OCCURRENCE DETAILS SUBMITTED TO RAAUS: 0.5 hours into the flight the pilot noticed an abnormal oil pressure indication, showing off the scale HIGH pressure. Approximately 2min later the oil temperature started to drop, followed shortly after by an RPM drop. Power was added to recover the RPM drop and the engine vibrated, tried to stall then surged. The engine continued to run at the new power setting. The pilot conducted an uneventful landing. Review identified engine damage from number 1 cylinder. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was being used in a Straight and Level lesson, and was in straight and level flight at cruise power when the engine suffered a partial loss of power. The pilot made a precautionary landing. The engine continued to operate throughout the flight until shutdown, albeit at reduced power. The maintainer identified piston damage in cylinder number 1. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Engine lost power in flight and remained at an idle, switched on fuel pump, no response. The pilot gave a mayday call, set the aircraft up at 60 knots best glide speed and retained that while trying to reach the clear spot ahead. The left wing impacted a tree which slowed the impact. The front wheel broke off, the nose impacted the ground and the aircraft became inverted. The cause of the failure was unable to be identified, however it is possible a small piece of corrosion came adrift from the bottom of the carburettor bowl and became lodged in the main jet. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting circuits, the aircraft was on crosswind on climb out for the second circuit when the engine dropped power and was running rough and vibrating. The pilot immediately turned the aircraft back towards the runway, made an emergency call and pulled the power back to idle, some 5 seconds later the engine stopped and the pilot glided it back to the runway for an uneventful landing. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A bird flew into the propeller on take-off. The aircraft was 1/3 of way down runway so the pilot aborted the take off and inspected for damage.
RAAus continues to gather statistical data on birds and wildlife that interfere with aviation activities around the country. It is particularly important that these incidents are reported so that RAAus can identify problematic areas and notify the relevant parties. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: DEFECT: The aircraft experienced a partial loss of electrical power inbound to an airport. On investigation stator was partially damaged. The pilot contacted the manufacturer and was advised that it was safe to fly, flew back to Wyreema Qld on 10th July and replaced the stator and the battery. All electrical systems have been checked and are determined to be satisfactory, however the entire aircraft will be assessed at it's next 100 hourly by the manufacturer.
The aircraft was unable to transmit using the radio as a result of the pilot electing to reduce the electrical load in flight. The transponder was operating satisfactorily and the pilot maintained a listening watch on a second radio fitted to use as a VOR receiver and therefore could not transmit. The pilot squawked 7600 on the transponder to alert other pilots and ATC of the radio issue but was able to ensure no conflicts by maintaining the listening watch. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During dual navex flight training a small loss of power was noticed, 50-100rpm or so. The instructor suspected maybe carby ice, however carby heat made no difference so removed the CH. About 30s-1 min later the aircraft experienced a sudden onset of severe vibration and loss of power - it felt like a cylinder had stopped, the instructor took over at this point. Reducing throttle to below 2000rpm got rid of the vibration a bit. Adding it back and the engine immediately started to vibrate badly and clearly not run on all 4. The aircraft had insufficient power to maintain altitude. The instructor chose a field to make an emergency landing whilst trying carby heat, fuel pump and mags to no avail. The aircraft landed uneventfully. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Touchdown on RW29R was normal, but during the rollout, the aircraft tended to veer to the left.
When moderate braking was applied, and Aircraft tracking corrected, the Brake lever went completely slack and Braking action was lost completely.
After exiting the runway, there was a tendency to track to the left and i felt that the left brake was dragging however i was able to taxi back to the hangar under power without assistance. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: While conducting lesson on circuits, the instructor attempted to demonstrate engine failure after take-off and return to the runway. Power was reduced to idle, adopted airspeed 65/70 knots and lowered the nose to maintain airspeed. The pilot attempted to round out, however as they rounded out struck the ground resulting in a very heavy landing.
The Instructor was requested to complete a review with the CFI, however has moved from the location and is no longer instructing. Prior to return to instructing a review process will be required. Members are reminded of the risks involved in attempting to turn back to the runway in an emergency and should elect to land straight ahead. |
