|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: During the landing phase with a student and instructor aboard, the aircraft veered left off the runway, potentially due to a crosswind, before coming to a stop after hitting a fence post. The student was not successful in applying sufficient rudder to prevent the aircraft leaving the runway and will undergo retraining. RAAus reminds members to maintain currency in all flight maneuvers and to set personal minimums before going flying. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Strong engine vibrations were detected during cruise. The pilot requested an emergency landing at Archerfield Airport and conducted a steep decent with full flap and normal landing.
A review conducted by a LAME found no problem and could not reproduce the vibrations. The pilot then carried out a circuit and found the vibration to still be occurring. This was subsequently reviewed by the factory who identified a broken outer inlet spring on cylinder number 3. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: On take-off kangaroos ran across the field, power was reduced, one Kangaroo suddenly changed direction and ran back in front of the aircraft's path resulting in it being hit
Inspection of the aircraft and propeller indicated no damage. subsequent test flight indicated no damage.
Kangaroos are listed in ERSA as a Hazard |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The hinge pin had frozen in the hinge tabs.
The pilot of a Jabiru SP470 was conducting a full free and movement when the control stuck, on checking, the pilot found the right hand aileron hanging down, and the complete hinge had broken off.
CASA have issued an airworthiness notice to address this issue as this is a commonly identified defect. Members are reminded of the importance in reviewing and carrying out airworthiness notices applicable to their aircraft. |

|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was conducting circuits on 17 and on the third circuit the engine totally failed just after turning crosswind. The pilot was able to land the aircraft uneventfully. On inspection, it was found that the piston in number 5 cylinder had disintegrated. |

|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst practising forced landing simulations overhead Gingin North ALA, the engine was run at idle power for a considerable time, with short increases of power to 1800 RPM to clear the engine. When the pilot went to increase power to abort the simulation, the engine ran rough. The pilot elected to land on the runway at Gingin North ALA.
The engine was inspected and it was determined that the spark plug on one cylinder has fouled. The spark plugs were replaced, the engine tested and the aircraft was flown back to its home airport of Muchea Greenside (YGSD), a distance of 16NM. |

|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: The pilot was returning to the airport after a navigation flight and gave a 10 mile inbound call. The AWIS was indicating 6 knots from 350 degrees variable from 330 to 010. The pilot gave a joining call on crosswind midfield for runway 35 and then a base call for 35 for a full stop. The pilot then heard a VH registered aircraft (Mooney) make an entering and backtracking call for runway 17. The RAAus pilot then made a turning final on 35 call and asked the VH aircraft on the runway their intentions. No response to the RAAus pilot was provided and the next call was "Mooney lining up on 17 ". the RAAus pilot gave a call stating they were on short final and commencing a go around. The Mooney pilot responded requesting where the RAAus pilot was, to which the RAAus pilot responded “above you” and passed overhead the Mooney at about 500ft. The Mooney then proceeded to take off and the RAAus pilot continued their circuit and landed safely.
About an hour later the Mooney returned to Bendigo. Both pilots discussed the incident, the Mooney pilot was apologetic for the error.
RAAus reminds all pilots to refer to CAAP 166-1(4) for recommended "alerted see and avoid" radio calls and procedures and encourages pilots to interact as these pilots have in the event of a conflict. Proximity events are a hazard at non-controlled airports and a high degree of situational awareness is required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: During solo training flight the aircraft had an electrical system fail.
Jabiru Service Letter JSL021-2 states as below for in-flight over-voltage situation management.
3.3 Inflight overvoltage situation
If there is a malfunction of the charging system that causes an overvoltage situation there are steps that can be taken to minimise risks of damage to instruments and charging system.
An overvoltage situation does not affect continued operation or the performance of the engine.
1. Reduce rpm until voltage reduces to within limits. Maintain for 5 minutes or as long as possible before increasing power again.
(The alternator is a permanent magnet device and its absolute voltage output is proportional to rpm.)
2. Proceed to nearest safe landing area. Manage voltage using rpm as much as possible. |

|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had flown a local flight and conducted one touch and go on runway 27 at West Sale. The wind on the East Sale ATIS was indicating 270 at 8kts and the wind at West Sale was similar but potentially up to 10kts. After one circuit and during the second touch and go, at approx. 30ft with full flap the airspeed rapidly decreased to approx. 50kts and the aircraft nosed forward and impacted the runway on the nose wheel causing the nose wheel to collapse and the tip of the prop to strike the runway. The aircraft was shut down and pushed from the runway. The ARO was contacted and immediately conducted a runway inspection which revealed a small amount of debris from the composite prop. The pilot and passenger were uninjured. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was taxiing to take off and was back tracking RWY 12. One emu was on the port gable markers and two emus were on the starboard gable markers, all well clear. The single emu attacked aircraft without warning (probably protecting other two emus) The pilot turned the aircraft away from the attacking emu but it ran into the rear of the propellor and was instantly killed. The engine stopped/stalled and the aircraft was towed back to hangar for inspection. |

|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: While an A320 was approaching runway 06 to land at Ballina Byron Gateway Airport, a Jabiru J230-D was tracking south toward Evans Head Airport, NSW. About 22 km south-west of Ballina Byron Gateway Airport, the two aircraft's flight paths intersected, with a vertical separation of approximately 600'. The A320 traffic collision avoidance system (TCAS) traffic advisory provided an alert to the A320 crew, however this has been determined to be a separate issue.
RAAus members are reminded of the importance of ensuring transponders are calibrated and operating with Alt when in flight and encouraged to review the CASA Be Seen, Be Heard, Be Safe guidance material, in addition to CAAP 166 when operating in an around busy non-controlled airports like Ballina. This occurrence was investigated by the ATSB and further information is available from their website: https://www.atsb.gov.au/publications/investigation_reports/2020/aair/ao-2020-062/ |

|
OCCURRENCE DETAILS SUBMITTED TO RAAUS: At the conclusion of a dual circuit training, the tower controller requested a telephone conversation. During the conversation, the instructor was told that they had conducted one of the circuits in the wrong direction. The non compliance with the airways clearance has been addressed satisfactorily by the CFI and Safety controller for the school. Attention to changes in ATC circuit direction clearances and noting them in flight can assist in mitigating the "expectation bias" than can occur when operating in CTA. |
