent to Cessnock 1200ft. Shortly after joining the circuit, downwind runway 35, the aircraft was abeam the threshold the engine ran rough and began to lose power. The aircraft was following another who was 1/2 a mile ahead so the pilot called turning base 35 for a full stop and flew an abbreviated circuit. All went well and the pilot exited the runway with communication. Review identified likely cause due to carburettor icing. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was making a return flight from Townsville to Atherton. After refueling at Atherton Airport the aircraft took off. When it reached an altitude of approx. 3,800 - 4,000 feet the engine started to shudder and vibrate for approx. 2-5 seconds before locking up and stopping completely. The pilot made one mayday call on 126.7 and then several more on 132.9 on the CTAF. The pilot completed a successful emergency landing into a paddock causing minor damage to the aircraft and no injuries to the pilot.
OUTCOME: Pilot made a successful landing in a field after catastrophic engine failure which caused minor damage to airframe, but no pilot injuries. The engine was later dismantled for investigation. During the investigation the engineer found that a valve stem had failed causing significant internal engine damage and the engine to seize after severe vibration. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A Jabiru J-230 entered and backtracked with the intention of taking off. The Jabiru entered the runway with a C172 and a PA44 already established on the mid- and early final approach phases respectively for the runway. Both aircraft completed a missed approach and go-around to prevent a further breakdown in landing separation and in response to the runway incursion.
The pilot of the Jabiru was interviewed and could not recall any separation issues on take off. Initially the date provided for the incident was misreported in error. A review of rules of the air and other regulatory requirements was undertaken by RAAus Operations with the pilot by telephone and the occurrence recorded for future monitoring if required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft departed Class D airspace at Launceston tracking on a northerly heading for Georgetown (YGTO). Listening on Melbourne Centre and Georgetown (YGTO) CTAF on standby frequency. A white aircraft flew within metres of the left hand side of the training aircraft. The aircraft immediately took evasive action to avoid that aircraft and broadcast on Melbourne Centre as well as Georggretown (YGTO) CTAF to try and make contact with the other aircraft with no response. The aircraft maintained visual separation with other aircraft until they were safely clear of it.
Pilots are reminded of the importance of "See and Avoid" and never assume no other aircraft are present, simply because no radio calls have been received. Refer to CAAP 166-01 and CAAP 166-02 for more guidance. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Poor fuel management resulted in fuel exhaustion and a forced landing into a paddock. the aircraft suffered undercarriage damage and has been repaired as required. No serious injuries occurred and the pilot has been debriefed regarding decision making and fuel monitoring procedures. The aircraft has a fuel tank inside the cockpit which can be quite opaque if not well lit. This was also noted as a possible factor in the accident occurring.
The aircraft owner has also implemented a fuel log to ensure pilots are aware of minimum fuel requirements prior to flight. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting a dual flight training exercise, after being airborne for approx. 35-40 minutes conducting air exercise without incident (all engine temps and pressures good). On recovery from a demo of stalling in a descending turn, the aircraft, on full power (3000 RPM) at about 5400ft, began to miss and run rough, throttle closed but engine seized within 5 seconds. A Mayday was issued to Melbourne Centre (120.75) and acknowledged. The Instructor selected suitable landing area (sheep paddock) and carried out a forced landing without further incident, damage or injury. They then called 000 to report their position and waited for emergency services.
The aircraft was recovered and taken to Jindabyne Airfield where an L2 Maintainer inspected engine and reported major failure of piston in No.1 cylinder. The engine is to be removed and returned to the manufacturer for further diagnosis. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The student was conducting their first solo after completing five circuits to a safe standard. Additional traffic had just landed, slowing down to exit the runway.
The student lined up and started rolling without realising the traffic still had not exited the runway via the taxiway. The traffic subsequently exited via taxiway safely but the student was startled during the initial take-off roll, reduced the power and came off the side of the runway with no damage.
The student will undertake further assessment flights with an Instructor prior to solo. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Flying from Flinders Island to Tasmania, via Cape Barren Island when the engine failed over water approximately five nautical miles South of Cape Barren Island. The airstrip on Cape Barren Island is on the Western side and the aircraft was positioned to the South. The pilot glided toward the airstrip, not expecting to get there, however, a dirt road ran from the south side of the island toward the airport. The pilot elected to land on the dirt road. The road was not wide enough for the wingspan and the right-hand wing impacted with the foliage spinning the aircraft 180 degrees. No injuries were incurred. On inspection of the engine, it was found that the exhaust valve on the number three cylinder had failed. |
