OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft had turned left off runway 31 at Gawler after landing. Grass left is used for glider aircraft landings. As the aircraft crossed the grass and taxied towards a taxiway, the nosewheel fell into a hole in the ground. The engine stopped immediately as the propellor made contact with the soft earth. The hole in the ground was grass covered and just slightly larger than the nosewheel and not evident to the view. |

The engine was flown the previous day with no issues. On doing the daily inspection the following day, loss of compression was detected on one cylinder. The Low compression was identified from #1 cylinder and it was stripped down revealing the exhaust valve seat had separated from the cylinder head. The engine is running on PULP 98. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A Senior instructor had a student carry out a simulated engine failure after take-off into a suitable paddock, in so doing the aircraft breached Edinburgh airspace.
The FTS has now fitted FLARMs to their aircraft, to review pilot flights and conduct retraining regarding airspace proximity as required. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting solo flight training, the pilot inadvertently infringed upon the airspace of R234. The incident occurred at a height of 2900ft, wherein R234 was 'ACTIVE' and designated LL 1500.
The FTS responsbile for hiring aircraft to pilots in this location has implemented fitment of FLARMs to aircraft along with a process of retraining and review of any airspace violations with pilots. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: The aircraft was parked, shut down, tied down and covers were placed over the aircraft. A solar panel was connected to the battery in order to keep the battery charged.
Witness noticed the cover was burning and ran out to the aircraft to remove it. The cover was removed from the aircraft and the fire was extinguished. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Pilot was seen to use runway 05 practicing circuits, and it became obvious that they were having difficulties, landing long, in light winds, late go arounds.
After about 5 circuits, and noting that the pilot was having some difficulty in putting the aircraft down, a Senior Instructor radioed the pilot and "talked them down" by reminding them to reduce power fully to land. After doing this, the pilot landed normally and taxied back to the hangar.
The Senior Instructor spoke to the pilot after the incident and then advised the CFI about the incident.
The CFI will review the events and corrective actions with the pilot. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Upon completion of flight instruction work at the Aerodrome, the instructor boarded their aircraft with the intention of ferrying the aircraft back to their base aerodrome.
A Standard Operating Procedure established for some time was to survey the aerodrome parking area before starting the aircraft to ensure there were no obstructions such as other aircraft or ground vehicles towing gliders etc. The instructor noticed an aircraft had commenced taxiing towards the South Eastern runway so they delayed start up with the intention of not causing themself excessive taxi and waiting time.
The instructor heard no taxi call from this aircraft which proceeded to the holding point of runway 16 and stopped. The aircraft then made a left turn and proceeded back to the parking point in which it had come from. The instructors assessment of this unusual back track may have been the result of a student reassessing their departure protocols.
As the instructors aircraft had been recently flying and the motor was still warm they started up, prepared the aircraft for take-off, taxied to the holding point of runway 16 for an intersection entry to the runway followed by a downwind departure to the north .
The Instructor was informed that there was a Flying Doctor aircraft approaching their departure runway from the South once they became airborne, they did not hear any radio calls from this alleged aircraft nor did they notice the approach of it but upon receipt of this information they immediately made a 90 degree left turn and continued to fly their crosswind and downwind legs uneventfully . |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: A student and instructor had started a lesson and were holding at the taxiway for a FlyDoc aircraft that they had heard on a 5 mile final for runway 34. While they were holding a Jabiru aircraft taxiied around them on their right hand side and appeared to begin to to backtrack runway 34. At this point, FlyDoc called on the radio for a one mile final runway 34.
This was when the instructor noticed that the speed of the Jabiru was beyond taxi speed, and they were in fact taking off on runway 16 - into opposite the oncoming path of the FlyDoc aircraft.
At this point, the student and instructor had taxied back onto the apron in anticipation of the FlyDoc aircraft needing extra space to vacate the runway, and there was nothing they could do but watch as the Jabiru made an avoidance turn to the left to clear out of the way of the oncoming traffic.
Pilots are reminded of the importance of listening to the radio and establishing any relevant traffic prior to entering a runway. |

Approx. 12 miles away the engine seized. The pilot trimmed the aircraft for best glide and made a successful landing at the airport. Further inspection revealed that the exhaust valve head had detached, shattering the #2 piston. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Whilst conducting circuit training with student doing touch and go's, another aircraft was heard inbound to track for a final on 10 from the southwest, which would be entering in a non-standard pattern. The aircraft conducting circuit's suffered a partial loss of power (assumed carby ice) aborted its take-off and radioed the intention to roll through to runway threshold of 28 but could not bring aircraft to a stop safely by Alpha 5 intersection and turned the aircraft around off to the far left side of the runway. This meant the aircraft was not clear of gable markers and then contacted the other aircraft to ask its intentions. The other aircraft was on short final and no response was provided. The aircraft landed, taxied off and gave a clear of all runways call.
Pilots are reminded of the importance of confirmation that a proposed runway is clear prior to landing and ensuring alerted see and avoid is maintained by responding to calls made in the circuit. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: An instructor and student were undertaking flight training in the Training Area adjacent to the Launceston CTZ. On climb at approx. 3500ft the instructor observed an indication of no oil pressure. The instructor immediately reduced engine revolutions and the engine then stopped. No abnormal sounds or other abnormal indications were present. A successful forced landing was carried out on a disused airstrip at Whitemore. |

A student on their second solo flight taxied back to hangar after two touch and go and a full stop landing, none of them heavy. As they approached the hangar, the right undercarriage leg slowly folded forward, the aircraft stopped and then settled on its right wing tip and tailplane. |

After landing, and while steering toward middle of runway the aircraft skidded on loose gravel and departed off the runway to the left. The nose wheel dug into the surface and the aircraft flipped over.
The pilot later reported a change in wind direction had occurred, resulting in a tailwind. |

OCCURRENCE DETAILS SUBMITTED TO RAAUS: Due to poor weather the pilot made a precautionary landing onto a private gliding strip. After waiting a period of days for the weather to improve, the pilot attempted to take off onto a runway which was only intended for landing. The aircraft was not configured correctly for a short field takeoff and the pilot did not abort the landing at their pre-determined abort point. The aircraft failed to get airborne in time and hit a stock fence at the end of the runway. The aircraft received substantial damage to wings, propellor, undercarriage and was trailered out. |
